Question: Has anyone else had this happen yet?

 

We received four of the Prepayment Reviews from Medicare, we called the doctors and received the notes for the patients and sent them all back for review. Three went through fine and we received payment. The fourth is a lady from the Air Force Base Hospital in town; this one did not get approval because the base physicians do not follow Medicare guidelines and do not test their patients for neuropathy yearly, and there was no documentation for poor circulation or foot deformity that they circled on the CPF. Now we have to bill the patient, and this is a regular yearly customer, for the shoes and custom foot orthoses -- which means we have lost a patient because they won't be back next year, and I don't blame them. So, is my only option here to not accept patients from the base hospital just in case Medicare decides to do this again? Many of my patients are from this military hospital.  

 

It sure is tough making a living in this profession. We did a cost analysis for custom molded diabetic foot orthoses made in our lab. Cost is about $70.00 a pair for materials, salaries, and time. This does not include fitting, adjustments and follow-up appointments. Aetna pays $32 a pair, the same amount they pay for A5512's. They don't cover our costs on shoes either. Who can afford to lose money for each Aetna patient? We can't and won't.

 

Is it worth it? I wonder.

Question: I have a billing question: what ICD-9 code does everyone use when billing for a previous foot ulcer? This would not be the same as foot ulcer, as we are saying previous ulcer. Any feedback will be greatly appreciated.

Response: The best I can come up with is 250.8 Diabetes with other specified manifestations, they suggest using a code to identify manifestation, as: 707.10-707.9 any associated ulceration.

Comment: Would you like to do video gait analysis in your shop, but think it is just too expensive? Here is a recipe for an inexpensive "gait lab."  You will need a powered treadmill (Craigslist $75-$100), a small laptop ($249 Walmart), a Logitech Webcam ($20 Walmart), and a desktop tripod (yes, Walmart $10). Remove the base from the webcam and attach it to the tripod. Install the software on the desktop and plug in the webcam. Position the treadmill somewhere convenient. While the treadmill is running at its lowest speed, take a silver Sharpie and draw a line down the middle of the tread, like a line dividing a highway. This will help for visual reference. Position the webcam at this line. Using the software that came with the webcam, capture the patient's gait. Use Windows Media Player to play it back frame by frame. Finally, burn the video to a DVD and tell the patient/customer to pay the people up front. I usually charge $99 for a full functional exam (Pedigraph, ROM and strength testing) plus the gait analysis. The patient goes home with the pedigraph, DVD and copy of the 3-page Gait Analysis report (forms available for free at www.learnpedorthics.com, bottom of page where it says "Forms here").

Addition to Comment: I just wanted to add one more tool to last week's recipe for a DIY Gait Analysis system. We recently purchased a Craftsman Laser Level (Sears $35) and Tripod ($28) to check for horizontal and vertical alignment. The way we use it is by projecting the line vertically, aligning it with the gluteal cleft, and noting any deviation superior at the neck and inferior at the floor, between the feet near midline. This makes LLD or spinal issues really jump out, as there may be some sort of lateral deviation. Next, using the tripod, we adjust the levels horizontal beam to match the knee at the popliteal, hips and shoulders. Finally, this works great to assess inversion/eversion of the rearfoot posteriorly, using the vertical beam, while standing. Any variance is noted.  If not previously diagnosed, we refer them to the appropriate healthcare team member for diagnosis and care. Care should be taken when using any Laser, as it may cause eye damage.

Previous response: I am a little confused.  You give customers a 3-page "gait analysis" report based on video you do using webcam and treadmill? What concrete measurements is your analysis based on?  Or is it just you watching the video and giving your assessment?  Sounds like a good little system to "observe" someone walk (or run) and make some general observations.  But calling it gait analysis with a 3-page report is a bit of a stretch.

