NEW QUESTIONS AND COMMENTS

Question: Has anyone had any success treating a Baxter's nerve entrapment/irritation with an FO?  If so, what type of FO was used...any special modifications?

Thanks!

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES

Comment: Would you like to do video gait analysis in your shop, but think it is just too expensive? Here is a recipe for an inexpensive "gait lab."  You will need a powered treadmill (Craigslist $75-$100), a small laptop ($249 Wal-Mart), a Logitech Webcam ($20 Wal-Mart), and a desktop tripod (yes, Wal-Mart $10). Remove the base from the webcam and attach it to the tripod. Install the software on the desktop and plug in the webcam. Position the treadmill somewhere convenient. While the treadmill is running at its lowest speed, take a silver Sharpie and draw a line down the middle of the tread, like a line dividing a highway. This will help for visual reference. Position the webcam at this line. Using the software that came with the webcam, capture the patient's gait. Use Windows Media Player to play it back frame by frame. Finally, burn the video to a DVD and tell the patient/customer to pay the people up front. I usually charge $99 for a full functional exam (Pedigraph, ROM and strength testing) plus the gait analysis. The patient goes home with the pedigraph, DVD and copy of the 3-page Gait Analysis report (forms available for free at www.learnpedorthics.com, bottom of page where it says "Forms here").

Addition to Comment: I just wanted to add one more tool to last week's recipe for a DIY Gait Analysis system. We recently purchased a Craftsman Laser Level (Sears $35) and Tripod ($28) to check for horizontal and vertical alignment. The way we use it is by projecting the line vertically, aligning it with the gluteal cleft, and noting any deviation superior at the neck and inferior at the floor, between the feet near midline. This makes LLD or spinal issues really jump out, as there may be some sort of lateral deviation. Next, using the tripod, we adjust the levels horizontal beam to match the knee at the popliteal, hips and shoulders. Finally, this works great to assess inversion/eversion of the rearfoot posteriorly, using the vertical beam, while standing. Any variance is noted.  If not previously diagnosed, we refer them to the appropriate healthcare team member for diagnosis and care. Care should be taken when using any Laser, as it may cause eye damage.

 - Rick E. Sevier BS Ed.

Response: I am a little confused.  You give customers a 3-page "gait analysis" report based on video you do using webcam and treadmill? What concrete measurements is your analysis based on?  Or is it just you watching the video and giving your assessment?  Sounds like a good little system to "observe" someone walk (or run) and make some general observations.  But calling it gait analysis with a 3-page report is a bit of a stretch.

Question: I have a billing question: what ICD-9 code does everyone use when billing for a previous foot ulcer? This would not be the same as foot ulcer, as we are saying previous ulcer. Any feedback will be greatly appreciated.

Women Risk Snapped Ligaments for Shoe Fashion

By Madison Park

Despite sprains, foot pains and blisters, towering footwear has legions of loyal female fans.

Among them are pop stars who dance intricate choreography wearing colossal shoes. Lady Gaga wore a pair of staggering 12-inch Alexander McQueen heels in her music video "Bad Romance." The theatrical shoes she wore were unveiled by the late British designer McQueen last fall, prompting some models to decline to walk the runway for him.

"We have entered a moment of heightened impracticality in footwear," said Elizabeth Semmelhack, author of "Heights of Fashion: A History of the Elevated Shoe." Heels tend to get higher during economic recessions, she added.  Full Text

Aetrex Plans to Put its First Retail Outlet in Englewood

By Joan Verdon

Larry Schwartz is ready to jump feet first into the future. Schwartz, 42, is chief executive officer of Aetrex Worldwide Inc. in Teaneck, a 64-year-old orthotics and footwear company his grandfather helped found. The company is very much a family operation. Brother Evan, 38, is president. Brother Matt, 35, is vice president-sales. "And I'm the old guy," says his father, Richard, chairman of the board.

Since its start as a New York-based orthotic inserts manufacturer, it has grown into an orthotics and shoe maker with more than $60 million in annual sales and 205 employees.

The company was a step ahead of the crowd in realizing the potential of the Internet in the 1990s and began creating Web sites for the retailers who sold its products. It locked in the Foot.com domain name in 2000. Two years later, it began supplying shoe stores with an electronic foot-scanning device called the iStep.

This year, the company is debuting the next generation iStep machine, the iStep Wave, which uses wireless technology to send foot information within a retail store or to a customer's cell phone or e-mail account. The company plans to open its first retail store - in Englewood in August. Full Text

Morris Kalinsky, a Recognized Name in Retail Footwear, Dies at 86

by Allyson Bird

A 25-year-old man with a rural upbringing and a chemistry degree showed up on King Street to sell shoes in 1950. Morris Kalinsky's father, who ran a successful department store in Holly Hill, urged his son to buy Bob Ellis Shoes, a run-down property with a made-up name. Kalinsky didn't draw a salary for seven years but, in the decades that followed, would become one of the most recognized names in retail footwear. Kalinsky, 86, died today. He is survived by his wife Sybil, two sons and three grandchildren. Full Text

India: Experts Warn Diabetes Patients to be Wary of Diabetic Foot

Experts who are convening for the mid-term continuing medical education (CME) on "High-risk diabetic foot" organized by the Association of Surgeons of India (ASI), UP Chapter and Indian Podiatry Association (IPA) in Kanpur on Sunday, 11 April 2010, are sounding an alarm bell for people with diabetes to be wary of diabetic foot.

"People with diabetes can, over time, develop nerve damage throughout the body. Some people with nerve damage have no symptoms. Others may have symptoms such as pain, tingling, or numbness - loss of feeling - in the hands, arms, feet, and legs" informed Professor (Dr) Rama Kant, who is the President of ASI, UP, and Head of the Department of Surgery, at CSM Medical University in Lucknow, India. Professor (Dr) Rama Kant will speak on "Surgical Approach to Diabetic Foot. How I do it" at this CME in Kanpur. Full Text

Diabetic Neuropathy: Preventing and Reversing the Damage

Imagine living with the haunting possibility that one day, you may lose all feeling in your feet and that this lost sensation could ultimately lead to ulceration, infection, and even amputation of your unsalvageable limbs. This grim but very real condition is called diabetic peripheral neuropathy (DPN), and according to the National Institute of Diabetes and Digestive and Kidney Disease 60-70 percent of diabetics suffer some kind of nerve damage.

That means as the number of type 2 diabetics continues to spiral out of control, we are facing a growing population suffering from pedal disasters like these. What's tragic is that with all we now know about diabetes, many of these conditions are preventable and largely reversible when they do occur. Full Text

Athletic Shoes Have Advanced Much Over the Years

By Bryan Malera

From cars to computers to mobile phones and even sneakers and sports equipment, everything in this era is better due to brilliant and amazing innovations in technology. Scientific engineering isn't only essential in making the electronic gadgets we use everyday, it is becoming imperative when designing athletic footwear and equipment.

In the early days of the National Basketball Association the shoe of choice was the Converse All-Star Chuck Taylor. The All-Stars do not compare to any of the newer shoes basketball players use now-a-day, don't even come close in comfort let alone design. With all the different brands, the wide variety of shoes that can help your game on the court is infinite. Full Text
