Comment: Would you like to do video gait analysis in your shop, but think it is just too expensive? Here is a recipe for an inexpensive "gait lab."  You will need a powered treadmill (Craigslist $75-$100), a small laptop ($249 Walmart), a Logitech Webcam ($20 Walmart), and a desktop tripod (yes, Walmart $10). Remove the base from the webcam and attach it to the tripod. Install the software on the desktop and plug in the webcam. Position the treadmill somewhere convenient. While the treadmill is running at its lowest speed, take a silver Sharpie and draw a line down the middle of the tread, like a line dividing a highway. This will help for visual reference. Position the webcam at this line. Using the software that came with the webcam, capture the patient's gait. Use Windows Media Player to play it back frame by frame. Finally, burn the video to a DVD and tell the patient/customer to pay the people up front. I usually charge $99 for a full functional exam (Pedigraph, ROM and strength testing) plus the gait analysis. The patient goes home with the pedigraph, DVD and copy of the 3-page Gait Analysis report (forms available for free at www.learnpedorthics.com, bottom of page where it says "Forms here").

Comment: From a recent article: "Toning shoes simulate the feeling of walking on sand and makes wearers stabilize their steps, leading to stronger leg, buttock, back and abdominal muscles, Skechers said on its Web site. A Reebok advertisement on its Web site said EasyTone can bring "better legs and a better butt with every step."

 

"The marketing of toning shoes has largely focused on women and exercise, neglecting the comfort and wellness attributes that will appeal more to men, Cohen said. Women had a 62 percent awareness rate of the footwear, compared with 37 percent for men, NPD said."

I have found this trend towards "toning" shoes a bit disturbing, particularly the Sketcher Shape-Ups. I fully understand the concept behind these shoes. They are nothing more than an extremely unstable, rocker sole shoe. Apparently the premise is that if you are unstable, you will recruit muscle groups into greater activity, toning the muscles in the process. I have seen a few patients in these shoes and they are incredibly unstable regarding inversion and eversion.

These shoes are counter to every orthopedic principle (except the rocker sole) that I have ever learned, read or applied in real world practice. The midsole under the hindfoot is the softest material I have ever seen used in a sole. It feels like a 20 durometer foam or EVA and it deforms easily under thumb pressure. I can only imagine how many cases of pf, sprains and post tib or peroneal tendon injuries these may cause. Marketing them to the "exercise" segment makes about as much sense as making a diabetic shoe with hidden compartments to hide candy bars and cigarettes.

I recently received an email catalog of the New Balance line that they will be introducing into this market. It actually used the term "designed instability" or something similar in their product info blurb. I can appreciate the rocker design for linear movements, limiting foot flexion and forefoot loads. BUT, using such a soft and unstable midsole in the hindfoot is just asking for trouble. Perhaps there is some genius to this that I am missing?

Response: I agree that "unstable" rocker sole shoes should only be worn by people with good control over their lower extremities. Even then, I recommend that "unstable" shoes be worn primarily on flat, level, hard surfaces. It's much easier to overcome the instability of Skechers and MBTs on level, hard ground. The problem is that we aren't always on level ground. Walking in the backyard, on side hills or on cobblestones can be very difficult in "unstable" shoes, regardless of what kind of shape you are in.

 

For people who really want the toning benefits of a rocker sole shoe, but have unstable feet, I recommend shoes in the "stable" side of the category. These shoes include Ryn, Finn Comfort "Finnamic" and Mephisto Sano. These shoes have exceptional-to-very-good medial/lateral stability, and provide much better stability. These brands are also very orthotic-friendly.

 

I have written an article and produced a short video, entitled "All About Rocker Sole Shoes" which talks about the similarities and differences between "unstable" and "stable" rocker sole shoes.

Response: We have been carrying MBT for the past four years and have yet to have an incident of sprained ankles or other injury.  I understand Brian's concern for how unstable some of the shoes can be.  I have one podiatrist who regularly sends patients to our store for the MBT's.  I always evaluate the customer's gait and also to determine where there is limited mobility or difficulty.  Some of the podiatrist's requests ended up with me placing a rocker bottom on a New Balance shoe.  We are always careful to not place those customers, especially the elderly, who have balance problems in the rocker bottom shoes.  We consider them 'exercise shoes' and tell our clientele that. 

 

I am a physical therapist with 40 years experience in evaluating gait and other lower extremity problems and I do have some reservations in fitting certain customers in the rocker bottom shoes; however, I have been surprised at how well some people function with the shoes.  I myself am an old polio patient - yes, there are still a few of us alive - and I have found that the shoes have allowed me to walk with more vigor and much less pain in DJD-filled knee joints.  I can refer to a number of customers who have had significant relief of various aches and pains.  Yes, this is anecdotal information, but it has some merit.  Is it worth the cost to customers to spend 2 1/2 times the money for the shoes as opposed to $100 or less for a modified shoe?  I guess the customers will answer that.  I do feel that some of the newer shoes are not as durable and effective as they should be.  We are bringing in some Ryn, Finn Comfort, and Clark rocker bottoms and they are more stable with anterior/posterior rockers only.  Hopefully this is helpful.

