Question: As an administrative director and instructor of a
Pedorthic Pre-Certification course, I have had some concerns regarding the
current hours requirement for pedorthic certification. While I agree that
pedorthists need experience, as a practical educational and employment-based
issue, I disagree with ABC and BOC's current time-line requirements. I would
propose that a prospective pedorthist be able to take a class, take a national
certification exam, and, at that point, be awarded an apprentice status,
similar to how the Canadian pedorthic board handles their certification. The
candidate would then be awarded full pedorthic certification only after
completing the required 1000 hours (which could possibly be started after the
class, but before sitting for the national exam). From an employer's point of
view, it would seem to make sense that a person would be hired with the
assurance that this person would be certified after a certain documented period
of time, instead of someone who may, for whatever reason, not be able to pass
the national certification exam, or not decide to sit for the national
certification exam. From an educator's standpoint, I deal with many
people who fear that they would not be able to remember details from a class
they took six months ago.
I pose this query to pedorthic educators and the
pedorthic community at large. What are your thoughts on the time-line for the
current requirements for pedorthic certification?

Response: I
would like to clarify that the comparison to the Pedorthic Association of
Canada's pedorthic certification program is misleading. The applicant for the
Canadian Certified Pedorthist exam must have a pedorthics, health science,
science or engineering degree plus 3,500 hours of clinical experience in a
pedorthic related field (1,500 of that must be on-site supervised work) before
they can sit for the exam. If an individual has a degree in kinesiology, they
are eligible to take a diploma course in pedorthics that consists of online
courses plus three on-site clinical practicums for a total of 1,350 hours of
clinical experience.
The
most important thing to understand in discussing this issue is that the 1,000-hour
requirement is a critical part of the education process for someone desiring to
enter our profession. A three week course does not give anyone the clinical
experience to understand the scope of practice, much less recognize those who
need care beyond your scope. By hiring a pedorthic exam candidate and
helping them complete their 1,000 hours of clinical experience, employers are
helping to ensure that only qualified, educated and clinically trained
individuals are entering our profession. An added bonus of helping to train
future pedorthists is the ability to hire those individuals who show great
promise during their clinical training.
As
an educator and passionate advocate for the pedorthics profession, I would hate
to see us lessen our standards and requirements in any way. We require
only the bare minimum in education and clinical experience as it is; we should
be striving to elevate our profession and bring better educated, better trained
and more qualified individuals into pedorthics, not lower the standards and
risk providing inadequate and potentially harmful patient care.

Comment: The recent
negative postings in the Pedorthic Newswire by a small number of pedorthists
only accomplishes exactly what the insurance companies want...divide and Conquer.
Posting negative comments on this website fuels the flames of prejudice and
ignores the educational backgrounds of many talented professionals. It does
nothing positive neither for the other pedorthists who read this List serve nor
for your profession. It diminishes what others of your profession have
accomplished over the last few years. The previous comments are also inaccurate
and out of touch with reality. They serve only to point out those pedorthists'
extremely prejudicial and myopic viewpoints.
For
a more positive spin:
As
a practicing podiatrist, I have had the joy of working for the last 28+ years
with many local pedorthists and orthotists who are true professionals. The same is true for my work with many
orthopedic, plastic and vascular surgeons. Each of us works together putting
the patient first and our own personal accomplishments second. This has
resulted in tremendous growth for all of us, with each of us now better
equipped to handle those patients we choose to treat on our own. The spirit of
cooperation has spilled over into most podiatrists being accepted into "main
stream" medicine like never before. I might suggest the previous commentators
take a similar approach for the betterment of their own professional growth.

Comment: I have been in the
field of Pedorthics since 1991 and have been Board Certified since 1993. After
the last PFA Symposium I refer to myself as a BORED Certified Pedorthist.
The content of the symposium was disappointing to say the least. In my opinion,
this has been the case for more than the past decade. Seriously, how many ways
can you use a foam crush box, make a diabetic insole or grind a rocker sole?
How can a presenter propose that they are an "expert" in biomechanics or that
they have developed a new orthotic that is biomechanically superior, and in the
same sentence say that a foam impression box was used? Fore foot to rear
foot relationships, first and lesser ray mal-alignments and fore foot equinus
are obliterated when using a foam box! It also seems that to be a presenter you
have to be part of the 'Good Old Boys Network.' They are the same presenters
year after year after year after year.
I
personally know of people whose presentations were turned down and their
educational course is two days long, which is worth as many continuing
education points as the entire symposium!