New Response: Mr. Torppey, I want to take a minute and clear up any "confusion" I may have caused you, or others about my post. This inexpensive "gait lab," as I referred to it in my original post, is only for doing observational gait analysis, one component of a comprehensive gait analysis. I would agree that it is only meant to "observe" someone walk or run, frame by frame, and record that data for observationally analytic and comparative purposes as a baseline. There is no force plate built into my Craigslist treadmill, and the only video analysis system is my well- trained, bifocal clad eyes. Yes, the results on my 3-page "observational gait analysis form" are subjective in nature, but translate into objective data through experienced observation. If you truly want "concrete data," I hear that some of the chain drug stores now have a gizmo that will analyze, measure and dispense foot orthoses like Snickers Bars, all based upon "concrete data." As for me, and I imagine many others, we can use a tool like this, along with our experienced eye, to do a pretty fair job, all without breaking the bank. Pedorthic Profession Convenes Education Conference 

 

The National Commission on Orthotic and Prosthetic Education (NCOPE) along with the American Board for Certification in Orthotics, Prosthetics and Pedorthics, Inc. (ABC), the Board of Certification/Accreditation International (BOC) and the Pedorthic Footwear Association (PFA) convened in Dallas on April 10 and 11 to critically assess the future of pedorthic practice and formal education. Also in attendance were educators from NCOPE approved pedorthic education programs. The 30 attendees were all members of the professional pedorthic community who provided a unique and important perspective on the future of the profession.

 

The meeting objective was to collectively identify the appropriate pedorthic education plan that will best teach the skills necessary for the delivery of quality pedorthic care in the coming decade. The group proposed a professional education model that NCOPE will now use as guidance to develop future educational standards for pedorthic education.

 

More information on NCOPE's pedorthic education standards will be available later in 2010.

NRF Survey: Retailers Eye Expansion, Technology in '10

 

Having survived 2009, retailers are cautiously optimistic about 2010 and will be focusing on store expansion and technology in the coming year, the National Retail Federation (NRF) Foundation's latest retail horizons report shows.

The report, "Retail Horizons: Benchmarks for 2009, Forecasts for 2010," is released annually by the NRF Foundation and KPMG and examines benchmarks for the retail industry, retailers' key priorities and plans, and tracks trends. It is based on a survey of more than 300 retailers.

San Antonio Podiatrist Introduces New Minimally Invasive Treatment for Heel Pain

 

Millions of Americans complain of heel pain, which is usually caused by plantar fasciitis. Plantar fasciitis is a condition in which the plantar fascia (ligament that runs from your heel to the toes) becomes inflamed. This condition can be very painful and cause a considerable amount of suffering.

 

According to podiatrists conditions such as stress fractures, tendonitis, arthritis and nerve irritation can also lead to heel pain. Podiatric physicians recommend conservative treatments before considering surgery to relieve heel pain. These include stretching, the use of reusable hot and cold pads, shoe modifications, orthotics, over-the-counter medications such as ibuprofen and weight control as excessive weight is often a trigger for heel pain.

 

If the condition persists surgical treatment may be suggested which involves cutting of the ligament on the heel bone. Open or endoscopic plantar fascia release surgery (EPF) is often used; complications such as increased pain, nerve injury, fascial rupture and infection can occur. However, a new minimally invasive treatment has arrived that may be more desirable for patients.

The new procedure is called TOPAZ and takes around 20 minutes for a treatment. The procedure involves a doctor using a series of small punctures in key areas of the heel. A wand is inserted through the skin using these small puncture incisions and radiofrequency energy is submitted for one-half second treatments. Full Text

"High Risk Diabetic Foot" Meeting Takes Place in India

This is exactly what the recently organized midterm Continuing Medical Education (CME) meeting on the 'High Risk Diabetic Foot' was about. Puma Gives the Boot to Cardboard Boxes

Puma is to eliminate the humble cardboard shoebox and plans to produce half of its sportswear from sustainable sources as part of a push to use ethical credentials to steal a march on rivals. Jochen Zeitz, chief executive, said Puma wanted to become the "most sustainable" sportswear company, but offered collaboration with other companies on its packaging technology, which includes the use of a corn starch-based product to replace plastic in wrapping clothing and in carrier bags in the group's stores.

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.

 

Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
