Comment: The American Board for Certification in Orthotics, Prosthetics and

Pedorthics, Inc. (ABC) is conducting a Practice Analysis Survey of the Orthotic Fitter,

and the participation of all ABC Certified Orthotic Fitters is critical. The survey results will help define the certification, education and training of future fitters; in the development of valid and reliable examinations; and convey to others outside our disciplines the scope of services provided by O&P professionals.

It is critical that ABC obtain as many informed responses as possible in order to ensure the quality of the certification examination and to make sure it fully assesses the domains, tasks, knowledge and skills needed in the profession.

The survey will be emailed in the next few months. ABC encourages all of its Certified Orthotic Fitters to participate in this important survey. If ABC does not have your current email address, please send it as soon as possible to info@abcop.org.

Certified Orthotic Fitters are in a unique position to provide vital information about the practice of and contribute to the future of the profession by completing and returning the survey.

Response: The ABC Orthotics, Prosthetics and Pedorthics Scope of Practice was revised recently. Most of the revisions were in the Glossary section and pertain to supervision. Question: Has anyone had any success treating a Baxter's nerve entrapment/irritation with an FO? If so, what type of FO was used...any special modifications?

Question: Has anyone else had this happen yet?

We received four of the Prepayment Reviews from Medicare, we called the doctors and received the notes for the patients and sent them all back for review. Three went through fine and we received payment. The fourth is a lady from the Air Force Base Hospital in town; this one did not get approval because the base physicians do not follow Medicare guidelines and do not test their patients for neuropathy yearly, and there was no documentation for poor circulation or foot deformity that they circled on the CPF. Now we have to bill the patient, and this is a regular yearly customer, for the shoes and custom foot orthoses -- which means we have lost a patient because they won't be back next year, and I don't blame them. So, is my only option here to not accept patients from the base hospital just in case Medicare decides to do this again? Many of my patients are from this military hospital.

It sure is tough making a living in this profession. We did a cost analysis for custom molded diabetic foot orthoses made in our lab. Cost is about $70.00 a pair for materials, salaries, and time. This does not include fitting, adjustments and follow-up appointments. Aetna pays $32 a pair, the same amount they pay for A5512's. They don't cover our costs on shoes either. Who can afford to lose money for each Aetna patient? We can't and won't.

Is it worth it? I wonder.

Question: With the recently announced meeting between NCOPE, ABC, PFA, BOC and the Pedorthic Educators, it seems that "formal" pedorthic education (which we all know translates into "associates degree") is finally looming on the very near horizon. As one of the 30 people invited to this function, I heard from many of the organizational heads of state that they thought this was the way to go. So, what do you think? Is this a good idea? If so, are you willing to upgrade your skills to match (Anatomy/Physiology/Biology/Physics)? What is this going to do to the already slim supply of pedorthists in conjunction with all of the new patients entering the system through the new Healthcare setup? Can you afford to send your employee to school in another State for 2 years, and help pay their student loans off over the next 10 years? Good questions, and this is a great forum to let the organizations know what the men and women on the street think.

Women's Shoe Design Keeps Repeating Itself

Flats, platforms, wedges and stilettos. Sandals, slippers, boots and clogs. Craftsmen and haute designers have been tweaking women's footwear for centuries to reflect culture, politics and utility, but few have broken through with truly renegade reinventions. "There are adaptations, but actual world-changing innovation is a lot less common than we might want to believe," said Elizabeth Semmelhack, senior curator of the Bata Shoe Museum in Toronto.

The museum collects, exhibits and interprets footwear from around the world, with 13,000 examples of early designs and styles. Many are still referenced today, from thong sandals of the ancient East to towering chopines of Renaissance Europe. Full Text

2009 Footwear Sales Top $17.1 Billion

The National Sporting Goods Association (NSGA) announced that despite a 4% decline in units sold, dollar sales of athletic and sporting goods footwear reached $17.1 billion in 2009. NSGA's soon-to-be-released "Sporting Goods Market in 2010," reports this represents a 1% decline from the $17.2 billion in sales in 2008. In 2008, sales had declined 2% from the previous year. The two declines were the first since 1999 when sales fell 4%.Full Text

Commonly Seen Foot Deformities

This week, we examine deformities of the bone structure of the foot, placing emphasis on some of the contributing factors. Although many foot deformities can be heredity, in most instances they are the result of improper or poorly fitted footwear. Footwear can actually shape the foot to the extent where bones of the foot can shift direction to fit the shape of the shoe. In most cases, bone realignment requires surgery to correct or minimize the resulting damage. Among those commonly acquired deformities are: flatfeet, claw feet, hammer toes and bunions.

Flatfoot - also known as 'pes planus' is a normal flatfoot and seen as 'congenital' and is believed to be inherited. Normal flatfeet are functionally healthy and usually cause no discomfort, however they can sometimes cause shoe fitting problems because as the foot sits in the shoe it occupies more space in the mid-foot area of the shoe. This fitting presents more direct pressure on the shank area. However, the concern lies with that small percentage of persons having acquired flatfoot, also known as 'pes valgus'. An 'acquired flatfoot' presents as a severe breakdown of the arch. The foot becomes misshapen, with an extremely depressed arch, and a rolling in and down of the inner ankle. The result, severe pronation (outward rotation) of the heel, and a pronounced out swing of the forefoot. Full Text

DIABETES

Moderate Drinking Linked to Lower Diabetes Risk

Adults who have a drink or two per day may have a lower diabetes risk than teetotalers -- and the link does not appear to be explained by moderate drinkers' generally healthier lifestyle, a new study finds.

A number of studies have found an association between moderate drinking and a relatively lower risk of developing type 2 diabetes. However, whether that reflects a benefit of alcohol has been unclear. A central issue is the fact that, compared with both non-drinkers and heavy drinkers, moderate drinkers tend to have a generally healthier lifestyle.

In the new study, researchers found that among more than 35,000 Dutch adults followed for a decade, those who averaged a drink or two per day were 45 percent less likely than teetotalers to develop type 2 diabetes. Full Text

Burton & adidas Announce Streetwear Collaboration

adidas Originals and Burton announced that they are teaming up for a limited edition collection of men's apparel and footwear. The line will consist of 21 apparel pieces and seven footwear styles and will launch in Burton and adidas Originals stores this November. The pieces will feature both the adidas Trefoil and Burton logos and range from $40 to $350 for apparel and $90 to $200 for footwear.

Ben Pruess, vice president of adidas Originals and a former professional snowboarder, said he knew the adidas Originals customer was snowboarding and, although they wanted to continue their relationship with the company, had to change their apparel to fit that purpose. So as "brand managers," he said, "whether it's denim or technical mountain gear, we don't want to stretch too far. So we went to the experts to add authenticity." Full Text

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute.