Response: I've been certified since 2000, and I've attended 8 of the 9 symposia
since my certification, presenting 3 times.
I was compelled to speak first in 2001 based on the rocker sole study
results that are groundbreaking for our field.
I've spoken in 2004 with Dane LaFontsee, and spoke at the most recent,
presenting an overview of Lisfranc injuries.
The goal with my talk was to try and explore an injury that isn't
often discussed, and pull off of my background, which is working in an
orthopedic clinic side-by-side with the surgeon, and giving pedorthists
perspective on what the doctors face as well as what we do. Having been to many talks, various topics
will inevitably be presented, and I have been, at times, wondering why I was
there in that particular talk not learning anything new. So I sympathize, and I heard people grumbling
in the exhibit hall regarding the same thing.
That being said, I agree with Dr. Kesselman: this newswire can at
times be a springboard for negativity.
It's very easy to be negative, and even easier to not put one's name
down at the end of one's comments. But
from what I know, PFA will look at new presenters, newer topics, etc. And criticism can be more constructive,
especially when you are evaluating your presenters, because they do read those
(I've gotten copies of the comments). And in defense of the "Good Old Boys,"
you're talking about past presidents of PFA, Seymour Lefton winners, COPE
members, PFA board members, and PFA Medical Advisors, just to mention a
few. I figure if they're talking, then
what they have to talk about is valid.
FYI: I plan on submitting an abstract re-presenting the rocker research
to a very international audience in Orlando: attendance is optional.

Question: I just hired a young woman who is
showing tremendous potential. I intend to send her to C. Ped.
school so that we'll have another licensed practitioner on staff. However,
there is one issue I need help with. She does a great job in the lab
pouring and finishing plaster foot models, but she is having difficulty cutting
the heavy material we use for making accommodative inserts. Is anyone aware
of electrical shears that are heavy duty enough to cut high density
material? We tried a pair from Lowe's, but they just didn't "cut
it," if you'll pardon the pun. Any suggestions would be greatly appreciated.

Previous Response: Not supporting a
specific brand here, but a Landis 5 in 1 is a great tool that I've used for
years. It can skive, cut and trim material thicker than 6mm depending on
durometer. I assumed all pedorthic training facilities had these? I would
encourage you to contact the school you attended and demand that they purchase
a few.

great idea (regarding your response in the last issue); we do have a
Five-In-One in our school and I completely forgot how useful it can be
especially for those who do not desire to use a bandsaw or box cutter. Although
it too has it's risks of injury but with safety features intact it is a safe
tool and easy to use.

Prolongs Duties on Chinese, Vietnamese Footwear
The European Union on Tuesday
extended punitive taxes on imports of Chinese and Vietnamese shoes. Ministers
from the 27 EU nations "today adopted a regulation extending, by a further
period of 15 months, the anti-dumping duty on imports into the EU of footwear
with leather uppers originating in China and Vietnam," a statement from
the bloc's presidency said. The move is designed to help southern EU producers
compete against lower-cost footwear imported by companies such as Nike Inc.,
Puma and Adidas.
The extension of the duties on leather shoes is a compromise because a U.K.-led
group of northern European nations opposed re-imposing the levies for the usual
five-year period, according to Bloomberg. Full

Alexandre Birman has been awarded
the Vivian Infantino Emerging Talent Award at the Footwear News Achievement
Awards, held at the Four Seasons Restaurant in New York City in December.

