Question: Does someone have Pedigraph inking strategies that are easy and/or less messy than the ink drops on pads?  I find it inconvenient and will sometimes just not do them because of the messiness.

Response: I don't know whether or not pressure stats are as good a diagnostic tool as the inking would be, but I had a C. Ped. tell me that he used pressure stats from Foot Logic. They sell boxes of 100, and they are every easy to use!  You can order by phone with a credit card. Their phone # is 866-385-6442. I am only a CFts, but heard this from a C. Ped.

Response: I ink a stencil roller, which is able to stay wet in its cup enclosure, and is transportable.  One brand is Marsh.   It rolls smoothly and evenly over the mat.   Hand sanitizer can remove messes.

Question: Had a patient come in with a prescription for a pair of therapeutic shoes and custom made insoles. Patient has never been diagnosed with diabetes, but has neuropathy and a history of re-occurring plantar ulcers. Insurance coverage is Medicare and BCBS. I informed the patient that therapeutic footwear and inserts are only covered by Medicare if the patient is diagnosed with diabetes (+ additional conditions). Patient insisted he had gotten footwear/ inserts before at no cost to him. I told him I would not be able to bill Medicare and therefore he would have to pay for the prescribed items. Patient stated he would go back to where he got the footwear for free the last two years.

 

Today the patient came by again to show me the shoes (Drew) and custom inserts (cork base and bi-lam top cover with toe filler for 5th) that he, as he states, got at no cost to him and that the, as he calls it, "Prosthetic shop" billed Medicare for.

 

Am I missing something here? Is there an option for non-diabetic patients to get therapeutic footwear and custom insoles paid by Medicare?

Response: This is exactly why we are getting a bad name with Medicare. The Prosthetist and Orthotist can deliver diabetic shoes with no training; they don't know what is required by Medicare to even file the claim. The only saving grace is that when (and it will happen) they get audited by Medicare they will owe the money back to them. In the meantime we get the bad rap. I would love to know if they billed for an L-5000 - Partial foot, due to the 5th digit amputation.  If so they will owe Medicare even more! I came from the O&P side of the business and have seen it happen.

Response: The lab must have billed code L5000 because of toe filler.  If patient was only missing 5th toe this is fraud, but he probably did it.  You don't have to be diabetic to bill this code, only a forefoot amputation.

Response: I am only a CFts, but I do a lot of Medicare billing, and have to make sure all the documentation is in place. Medicare only pays for shoes/inserts for diabetics, and requires that the physician who treats a patient's diabetes must sign a Certifying Statement that says the patient had diabetes, which type, and that they are the treating physician for that diabetes. Also they must document one or more of the following: partial/complete amputation of foot, history of previous foot ulceration, history of pre-ulcerative callus, peripheral neuropathy, deformity, or poor circulation. There must also be an order for the shoes/inserts.

 

I have asked Medicare (Noridian is our carrier in Missouri) the question about coverage for other conditions, and was told that they NEVER pay for diabetic shoes/inserts for conditions other than diabetes!! Sounds like someone is committing fraud, because if they are getting paid, they must be billing as if the patient had diabetes!! If they ever get audited, they will be in trouble with no proper documentation!

They aren't manufacturing them anymore and I am now having a difficult time finding a manufacturer that has these for sale. It 3/4 length with a medial flange. Question: We have had a time with Medicare in the last few months. I was wondering if anyone else has had the same problems? In August 09 they changed a few things and if they bothered to tell anyone, I wouldn't know. Now we have to put Lt Rt modifiers along with the KX on A-5500 to let them know it is a pair of shoes. I am still unclear if this is 1 unit or 2. Has anyone heard of a GY modifier to let them know they are not diabetic? We have to get the denial from Medicare for the secondary to pay (sometimes they will), and they want a GY modifier. I would think an L code would tell them that. There I go thinking again! Medicare is sitting on thousands of dollars due to us because of their games. I know this time of year they are out of money so they shake things up and deny every thing they can. I get it; I am out of money, too! The Newswire is the perfect place to post these types of things.

