Comment: I have had several people ask about use of a thermometer in routine exams of their diabetic patients. The questions have been "Is this required?" and "How exactly is it done?"

 

Medicare DMEPOS Standards (Oct 2008) state that professional personnel (The Pedorthist) must "perform an in person diagnosis-specific functional clinical examination as related to the beneficiary's use and need of the orthoses/prostheses (e.g., sensory function, range of motion, joint stability, skin condition (integrity, color, and temperature), presence of edema and/or wounds, vascularity, pain, manual muscle testing, compliance, cognitive ability and medical history)." Thus use of the thermometer is a requirement for Medicare compliance.

 

I teach that the use of the thermometer should be done at two places on each foot: Plantar/medial arch and dorsum, over the midpoint of the third metatarsal. I use an IR thermometer with a focal point at the surface of the skin, but thermometers that focus on core/venous temps can be used as well. I look for high/low or differential temps. More than four degrees differential between feet may indicate circulatory issues and with a physician referral indicated. Other indications for a physician referral may include low temp unilateral (possible venous blockage), low temp bilateral (possible systemic dysvascularity) or high temp (possible infection). It is always better to be safe than sorry and your physicians will appreciate you acting as watchdog for them. Now, the ever-looming disclaimer...this use of thermometers is not meant to be diagnostic in nature. They are meant to allow us to gather "clinical findings" so as to refer our patients to the appropriate healthcare team member for proper diagnosis and treatment. Regards Ya'll!

Question: I get prescriptions from physicians i.e. "Diabetic shoes, DX 250.60." Now, neuropathy alone would not qualify the patient for therapeutic footwear/inserts through the Medicare shoe bill unless it is neuropathy with evidence of callus formation. The patient's feet also do not show evidence of calluses at time of 1st shoe fitting/evaluation.

 

I send out a "Statement of Certifying Physician for Therapeutic Shoes" to the physician and usually get them back with the field "Periph. neuropathy with evidence of callus formation" checked. Should I question this "condition" or is this considered enough "proof" of qualification (the physician signed it), that I can go ahead and dispense the footwear/inserts?

Response: This is an important time for a personal visit, introducing yourself and your services and qualifications. It is a time to educate physicians so that they can always make informed decisions, including all of the appropriate deformities that should be considered for therapeutic footwear. They should be shown the various options and criteria for choosing footwear for their patients. If you teach them well enough, they might send every patient directly to you and no one else.

Response: As part of the dispensing process, C. Peds should also engage in taking a patient history along with a pedograph on each foot. The pedograph would instantly show you plantar dark stains. These dark stains indicate the presence of bones. In turn, these bones would lead to callus formation. Disregarding a callus may lead to an amputation. If the patient gets regular pedicures and has meticulous footcare, you may not see a callus.

ABC, NCPA Establish Training Standards for Pharmacists Providing Therapeutic Shoes


The American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC) has worked with the National Community Pharmacy Association (NCPA) to establish training standards for pharmacists providing therapeutic shoes to Medicare beneficiaries. ABC recognizes that many different healthcare providers are engaged in delivering some aspect of DMEPOS items and services. Community pharmacies have a tradition of providing therapeutic shoes and inserts to their clients and are recognized by CMS as qualified providers. NCPA and ABC have collaborated to ensure that any ABC accredited pharmacy providing therapeutic shoes and inserts not only has the appropriate setting to deliver these services, but has a pharmacist who is specifically trained in the provision of therapeutic footwear.

 

ABC evaluated the extensive and comprehensive educational background of pharmacists in making this decision. NCPA believes that pharmacists know that they are not adequately trained to treat patients that have complex foot issues and understand that they must refer these patients to a Certified Pedorthist or Certified Orthotist for care.

 

The ABC board has determined that any pharmacist that wishes to provide therapeutic shoes and inserts must have proof of attendance at an NCOPE-approved therapeutic shoe fitter course in addition to the license to practice in their state. A manufacturer's representative in-service is not sufficient to meet this requirement.

 

"An important element of this collaboration will be in educating NCPA's members on the scope of practice and value of referring patients with complex issues to a Certified Pedorthist.  This will include basic information about the education, training and expertise of ABC Certified Pedorthists along with information on how to locate a C. Ped. in their area for referrals," says D. Deane Doty, Jr., CPO, and ABC President.

 

This collaborative approach will help ensure quality patient care and foster education of the pharmacists about the value of working with a C. Ped. as part of the diabetic healthcare team.  The ABC Board of Directors has authorized the provision of A5513's in pharmacies that qualify for therapeutic footwear facility accreditation. Licensed pharmacists must provide proof of completion of an NCOPE-approved therapeutic shoe fitter course in order to qualify. These facilities are eligible to only bill for non-custom therapeutic shoes (A5500), non-custom therapeutic inserts (A5512) and custom molded therapeutic inserts (A5513). The scope of practice for therapeutic shoe fitters remains unchanged.

Snowstorms Hurt Clothing Sales in February

 

Snowstorms that shut in shoppers last month also chilled sales at women's clothing stores compared with a year earlier, but most retail sectors saw gains in February, data released Wednesday show. The latest numbers from MasterCard Advisors' SpendingPulse show footwear, men's clothing, consumer electronics and luxury sales all rose, as did online sales.

The figures, which track transactions in all forms including cash, really just signal stabilization, however, analysts said, because February 2009 sales were abysmal and consumer confidence had hit an all-time low. Full Text

Would Centers Add to Quality Care

Three centers devoted to wound care will open at hospitals in the Buffalo area over the next few months, including one today at St. Joseph Hospital, a reflection of the growing sophistication of treating difficult wounds.

 

The units, managed in partnership with companies that specialize in chronic, nonhealing wounds, focus on such problems as foot ulcers in diabetics, pressure ulcers in the elderly and challenging wounds in trauma patients. Diabetes Status Tied to Infection Risk After Foot, Ankle Surgery

 

Patients undergoing foot and ankle surgery are more likely to have postoperative infections if they have complicated diabetes mellitus, while patients with uncomplicated diabetes do not appear to have a higher risk than patients without diabetes, according to research published in the Feb. 1 issue of the Journal of Bone & Joint Surgery.

 

Dane K. Wukich, M.D., of the University of Pittsburgh Medical Center, and colleagues reviewed charts for 1,000 orthopedic foot and ankle surgery patients to compare infection rates for patients with and without diabetes. Full Text

Turf Toe Ends Football Player's Career
