Question: Happy New Year to all!


We have had a time with Medicare in the last few months. I was wondering if anyone else has had the same problems? In August 09 they changed a few things and if they bothered to tell anyone, I wouldn't know. Now we have to put Lt Rt modifiers along with the KX on A-5500 to let them know it is a pair of shoes. I am still unclear if this is 1 unit or 2. Has anyone heard of a GY modifier to let them know they are not diabetic? We have to get the denial from Medicare for the secondary to pay (sometimes they will), and they want a GY modifier. I would think an L code would tell them that. There I go thinking again! Medicare is sitting on thousands of dollars due to us because of their games. I know this time of year they are out of money so they shake things up and deny everything they can. I get it, I am out of money, too! The Newswire is the perfect place to post these types of things.


On a positive side, J, what if we as a group came together with one basic idea: to advertise Pedorthics? When we do our local advertising, the word "Pedorthic" is prominent, or I see a dark background with "PEDORTHICS?" in white and that is all. Bumper stickers can be cheap and simple, billboards can be expensive, so somewhere in between, or all of the above, could work. This would draw people to search "Pedorthics" on the Internet just to see what it is and find out who we are. Locally we have a car dealer that has five-second commercial spots on TV; they are quick and to the point. I can see "PEDORTHICS?" in this type of format. Anyone out there like these ideas or have their own to share?

Question: What experience have you had using OTC rocker soled shoes for athletic activities?

Response: There is considerable attention being given to this relatively new category of footwear. While rocker-sole shoes have been in existence for quite some time, MBT popularized this concept as "physiological footwear," which in turn has spawned a wide variety of offerings from manufacturers, including Skechers, RYN, Mephisto, Finn Comfort, Cogent, XSensible, Curves and likely several others. Each brand will have a different "feel," and just like cars, each model will "drive" differently based on their design and construction.

 

We use OTC rocker soles for active walkers that want to get more out of their morning and evening walks. The rocker sole "mutes" the impact forces on the heel and metatarsal heads, while promoting a smooth rolling gait. Calf muscles, hamstrings and gluteal muscles are noticeably challenged, especially when the wearer walks up hills. OTC rocker sole shoes are not for everyone, but they have been very well received in our store, with very few complaints or returns. We do not recommend OTC rocker shoes for running, unless the wearer is very aware of their running technique in the shoes. For walkers with unstable feet and ankles, we recommend brands with better medial/lateral stability, such as RYN or Finn Comfort.

 

Another athletic application of OTC rocker shoes is using them on elliptical trainers. I discovered this after experiencing knee pain on elliptical trainers in ordinary athletic shoes. The rocker sole "adapts" to the pitch of the pedals and places no pressure on my knees, allowing me to work as hard as I want without experiencing knee pain. Several of our customers have since remarked that they love using OTC rocker shoes on elliptical trainers.

 

OTC rocker shoes have been shown to be quite beneficial for customers with hallux rigidus, structural hallux limitus and can be beneficial for those suffering with plantar fasciitis pain. I also believe this category of footwear could be beneficial as a recovery shoe for distance runners.

Response: I presume you mean "shoes other than a running shoe" as running shoes are an OTC product and also a form of a rocker sole and are widely used. I have had no additional experience in using OTC rocker soles for athletic activity other than the following: I have used the Z-Coil shoe for running and it has a forefoot rocker. As for other shoes for running such as the MBT's and other versions, I have had no experience, but am worried about the stresses on the Achilles tendon. The Z-Coil has not presented any issues for me, but I have found these other shoes to be at a much more negative angle and thus I have been reluctant to try them in running in fear of injury. Other than this, in my experience with the exclusion of running, I have not had any experience in utilizing rockers for other athletic activities and would request the consultation of an exercise physiologist, physical therapist or other qualified health care practitioner before recommending them.

Question: As an administrative director and instructor of a Pedorthic Pre-Certification course, I have had some concerns regarding the current hours requirement for pedorthic certification. While I agree that pedorthists need experience, as a practical educational and employment-based issue, I disagree with ABC and BOC's current time-line requirements. I would propose that a prospective pedorthist be able to take a class, take a national certification exam, and, at that point, be awarded an apprentice status, similar to how the Canadian pedorthic board handles their certification. The candidate would then be awarded full pedorthic certification only after completing the required 1000 hours (which could possibly be started after the class, but before sitting for the national exam). From an employer's point of view, it would seem to make sense that a person would be hired with the assurance that this person would be certified after a certain documented period of time, instead of someone who may, for whatever reason, not be able to pass the national certification exam, or not decide to sit for the national certification exam.  From an educator's standpoint, I deal with many people who fear that they would not be able to remember details from a class they took six months ago.