 

On a positive side, J, what if we as a group came together with one basic idea: to advertise Pedorthics? When we do our local advertising, the word "Pedorthic" is prominent, or I see a dark background with "PEDORTHICS?" in white and that is all. Bumper stickers can be cheap and simple, billboards can be expensive, so somewhere in between, or all of the above, could work. This would draw people to search "Pedorthics" on the Internet just to see what it is and find out who we are. Locally we have a car dealer that has five-second commercial spots on TV; they are quick and to the point. I can see "PEDORTHICS?" in this type of format. Anyone out there like these ideas or have their own to share?

Question: I was wondering if anybody would know where we could find a used shoe finisher within the state of Washington or Oregon.  We are looking for something basic with a 4"-5" wheel on one side and 1.5"-2" on the other.  A used 5 in 1 or similar trimmer would be nice, too.  Thanks for your help.

AAFA Launches Industry Haiti Reconstruction Task Force

 

The American Apparel & Footwear Association (AAFA) has created the Haiti-AAFA Recovery and Reconstruction Team (HARRT), a member-led task force that will provide key apparel, footwear, and textile industry insight as relief operations continue and reconstruction planning begins in earthquake-stricken Haiti.

"As we support the relief work continuing on the ground in Haiti, it is important for the apparel and footwear industry to begin reconstruction activities," said AAFA President and CEO Kevin Burke. "Because apparel production is one of Haiti's most prominent industries, these sectors will play a critical role in Haiti's recovery. NASP Foot Care Clinic Sees Massive Turnout

ASSETERRE, St. Kitts - A one-week free clinic offered by the Canada-based North American School of Podology (NASP) may have begun as a routine practical coursework trip for students, but the quality education in foot care received has proven to be life changing for hundreds.

From day one, nationals and residents of the Federation came in by the dozens to the Football House to receive foot care that is uncommon on the island.

 

The procedure is more than a manicure but less than a chiropody treatment, and is important in identifying systemic diseases affecting the feet, skin disorders and diabetic foot syndromes.    

The clinic had 10 foot care stations set up on the third floor of the facility with two on the first floor to allow easier access for the elderly. About 90 persons were accommodated each day and their responses to the quality care provided were all favourable.

 

Marjorie Burroughs of Lower Cayon told SKNVibes that after having problems with cracked heels and diabetes, she decided to give the foot care a try. "It was a very good experience. They looked at my feet and massaged them and told me how to take care of them. I am a diabetic but I never really knew how to care for the feet. Now I know. I will definitely take better care of my feet," she said. Full Text

Perhaps Jim Caldwell should have suspected something was wrong sooner. He was overweight, after all, and a smoker, and had a family history of diabetes, yet he had never been diagnosed with the disease and rarely visited the doctor. So when he noticed a patch of dry skin on his foot, he thought nothing of picking at it until it bled.

 

"I should have realized, but I didn't," says the 61-year-old former sales office manager from Coatbridge. "My father was diabetic and my older brother was diabetic, but I never thought that it might run in the family.

"I was not one to go to the GP," he adds.  "I hadn't been for about ten years"

Where You Live, Not Just Lifestyle, May Contribute To Diabetes

A new study finds that black women in poor neighborhoods are at higher risk for diabetes regardless of how well-educated they are or how much money they earn.The research suggests that reducing rates of type 2 diabetes isn't just a matter of educating people about good diet and exercise habits. Where they live is also a factor, according to study co-author Dr. Yvette Cozier, of the Boston University School of Public Health.

 

"Neighborhoods that are predominantly black have far fewer full-service supermarkets, for example, offering fresh produce and lean cuts of meats ... walkable space outside, safe playgrounds for children and other things that promote physical activity," said Cozier, an assistant professor of epidemiology.
