Question: Does someone have Pedigraph inking strategies that are easy and/or less messy than the ink drops on pads?  I find it inconvenient and will sometimes just not do them because of the messiness.

Question: Had a patient come in with a prescription for a pair of therapeutic shoes and custom made insoles. Patient has never been diagnosed with diabetes, but has neuropathy and a history of re-occurring plantar ulcers. Insurance coverage is Medicare and BCBS. I informed the patient that therapeutic footwear and inserts are only covered by Medicare if the patient is diagnosed with diabetes (+ additional conditions). Patient insisted he had gotten footwear/ inserts before at no cost to him. I told him I would not be able to bill Medicare and therefore he would have to pay for the prescribed items. Patient stated he would go back to where he got the footwear for free the last two years.

 

Today the patient came by again to show me the shoes and custom inserts (cork base and bi-lam top cover with toe filler for 5th) that he, as he states, got at no cost to him and that the, as he calls it, "Prosthetic shop" billed Medicare for.

 

Am I missing something here? Is there an option for non-diabetic patients to get therapeutic footwear and custom insoles paid by Medicare?

Question: I have been using leather arch shells #275 from the Carl Sher Co. for 30 years. They aren't manufacturing them anymore and I am now having a difficult time finding a manufacturer that has these for sale. It 3/4 length with a medial flange. Question: We have had a time with Medicare in the last few months. I was wondering if anyone else has had the same problems? In August 09 they changed a few things and if they bothered to tell anyone, I wouldn't know. Now we have to put Lt Rt modifiers along with the KX on A-5500 to let them know it is a pair of shoes. I am still unclear if this is 1 unit or 2. Has anyone heard of a GY modifier to let them know they are not diabetic? We have to get the denial from Medicare for the secondary to pay (sometimes they will), and they want a GY modifier. I would think an L code would tell them that. There I go thinking again! Medicare is sitting on thousands of dollars due to us because of their games. I know this time of year they are out of money so they shake things up and deny every thing they can. I get it; I am out of money, too! The Newswire is the perfect place to post these types of things.

 

On a positive side, J, what if we as a group came together with one basic idea: to advertise Pedorthics? When we do our local advertising, the word "Pedorthic" is prominent, or I see a dark background with "PEDORTHICS?" in white and that is all. Bumper stickers can be cheap and simple, billboards can be expensive, so somewhere in between, or all of the above, could work. This would draw people to search "Pedorthics" on the Internet just to see what it is and find out who we are. Locally we have a car dealer that has five-second commercial spots on TV; they are quick and to the point. I can see "PEDORTHICS?" in this type of format. Anyone out there like these ideas or have their own to share?

Question: I was wondering if anybody would know where we could find a used shoe finisher within the state of Washington or Oregon.  We are looking for something basic with a 4"-5" wheel on one side and 1.5"-2" on the other.  A used 5 in 1 or similar trimmer would be nice, too.  Thanks for your help.

Average Price of U.S. Athletic Footwear Rises in 2009

Preliminary data from NSGA's "Sporting Goods Market in 2010" puts the average price point for athletic footwear in 2009 at $45.04, a 3.0% increase from 2008. In 2008, the average price had risen 1.6% to $43.71 versus $43.03 in 2007. On average, males spent more on athletic footwear than females in 2009, $46.39 versus $44.02. The previous year (2008) it was $45.16 versus $42.75.

"The increase in average price paid for athletic footwear indicates that retailers have not needed to offer deep promotional pricing in 2009," said NSGA Vice President of Information & Research Thomas B. Postoperative Infection Rates in Foot and Ankle Surgery: A Comparison of Patients with and without Diabetes Mellitus

Background Patients with diabetes mellitus may be at increased risk forinfection following foot and ankle surgery. This study aimedto determine whether patients with a diagnosis of diabetes mellitushave an increased rate of infection following foot and anklesurgery compared with a cohort of patients without diabetes.Furthermore, our study sought to demonstrate whether patientswith complicated diabetes are at greater risk of postoperativewound infection than are patients with uncomplicated diabetesor patients without diabetes.

 

Methods We conducted a retrospective review of the charts of 1000 patientswho had orthopaedic foot and ankle surgery. The following datawere extracted: patient age, sex, history of diabetes mellitus,development of postoperative infection, severity of infection,inpatient or outpatient surgery, use of internal or externalfixation, tobacco use, history of organ transplantation, historyof rheumatoid arthritis, length of surgery, follow-up time inweeks, and comorbid conditions. Take Care of Your Diabetic Foot

The concept of reconstructive and corrective surgery for the diabetic foot has evolved very recently, as a means to correct foot deformities. This latest technique is presently being practised in very few countries of the world. There are a few selected centres in the US, UK, Germany, Russia, Spain and Australia, with USA, which has 7 podiatric schools performing such operations, topping the list. In India, Amrita Institute of Medical Sciences, situated in Kochi, is the only institute doing this type of reconstructive surgery in high risk patients of totally deformed diabetic foot, thus obviating the traumatic need for amputations.

There is an entity called 'Charcot' in diabetic foot problems. In prolonged diabetic foot problems there is an increased blood flow to the lower limbs, due to autonomic neuropathy, which causes demineralisation of the bones. It is a tsunami like effect, just washing away the minerals of the bones. Moreover, it stimulates certain cells which simply eat up the bones. All this weakens the bones, and makes them soft and pudgy. This results in the collapse of the foot and ankle bones, making it impossible for the patient to walk. Moreover, multiple increased pressure points are developed on the soles of the affected feet, where ulcers and calluses develop. These act as portals for entry of bacteria. As the cellular immunity of the patient is suppressed, even the slightest bacterial infection can cause a serious life threatening problem, so much so that if the patient is not given proper treatment within 24 to 48 hours of the infection setting in, it may result in amputations or even death. Natalia Rivera-Espana ran about half a mile with blades of grass sliding between her toes and cold mud oozing under her feet. Then she made a beeline for her socks and shoes.

Barefoot running isn't for everyone.

 

"It's cold. I'm not enjoying it as much as I should be," Rivera-Espana, 26, explained as she tightened her laces.

 

The Santa Cruz resident was one of more than 20 runners who bared their soles at Skypark on Wednesday during a barefoot running clinic organized by the Santa Cruz Triathlon Association's Nu2Tri program. Like Rivera-Espana, most participants hadn't run barefoot since elementary school and were hesitant to doff their high-tech shoes. Full Text