 

I pose this query to pedorthic educators and the pedorthic community at large. What are your thoughts on the time-line for the current requirements for pedorthic certification?

Response: I am in total agreement with Dennis Janisse [whose response appeared in the previous issue] on advancing pedorthic education requirements. Now is no time for our profession to regress -- to do so would, in my opinion, pose a grave risk to its future.

 

I also agree with Rick Sevier regarding the freedom (some would say an obligation) to speak out when a practitioner sees a need for improvement.  There seems to be a tendency these days to shush those who don't agree with an issue by trotting out the weak argument that disagreement and civil discussion are markers for disunity and divisiveness. Freedom of expression, consensus decision-making and accountability are what I support. And, oh yes, respect for those who don't share my views.

Comment: I have been in the field of Pedorthics since 1991 and have been Board Certified since 1993. After the last PFA Symposium I refer to myself as a BORED Certified Pedorthist.  The content of the symposium was disappointing to say the least. In my opinion, this has been the case for more than the past decade. Seriously, how many ways can you use a foam crush box, make a diabetic insole or grind a rocker sole? How can a presenter propose that they are an "expert" in biomechanics or that they have developed a new orthotic that is biomechanically superior, and in the same sentence say that a foam impression box was used?  Fore foot to rear foot relationships, first and lesser ray mal-alignments and fore foot equinus are obliterated when using a foam box! It also seems that to be a presenter you have to be part of the 'Good Old Boys Network.' They are the same presenters year after year after year after year.

 

I personally know of people whose presentations were turned down and their educational course is two days long, which is worth as many continuing education points as the entire symposium!

On behalf of the Committee on Pedorthic Education (COPE) and the Pedorthic Footwear Association (PFA), I am writing to you in response to your recent comment in Pedorthic Newswire - Issue #343 on December 16, 2009. Let me start by saying that as PFA continues to be a primary provider of continuing education for the pedorthic profession, we most definitely are always looking for high quality presentation ideas, especially in the production of our ever-growing Annual Symposium & Exhibition. All of PFA's successful conferences have been the result of the collaboration of many, but it is the hard work and tireless efforts of COPE throughout the entire year planning the program's content, and ultimately supporting PFA and its mission to enhance the effectiveness and efficiency of the credentialed pedorthist through education. It is our belief that education is key in the continued elevation of the pedorthic profession. And once again, COPE was delighted in 2009 by the number of exceptional presentation proposals that were submitted for the Annual Symposium. As always, all of the submissions were reviewed by the members of COPE, which Mr. Anderson, I must point out is in fact comprised of many educators. Comment: The recent negative postings in the Pedorthic Newswire by a small number of pedorthists only accomplishes exactly what the insurance companies want...divide and Conquer. Posting negative comments on this website fuels the flames of prejudice and ignores the educational backgrounds of many talented professionals. It does nothing positive neither for the other pedorthists who read this List serve nor for your profession. It diminishes what others of your profession have accomplished over the last few years. The previous comments are also inaccurate and out of touch with reality. They serve only to point out those pedorthists' extremely prejudicial and myopic viewpoints.

As a practicing podiatrist, I have had the joy of working for the last 28+ years with many local pedorthists and orthotists who are true professionals.  The same is true for my work with many orthopedic, plastic and vascular surgeons. Each of us works together putting the patient first and our own personal accomplishments second. This has resulted in tremendous growth for all of us, with each of us now better equipped to handle those patients we choose to treat on our own. The spirit of cooperation has spilled over into most podiatrists being accepted into "main stream" medicine like never before. New Response: The freedom of speech does not allow one to yell, "Fire in a movie theatre," nor does it give one an inalienable right to throw out comments, which are irresponsible, false, inaccurate and potentially slanderous and liable without repercussions or rebuttal.  The constant stream of false accusations is not really accomplishing anything positive for your profession. I certainly don't expect the editor to deny one's right of free speech. I will also exercise the responsibility to ensure that statements which have little if any accuracy are appropriately corrected, particularly those which falsely denigrates my profession.


If you really want to grow your profession, stop looking outward and look inward. That is how podiatry got itself "mainstreamed" and it's the only way you will accomplish the same.  Your leadership understands this... but some of your membership still doesn't get it!

