Question: We have a customer with a size 4.5 narrow foot looking for dress casual flat.  I am only able to find that size in an Aetrex shoe.  Does anyone know of other brands?

Question: I was wondering if anybody would know where we could find a used shoe finisher within the state of Washington or Oregon.  We are looking for something basic with a 4"-5" wheel on one side and 1.5"-2" on the other.  A used 5 in 1 or similar trimmer would be nice, too.  Thanks for your help.

Comment: Pedorthic Assessment of Skin Conditions of the Foot Using UV Light


A Wood's lamp is an assessment tool by which ultraviolet light (what us 70's generation folks would know as a "black light") is shone onto the skin of the patient. The clinician then observes any subsequent fluorescence. Different colors of fluorescence may indicate that different pathological conditions exist.  Now for those of you who are screaming that this is out of our scope of practice, let me say up front that, as Pedorthists, we do not diagnose, but we do assess and gather clinical findings. This is a very non-invasive method of data gathering, for the express purpose of determining a referral to the appropriate Allied Healthcare Team Member. With the mandatory disclaimer out of the way, we can proceed.

 

Normally skin will not shine, or fluoresce, under the ultraviolet light. The ultraviolet radiation itself is invisible to the human eye. However, different pathological conditions will give off certain chemicals that fluoresce or phosphoresce at different wavelengths producing different visible colors when view under UV light. Typically, blue, non-fluorescing skin is healthy skin, white spots are a thick, layer of dead skin cells (corns and calluses), light purple is dehydration, dark purple is sensitive, thin and dehydrated, brown spots indicate pigmentation and sun damage, orange is oily skin.

 

Different pathologies typically fluoresce as the different colors below:

 

Coral Red (Erythrasma - Bacterial skin infection)
Golden Yellow (Tinea Versicolor - Fungal skin infection)
Pale Green (Trichophyton Schoenleini - Fungal nail infection)
Bright Yellowgreen (Microsporum Audouini - Ringworm)
Aquagreen To Blue (Pseudomonas Aeruginosa - Hemmorage, Necrosis & Septicaemia)
Pink To Pink-orange (Porphyria Cutanea Tarda - Sensitivity to light)

 

The UV Light can be useful both in distinguishing fluorescent conditions from other conditions and in locating the precise boundaries of the condition. A typical exam is done with the patient seated in a dark room. The clinician turns on the UV light, holds it 4 to 5 inches from the area of skin being studied, and looks for any skin color changes. You should never look directly into the light as it may cause flashbacks to bell bottoms, Pink Floyd and your parent's dark basement filled with smoke.

Question: In my practice I'm seeing a lot of children. I would like to get more information on pediatric foot-related problems, in particular on how to measure and fit twister cables for tibial torsion. Are there any workshops or other information available on this topic or other pediatric issues, like courses or hands-on training?

Response: This is a response to the recent question posed by Moses Cohn, C.Ped. about obtaining information regarding "how to measure and fit twister cables for tibial torsion." The American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC) scope of practice of a credentialed Pedorthist (C.Ped.) does not include the provision of twister cable type orthoses. Question: Happy New Year to all!

 

We have had a time with Medicare in the last few months. I was wondering if anyone else has had the same problems? In August 09 they changed a few things and if they bothered to tell anyone, I wouldn't know. Now we have to put Lt Rt modifiers along with the KX on A-5500 to let them know it is a pair of shoes. I am still unclear if this is 1 unit or 2. Has anyone heard of a GY modifier to let them know they are not diabetic? We have to get the denial from Medicare for the secondary to pay (sometimes they will), and they want a GY modifier. I would think an L code would tell them that. There I go thinking again! Medicare is sitting on thousands of dollars due to us because of their games. I know this time of year they are out of money so they shake things up and deny every thing they can. I get it; I am out of money, too! The Newswire is the perfect place to post these type of things.

 

On a positive side, J, what if we as a group came together with one basic idea: to advertise Pedorthics? When we do our local advertising, the word "Pedorthic" is prominent, or I see a dark background with "PEDORTHICS?" in white and that is all. Bumper stickers can be cheap and simple, billboards can be expensive, so somewhere in between, or all of the above, could work. This would draw people to search "Pedorthics" on the Internet just to see what it is and find out who we are. Locally we have a car dealer that has five-second commercial spots on TV; they are quick and to the point. I can see "PEDORTHICS?" in this type of format. Anyone out there like these ideas or have their own to share?

Dangers of High Heels Reinforced

Recent medical studies have revealed that chickenpox vaccinations prevent chickenpox, that exercise improves the cardiovascular health of children and that adults gradually lose fitness with age.The latest entry in the "no-duh" derby: The wearing of high heels might make feet hurt.

 

The findings -- published in the November issue of Foot & Ankle International, the scientific journal of the American Orthopaedic Foot & Ankle Society -- confirm the "close association between heel height, plantar (sole) pressure and soft-tissue compressibility," said Dr. Chung-Li Wang, a co-author.

 

In the study, 21 test subjects took off their shoes and stood with their heels atop wooden blocks of various thicknesses. Detailed measurements found that higher heels put greater pressure on the area just behind the toes. Full Text

2,000 Diabetics in Pakistan Lose their Feet Every Year: Doctors

About 2,000 diabetics lose their feet in Pakistan due to gangrene and other complications annually; this was disclosed at an awareness-raising programme organised under the auspices of Al-Mustafa Welfare Society on Sunday.

Addressing the occasion, Dr Obaidullah Hashmi said the ratio of patients suffering from diabetes was increasing in Pakistan. As per a report by the World Health Organisation (WHO), two diabetic patients lose their legs every minute in the world, he added. Full Text

Foot Saving Gene Gel

 

Researchers are testing a new topical gel for non-healing wounds in patients with diabetic neuropathy, called Excellarate™. The gel is made from bovine collagen and is composed of a gene-activated biocompatible Matrix™. The gel is applied to the wound. Then, Scott Lipkin, D.P.M., Podiatrist/Researcher with Lehigh Valley Health Network in Allentown, PA, says cells migrate into the Matrix and take up an embedded carrier virus (adenovirus). The adenovirus has been genetically altered to stimulate the cells' release of platelet-derived growth factor-B (PDGF-B), a key component of wound healing. In turn, that sets off the healing process, stimulating the growth of new blood vessels and tissue.

 

In an early phase 1/ 2 study, 10 out of 15 patients (67 percent) had wound closure within 14 weeks of treatment with Excellarate. Seven of the ten had received just one application of the product. A phase 2b study of about 125 participants showed 48 percent of patients had complete wound closure within 12 weeks after a single application of Excellarate (compared to 31 percent wound closure for patients receiving standard care). The company is now in talks with the FDA to initiate Phase 3 trials with the wound-healing drug. Full Text

Buyers Optimistic at Agenda Show

Skate and athletic shoe buyers are hoping for better times this fall, as trends shift from vulcanized canvas to basic running silhouettes and colorways. At the Agenda show, held in January at the Hyatt Regency Huntington Beach in Huntington Beach, Calif., Sport Chalet buyer Robert Follett said the footwear business is on the upswing.

"I am a buyer, so I'm always optimistic," Follett said. "But I feel like the mood is lighter, and I feel good about things."

Follett added that the success of the toning category, led by Reebok's EasyTone and Skechers' Shape-ups styles, could reinvigorate the women's lifestyle category, which yielded lackluster results throughout 2009.

"We're here looking for exciting women's product," he said of the trade show. "Women's casual, lifestyle has been brutal. [Based on the success of toning products], there's a lot going on now in women's, so I'm looking forward to seeing what I can find." Full Text

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS.
