Question: In my practice I'm seeing a lot of children. I would like to get more information on pediatric foot-related problems, in particular on how to measure and fit twister cables for tibial torsion. Are there any workshops or other information available on this topic or other pediatric issues, like courses or hands-on training?

Comment: I have been in the field of Pedorthics since 1991 and have been Board Certified since 1993. After the last PFA Symposium I refer to myself as a BORED Certified Pedorthist.  The content of the symposium was disappointing to say the least. In my opinion, this has been the case for more than the past decade. Seriously, how many ways can you use a foam crush box, make a diabetic insole or grind a rocker sole? How can a presenter propose that they are an "expert" in biomechanics or that they have developed a new orthotic that is biomechanically superior, and in the same sentence say that a foam impression box was used?  Fore foot to rear foot relationships, first and lesser ray mal-alignments and fore foot equinus are obliterated when using a foam box! It also seems that to be a presenter you have to be part of the 'Good Old Boys Network.' They are the same presenters year after year after year after year.

 

I personally know of people whose presentations were turned down and their educational course is two days long, which is worth as many continuing education points as the entire symposium!

Response: To C. Peds Anderson, Powell, Sevier (who responded in previous issues):  While I can appreciate your desire to be mentally stimulated, I have to question if any of you took the time to add to your PFA symposium evaluation any of the specific topics you would like to hear more about?  Every year PFA gives you the opportunity to ask for and/or present the stimulating subjects you're requesting.  How should PFA reasonably respond to a lack of information on the evals and qualified individuals like yourselves who do not offer the present?

 

I know for a fact that Mr. Anderson's concerns were indeed addressed at the symposium by a member of the PFA Executive Committee.  Subsequently, PFA's personal attention to its membership was ignored, resulting in his complaint here. Let me reiterate, PFA is OURorganization.  WE make it better or worse.  Please, become part of the solution by making brilliant topic and speaker suggestions directly to PFA and not through the back door here.

Question: Happy New Year to all!

 

We have had a time with Medicare in the last few months. I was wondering if anyone else has had the same problems? In August 09 they changed a few things and if they bothered to tell anyone, I wouldn't know. Now we have to put Lt Rt modifiers along with the KX on A-5500 to let them know it is a pair of shoes. I am still unclear if this is 1 unit or 2. Has anyone heard of a GY modifier to let them know they are not diabetic? We have to get the denial from Medicare for the secondary to pay (sometimes they will), and they want a GY modifier. I would think an L code would tell them that. There I go thinking again! Medicare is sitting on thousands of dollars due to us because of their games. I know this time of year they are out of money so they shake things up and deny every thing they can. I get it; I am out of money, too! The Newswire is the perfect place to post these type of things.

 

On a positive side, what if we as a group came together with one basic idea: to advertise Pedorthics? When we do our local advertising, the word "Pedorthic" is prominent, or I see a dark background with "PEDORTHICS?" in white and that is all. Bumper stickers can be cheap and simple, billboards can be expensive, so somewhere in between, or all of the above, could work. This would draw people to search "Pedorthics" on the Internet just to see what it is and find out who we are. Locally we have a car dealer that has five-second commercial spots on TV; they are quick and to the point. I can see "PEDORTHICS?" in this type of format. Anyone out there like these ideas or have their own to share?

Soles4Souls Finding Rare Success in Distribution of Emergency Aid in Haiti

Physiological Footwear Sales Booming

New data by The NPD Group, Inc. a US-based market research company, has revealed that the physiological footwear category is booming. Physiological footwear's unstable sole construction simulates barefoot walking on soft, uneven natural ground, thus activating the small muscles throughout the body. According to NPD, the category's sales have increased four-fold during the 12 months ending November 2009 vs. the previous year. NPD has calculated the US market to be worth $76 million, in the four retail channels that the company tracks. Estimates by the trade magazine Footwear Insight value the worldwide market to $100 million. They don't make orthopedic shoes like they used to. But that's not altogether a bad thing, said Raymond Blasingame, owner of RAB's Active & Comfort Shoes, just across the parking lot from his other business, Leddy Brace & Shoe.

 

Shoemakers "just got into comfort" for everyday shows, said Blasingame, explaining that few companies continue to manufacture the rigid support shoes with the leather soles and separate heels. Blasingame can special order true orthopedic shoes, but his stock consists of shoes like the hard-to-find SAS brand that "are a lot more comfortable, and they do the same kind of thing" - provide support in the right spots. Diabetic Wound Care

 

Diabetics sometimes develop chronic wounds on their legs or feet that do not heal on their own. West Texan residents suffering from chronic wounds have a place to find healing at the Shannon Center for Wound Care and Hyperbaric Medicine.

 

Secondary to their diabetes, some diabetic patients may develop neurological and neuropathic changes that desensitize their legs and feet and make them prone to injury to those areas without them being aware that the injuries have even occurred. Peripheral neuropathy causes a numbing process where diabetics do not feel things in these extremities the way that most people would normally.

 

"If they step on something or if a shoe is rubbing their foot or hurting the foot, they may not even know that is happening until the end of the day or until several days later when they look at their feet," said Lynn Johnson-Dubbs, clinical coordinator, Shannon Center for Wound Care and Hyperbaric Medicine. Stem Cells Save Legs

 

A stem cell therapy for CLI is currently being evaluated by eight medical centers in the United States. For the procedure, a patient's limb is injected with stem cells from their bone marrow. The hope is the stem cells will generate new blood vessels in that area. Studies in India show 87.5 percent of 30 patients treated had their limbs saved. Girls' Knee Problems Are Often Foot-related

Hey wait a minute, not only girls, but everyone! Persistent knee pain, especially when it's not related to trauma or specific injury, is often aggravated by foot mechanics. Whether it's the youth athlete, baby boomers, or senior citizens, foot type (flat feet, high arches, etc.) affect the knees. By the way, since most foot types are inherited, it's not unusual to see generations with the same mechanical foot-related problems -- back problems included!

 

The startling amount of knee injuries in women and girls has puzzled doctors, therapists, and trainers for decades. Estimates are that females are five to seven times more likely to injure or tear the anterior cruciate ligament than their male counterparts. The ACL is one of the main knee stabilizers. High impact sports with lots of running, jumping, and direction changes like basketball, volleyball, soccer, or softball, are notorious for these injuries.