R.I. Merchants Dealt a Major Blow by Rising Waters

Devastating floods cascaded into hundreds of businesses across Rhode Island last week, swamping sales floors, ruining merchandise and closing one of the state's largest malls indefinitely. Flooding at the Warwick Mall shut all 70 stores in the million-square-foot facility for the first time in its 40-year history.

 

Water climbed up to 4 feet in some spots and the mall's parking lot was transformed into a lake so vast it swallowed cars and blocked the owner and retailers from getting into the building to assess the damage. "We have no idea what's even going on there," mall owner Aram Garabedian said late Wednesday. "It's a devastating thing."

 

Along Route 2 -- one of Rhode Island's most heavily trafficked thoroughfares -- and in strip malls across the state, murky water invaded supermarkets, lapped at kitchen equipment and inundated big-box stores, causing millions of dollars in damage and leaving employees fretting about their jobs.  Full Text

Flat, Flexible Shoes Best for Arthritic Knees

If you have knee pain from arthritis, wearing flat, flexible shoes like flip-flops may reduce the stress on your joints and keep you more comfortable, according to new research. In a comparison study that evaluated the force or 'load' on arthritic knees while wearing clogs, training shoes with stability features, flat walking shoes, flip flops and going barefoot, the flat and flexible shoes won, says study lead author Dr Najia Shakoor.

 

Her study is published online in Arthritis Care & Research.

 

''We know barefoot is good for your knee load from previous studies," Shakoor tells us. ''Then we thought, do different shoes have different effects on the knee?" So they compared the four shoe types with going barefoot. Full Text

Dougherty to Focus on Diabetic Foot Health

 

Poor blood flow, nerve damage and other conditions can turn minor foot problems into career-ending and sometimes life-threatening complications for diabetic patients. Those with the disease are prone to foot problems, which can develop without their knowledge. This is why the Dougherty County Health Department has chosen foot care for diabetics as the topic for its next monthly lecture.

 

"With a high incidence rate for diabetes, it's a big issue," said Alice Rodman, public health nurse specialist at the health department. Nerve damage, or neuropathy, is a common problem.

 

"It can be a serious issue," Rodman said. "With neuropathy, you don't have the ability to feel pain, heat or cold. You may notice numbness or tingling." Full Text

Arizona: Today is Diabetic Peripheral Neuropathy Awareness Day

 

The Arizona Legislature today recognized April 6 as Arizona Diabetic Peripheral Neuropathy (DPN) Awareness Day, mirroring the national month of foot health awareness established by the American Podiatric Medical Association. DPN is a serious condition that results from damage to nerves due to prolonged exposure to high amounts of glucose in the bloodstream as a result of diabetes. It often results in pain or numbness in the feet, but also manifests itself in intense pain often described as aching, tingling, burning and numbness, but because DPN damages nerve fibers, virtually any nerve in the body can be affected.

 

HCR2065 was spearheaded by Rep. Cloves C. Campbell, Jr. (D-Dist. 16) and Sen. Leah Landrum Taylor (D-Dist. 16). "This is an important awareness effort for all Arizonans," said Campbell. "DPN is a disabling condition that many suffer from in Arizona. Educating our citizens, especially those with diabetes, about seeing a doctor regularly is an important cause that I stand behind."

 

More than half of all diabetics suffer from DPN. In the U.S., African Americans are 1.6 times more likely to have diabetes than non-Hispanic whites, and Hispanic/Latino Americans are 1.8 times more likely to have diabetes than non-Hispanic whites. In Arizona, nearly 23 percent of Native Americans over the age of twenty have been diagnosed with noninsulin-dependent diabetes making this awareness day that much more important. Full Text

Long-term Marathon Running Linked with Increased Coronary Calcification

Long-term marathon training and racing might not be as good for the heart as some runners think, a new study suggests. Researchers have shown that long-term marathon runners, those who have completed at least 25 marathons over the past 25 years, have increased coronary calcium and calcified plaque volume.

"The last thing we want to see come out of this study is a suggestion that marathons are bad for you and that runners shouldn't be running them," lead investigator Dr Jonathan Schwartz (University of Colorado, Denver) told heartwire. "Running is healthy and part of a healthy lifestyle, but the data suggests that long-term marathon running, with the training that goes into them, might lead to increased levels of calcification across the lifetime." Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The course includes the required tests for competency to sit for the ABC or BOC exams.

 

Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC or BOC exam to earn their Pedorthic Certification (CPED or BOC Pedorthist).
