Question: I have a billing question: what ICD-9 code does everyone use when billing for a previous foot ulcer? This would not be the same as foot ulcer, as we are saying previous ulcer. Any feedback will be greatly appreciated.

Comment: From a recent article: "Toning shoes simulate the feeling of walking on sand and makes wearers stabilize their steps, leading to stronger leg, buttock, back and abdominal muscles, Skechers said on its Web site. A Reebok advertisement on its Web site said EasyTone can bring "better legs and a better butt with every step."

"The marketing of toning shoes has largely focused on women and exercise, neglecting the comfort and wellness attributes that will appeal more to men, Cohen said. Women had a 62 percent awareness rate of the footwear, compared with 37 percent for men, NPD said."

I have found this trend towards "toning" shoes a bit disturbing, particularly the Sketcher Shape-Ups. I fully understand the concept behind these shoes. They are nothing more than an extremely unstable, rocker sole shoe. Apparently the premise is that if you are unstable, you will recruit muscle groups into greater activity, toning the muscles in the process. I have seen a few patients in these shoes and they are incredibly unstable regarding inversion and eversion.

These shoes are counter to every orthopedic principle (except the rocker sole) that I have ever learned, read or applied in real world practice. The midsole under the hind foot is the softest material I have ever seen used in a sole. It feels like a 20 durometer foam or EVA and it deforms easily under thumb pressure. I can only imagine how many cases of pf, sprains and post tib or peroneal tendon injuries these may cause. Marketing them to the "exercise" segment makes about as much sense as making a diabetic shoe with hidden compartments to hide candy bars and cigarettes.

I recently received an email catalog of the New Balance line that they will be introducing into this market. It actually used the term "designed instability" or something similar in their product info blurb. I can appreciate the rocker design for linear movements, limiting foot flexion and forefoot loads. BUT, using such a soft and unstable midsole in the hind foot is just asking for trouble. Perhaps there is some genius to this that I am missing?

Question: I was wondering if anybody would know where we could find a used shoe finisher within the state of Washington or Oregon. We are looking for something basic with a 4"-5" wheel on one side and 1.5"-2" on the other. A used 5 in 1 or similar trimmer would be nice, too. Thanks for your help.

Record Number of Candidates Earn ABC Certification

The American Board for Certification in Orthotics, Prosthetics and Pedorthics, Inc. (ABC) announces that a record number of examination candidates chose ABC certification in 2009. ABC's professional credentialing program continues to be the gold standard for the orthotic, prosthetic and pedorthic professions, with more candidates sitting for ABC exams and maintaining their credential over the course of their career than any other credentialing body.

A total of 2,596 certification exams across all disciplines were administered in 2009, a 17% increase over 2008. A total of 2,278 examination candidates sat for the written, written simulation and technician exams, while 318 candidates sat for the Clinical Patient Management Exams. The overall passing rate for 2009 was 77%. Among those tested, ABC credentialed 1,392 practitioners, pedorthists, fitters and technicians, an increase of 56% over 2008. Experts Debate Dangers of Wearing Popular Sheepskin Boots

Despite the fact that Uggs and their many knockoff incarnations have long been considered a "fashion don't," lots of women love and continue to wear these suede and sheepskin boots all winter long. They're warm, they're cozy, they're easy to throw on with any outfit, they feel like slippers, and they're a celebrity favorite. Of course this makes the boots very appealing and popular, especially during chillier months. But are they actually good for your feet? Experts say no.

In a recent Daily Mail article, professionals spoke out about the health risks of wearing cheap, imitation Ugg boots. Dr. Ian Drysdale, the head of the British College of Osteopathic Medicine, said, "Because these boots are warm and soft, young girls think they are giving their feet a break. In fact, they are literally breaking their feet." Drysdale continued, "Their feet are slipping around inside. With each step, the force falls towards the inside of the foot and the feet splay. This flattens the arch and makes it drop. The result can be significant problems with the foot, the ankle, and ultimately, the hip." Full Text

The Walking Company Holdings, Inc. to Emerge From Bankruptcy in April

The Walking Company Holdings, Inc. has announced that the confirmation hearing to emerge from chapter 11 bankruptcy protection is scheduled for April 23, 2010. The Company expects their reorganization plan to be confirmed and the funding for reorganization to be closed in order to emerge from bankruptcy early in the week of April 26.

Working closely with its bank, landlords, vendors, and shareholders since filing in December of 2009, the Company has been able to restructure its balance sheet and long-term financial obligations. Retailers Finding Ways to Put Buying Power in Shoppers' Hands

Retailers and a wave of start-up companies are rushing to create smart-phone and iPhone applications that let shoppers compare prices, redeem coupons, lend a friend $25, buy movie tickets and make dinner reservations, and even find the nearest bathroom in the mall. Right now, most of what's happening in mobile retail is marketing and promotion, rather than sales and revenue-producing activities. Footwear Sales Lead E-Retail to Recovery

Sales of footwear over the web were up by 47 per cent in February 2010, compared to the same month one year before. The category - along with a 43 per cent increase in accessories sales online - helped the overall e-retail market grow by 13 per cent during the course of the year, according to the IMRG Capgemini e-retail Sales Index. Low Recurrence Rate of Diabetic Foot Ulcer After Nerve Decompression

Abstract

Background: This study reevaluates the previously reported subjective benefits of surgical nerve decompression in diabetes with an easily observable, fully objective outcome measure to eliminate the placebo effect and observer bias.

Methods: A retrospective review was conducted of a series of 75 feet in 65 patients with diabetes and previous neuropathic ulcers who had surgical decompressions of the peroneal and posteriortibial nerve branches at anatomical fibro-osseous tunnels. After a minimum of 12 months of follow-up, the incidence of ipsilateral ulcer was assessed.

Results: Postoperatively, four ulcer recurrences and four new-site ulcers developed in 187 patient-years. Mean follow-up was 2.49 years (range, 1-13 years). The combined linear annual risk of ipsilateral recurrence and new ulcer is 4.28%, the lowest reported in the scientific literature.

Conclusions: Surgical decompression of lower-extremity nerves of high-risk feet at fibro-osseous anatomical tunnels was followed by a low annual incidence of ulcer recurrence. This objective outcome measure suggests benefits of nerve decompression in diabetic neuropathy, as have previous reports using pain and sensory change as subjective measures. Unrecognized nerve entrapment may frequently coexist with diabetic sensori motor peripheral neuropathy in patients with diabetic foot ulcer. Leptin Therapy Shows Promise for Diabetes

UT Southwestern Medical Center researchers have found that using leptin alone in place of standard insulin therapy could abate symptoms of type 1 diabetes. Using mouse models, the researchers found that leptin administered instead of insulin showed better management of blood-sugar variability and lipogenesis, the conversion of simple sugars into fatty acids.

Leptin is a hormone produced by fat cells and involved in the regulation of body weight. Dr. Roger Unger, professor of internal medicine at UT Southwestern, led the study. Full Text

Pedorthic Pre-Certification Training Course July 26th - August 8th, 2010

In cooperation with the ABC and BOC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics.