American
Orthopaedic Foot & Ankle Society Offers Advice for Treatment of Heel Pain
Holiday shopping and holiday
feasting may make you a prime candidate for a case of plantar fasciitis.
Prolonged walking or standing and an increase in body weight are two leading
causes of plantar fasciitis, a painful overuse injury affecting the sole of the
foot, warns the American Orthopaedic Foot & Ankle Society (AOFAS).
According to the AOFAS, plantar
fasciitis typically starts gradually with mild heel pain. The pain classically
occurs with the first step in the morning. Treatment options include stretching
exercises, and modifying activities until the initial inflammation goes away.
Ice application to the sore area for 20 minutes three or four times a day may
also relieve symptoms. Your orthopaedic surgeon may also prescribe a
non-steroidal anti-inflammatory medication. But your best course of action is a
routine of home exercises to stretch the Achilles tendon and plantar fascia. Full Text

Diabetic
Shoes Bring Comfort, Save Lives

Would you believe it if someone told
you a pair of shoes could actually save a person from having a limb
amputated? According to Auburn Collins,
a representative with The National Diabetic Center, in some cases that is all
it takes.
"If all the people who needed
diabetic shoes had them," he said. "Then 80 percent of all amputations would
not take place in the United States... that's a powerful statistic."
What would you say if many people on Medicare and Medicaid were entitled to a
pair of diabetic shoes absolutely free? They are.
Collins, along with fellow
representative Bobby Singletary, travel around the Grand Strand and Pee Dee
measuring and ordering diabetic shoes for those who qualify. Last week the two
were in Hemingway, meeting with area residents at Williamsburg Apartments on
Baxley Road.
Many who have Medicare, Medicaid or
other insurance programs are entitled to one free pair of diabetic shoes a

Doctor's Word--Prevent Diabetes with Diet and Exercise

By all accounts, Frances Vasquez
ought to be a diabetic. Raised on a diet of fried steak, fried pork chops and
lots of rice, her father, mother, and two sisters suffered from the disease. At
age 47, Frances was overweight and already experiencing high blood sugar.
But over the past 11 years, Frances has been able to avoid diabetes, and her
sugars are now normal. By participating in a ground-breaking, government-funded
study, she learned how to make exercise and a healthy diet an integral part of
her life - and avoid the insulin injections and heart and kidney problems that
plagued her parents when they were in their 50's.
"At first it was hard, but I took it as a religious thing," she said recently.
"If I hadn't done this, I'd be a diabetic for sure."
The study, called the Diabetes Prevention Program (http://www.diabetes.niddk.nih.gov/dm/pubs/preventionprogram/
), included more than 3,000 adults, all of whom were overweight and had sugars
that were high but not yet in diabetic range. Diabetes is a condition in which
the body is unable to control blood sugar. It often leads to heart disease,
kidney failure, blindness, and circulation and other problems. About nine out
of 10 diabetics have what's called type 2 diabetes - their bodies make a
sugar-regulating substance called insulin, but their cells don't respond to it
adequately. Having a family history of diabetes and carrying too much weight
increase a person's risk of developing type 2 diabetes. In recent years,
diabetes has become an epidemic in the United States, mainly because many more

Is
Running Bad for Your Knees? Maybe Not

Perhaps because it
seems intuitively true, the notion persists that running, especially when done
long-term and over long distances, is bad for the joints. Indeed it would be
hard to think otherwise when, with each foot strike, a runner's knee withstands
a force equal to eight times his body weight - for a 150-lb. person, that's
about 1,200 lbs. of impact, step after step.
The common wisdom is that regular
running or vigorous sport-playing during youth subjects the joints to so much
wear and tear that it increases a person's risk of developing osteoarthritis
later in life. Studies have suggested that may be at least partly true: in one
study of about 5,000 women published in 1999, researchers found that women who
actively participated in heavy physical sports in their teenage years, or
weight-bearing activities in middle age, had a higher than average risk of
developing hip osteoarthritis by age 50. Full Text

Pedorthic Pre-Certification Training Course
February 1st - 14th, 2010
In cooperation with the ABC requirements, the
Eneslow Pedorthic Institute (EPI) is offering a three-level competency training
course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a
portion of the level one curriculum will be administered through a distance
learning program that includes required readings, and testing of all mandatory
topics. Upon passing the level one test, students will attend the remainder of
the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The
course includes the required tests for competency to sit for the ABC exam.

Pedorthic Pre-Certification Training Course

In cooperation with the ABC requirements, the Eneslow
Pedorthic Institute (EPI) is offering a three-level competency training
course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a
portion of the level one curriculum will be administered through a distance
learning program that includes required readings, and testing of all mandatory
topics. Upon passing the level one test, students will attend the remainder of
the pedorthic pre-certification course at the Eneslow Pedorthic Institute. The
course includes the required tests for competency to sit for the ABC exam.