Nearly 3.6% Rise Recorded by Holiday Retail Sales

Christmas was not all that disappointing for retailers after all, at least according to data shared by MasterCard Advisors' SpendingPulse, which revealed that US retail sales hiked by an estimated 3.6% this holiday season as compared to last year, mainly on the back of online shopping and sales of electronics.

The spending estimate, like every year, has been for the period from November 01 to December 24, and automotive and gasoline sales have not been included.

 

A surge in sales during the week leading to Christmas helped year-on-year electronics sales hike by 6% since Black Friday on November 27, and 5.9% for the holiday season commencing on November 1.  Full Text

Minnesota Couple Give $1M for Clubfoot Treatment

 

A Minnesota couple has given $1 million to an association that trains doctors and nurses to treat clubfoot with a method developed by former University of Iowa surgeon Ignacio Ponseti. Ponseti developed a low-cost, non-surgical alternative to treat clubfoot, a genetic defect that leaves children unable to walk normally. He died Oct. 18 at age 95.

"Right now, we are not at a point where we can be self-sustaining, so this gift will allow us to achieve our fundamental goals of worldwide education of the Ponseti method," said Dr. Stuart Weinstein, an orthopedic surgeon who trained and worked with Ponseti from the early 1970s until Ponseti's death. "Even though we have a foundation, we don't have any funds."

The money will allow the Ponseti International Association to hire a full-time staff for fundraising and pay part of the cost of sending physicians to teach the method in dozens of countries. The association also hopes nurses, midwives and physical therapists can learn the method and take it back to places with limited access to doctors. Shoe Store Treads New Ground to Survive

Just as its customers' feet have grown and changed over the years and needed new shoes, a longtime Akron-area shoe store has had to adjust to survive. Lucky Shoes is in its third generation of owners, with a member of the fourth generation in the leadership group and ready to step in when needed.

 

But now, more than 50 percent of Lucky Shoes' business is in licensed stores for New Balance athletic shoes and not from its general shoe-store business, where the company got its footing. Lucky Shoes just opened its eighth New Balance store in Strongsville, has the exclusive license to open New Balance stores from Cleveland to Columbus and is in negotiations to open two more Northeast Ohio stores. Relieving Stress on Insulin-Producing Cells May Prevent Diabetes

 

Cells in your body are constantly churning out poisonous forms of oxygen (oxidants) and mopping them up with a countervailing force of proteins and chemicals (anti-oxidants). This balancing act of oxidative stress is particularly likely to go haywire in beta cells, the insulin-producing cells that malfunction and then start to die off in type 2 diabetes. Scientists at Joslin Diabetes Center now have found that a relatively little-studied enzyme plays a central role in defending beta cells against oxidants, but is damaged by the high levels ofblood glucose produced in diabetes.

 

Joslin Principal Investigator Robert Stanton, M.D., who led the research, says the discovery raises hopes of finding drugs that protect the enzyme, and thus the beta cells and their insulin production. Such drugs could help to stem the tide against type 2 diabetes, which now afflicts more than a quarter of a billion people worldwide. Full Text

Foot Amputations Ravage Aboriginal Diabetics

 

Thousands of aboriginal people with diabetes undergo unnecessary foot amputations because the federal government won't pay for them to have proper shoes, the head of a Manitoba medical outreach program said Tuesday.

Caroline Chartrand of the Diabetes Integration Project said diabetes patients in 33 First Nations communities in northern Manitoba get no medical funding for foot care. With aboriginal people being at high risk for diabetes, she said, it is essential that they receive foot inspections and special shoes to deal with circulatory, nerve and infection problems related to the disease.

"Ninety per cent of amputations are preventable," Chartrand said. "We need to make changes for foot-care policies because Health Canada doesn't cover footwear for our people." Full Text

Pelé Takes a New Shot After Failed Ventures

 

Yet another business venture involving soccer legend Pelé has gone belly-up. Earlier this year Pelé signed a deal to develop a soccer cleat with little-known Swiss boot manufacturer Nomis. But Nomis, which had a reputation for high-quality performance cleats, filed for bankruptcy in November before it could produce the shoes.

 

Now Pelé, who has owned a construction company that went bust and a sports-marketing firm that collapsed amid a financial scandal, has launched his own cleat and apparel-manufacturing venture-Pelé Sports AG-with a group of German investors. Full Text

Pedorthic Pre-Certification Training Course February 1st - 14th, 2010

 

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute.
