Question: As an administrative director and instructor of a Pedorthic Pre-Certification course, I have had some concerns regarding the current hours requirement for pedorthic certification. While I agree that pedorthists need experience, as a practical educational and employment-based issue, I disagree with ABC and BOC's current time-line requirements. I would propose that a prospective pedorthist be able to take a class, take a national certification exam, and, at that point, be awarded an apprentice status, similar to how the Canadian pedorthic board handles their certification. The candidate would then be awarded full pedorthic certification only after completing the required 1000 hours (which could possibly be started after the class, but before sitting for the national exam). From an employer's point of view, it would seem to make sense that a person would be hired with the assurance that this person would be certified after a certain documented period of time, instead of someone who may, for whatever reason, not be able to pass the national certification exam, or not decide to sit for the national certification exam.  From an educator's standpoint, I deal with many people who fear that they would not be able to remember details from a class they took six months ago.

 

I pose this query to pedorthic educators and the pedorthic community at large. What are your thoughts on the time-line for the current requirements for pedorthic certification?

The recent negative postings in the Pedorthic Newswire by a small number of pedorthists only accomplishes exactly what the insurance companies want...divide and Conquer. Posting negative comments on this website fuels the flames of prejudice and ignores the educational backgrounds of many talented professionals. It does nothing positive neither for the other pedorthists who read this List serve nor for your profession. It diminishes what others of your profession have accomplished over the last few years. The previous comments are also inaccurate and out of touch with reality. They serve only to point out those pedorthists' extremely prejudicial and myopic viewpoints.

 

For a more positive spin:

 

As a practicing podiatrist, I have had the joy of working for the last 28+ years with many local pedorthists and orthotists who are true professionals.  The same is true for my work with many orthopedic, plastic and vascular surgeons. Each of us works together putting the patient first and our own personal accomplishments second. This has resulted in tremendous growth for all of us, with each of us now better equipped to handle those patients we choose to treat on our own. The spirit of cooperation has spilled over into most podiatrists being accepted into "main stream" medicine like never before. I might suggest the previous commentators take a similar approach for the betterment of their own professional growth.

I have been in the field of Pedorthics since 1991 and have been Board Certified since 1993. After the last PFA Symposium I refer to myself as a BORED Certified Pedorthist.  The content of the symposium was disappointing to say the least. In my opinion, this has been the case for more than the past decade. Seriously, how many ways can you use a foam crush box, make a diabetic insole or grind a rocker sole? How can a presenter propose that they are an "expert" in biomechanics or that they have developed a new orthotic that is biomechanically superior, and in the same sentence say that a foam impression box was used?  Fore foot to rear foot relationships, first and lesser ray mal-alignments and fore foot equinus are obliterated when using a foam box! It also seems that to be a presenter you have to be part of the 'Good Old Boys Network.' They are the same presenters year after year after year after year.

 

I personally know of people whose presentations were turned down and their educational course is two days long, which is worth as many continuing education points as the entire symposium!

Response: I somewhat agree with this view point. I would think the Annual Symposium would be a great opportunity to present other aspects of pedorthics. It seems all my continuing education involves 'the neuropathic foot,' 'the diabetic foot,' 'plantar fasciitis,' and other basic foundations of pedorthics - foundations most of us already acquire.
 
Like he said, the speakers generally remain the same. There are many other C. Ped's I'd like to learn from but don't hear from.  I was encouraged to finally see a few female presenters, and I'm hoping this continues.

 

I would like to see more aspects of the field presented which are more sports specific. There are C. Ped's specialized in working with professional and amateur road cyclists, others with alpine skiers, others with ballet dancers and/or other athletes. There are C. Ped's who've gone to the Olympics. I'd like to hear some stories from them. I'd like to hear from C. Ped's who specialize in pediatrics.....something I never hear our industry talk about. There's just so much more to our field than diabetic feet and heel pain.

Response: Don't you realize that PFA is us, and we are them?  If we want OUR organization to prosper, we have to contribute more than standing in a corner pointing fingers.  I won't bore you with my list of contributions over the years.  Nor as an educator, do I apologize for contributing a well-received presentation this year. 

 

It's great that you took the time to be specific about what you would like to see changed.  Did you submit it to PFA?  No.  You've totally ignored the fact that change comes from within, not standing outside.  PFA must be doing something right, since there were enough submissions of abstracts that some folks had to be turned away this year; hopefully to resubmit for 2010.  FYI, PFA has worked on every point you raised, but there is still room for improvement.  Got a better system?  Send it to PFA.   Since this is still a democracy, not a biomechanical dictatorship, how about using your smarts to get involved to make OUR organization even better?

Question: I just hired a young woman who is showing tremendous potential. I intend to send her to C. Ped. school so that we'll have another licensed practitioner on staff. However, there is one issue I need help with. She does a great job in the lab pouring and finishing plaster foot models, but she is having difficulty cutting the heavy material we use for making accommodative inserts. Is anyone aware of electrical shears that are heavy duty enough to cut high density material? We tried a pair from Lowe's, but they just didn't "cut it," if you'll pardon the pun. Any suggestions would be greatly appreciated.

Response:Not supporting a specific brand here, but a Landis 5 in 1 is a great tool that I've used for years. It can skive, cut and trim material thicker than 6mm depending on durometer. I assumed all pedorthic training facilities had these? I would encourage you to contact the school you attended and demand that they purchase a few.

Response: High volume labs have the materials pre-cut by an employee responsible for organizing the inventory room. Otherwise, box cutters or a bandsaw is what works best versus scissors. Comment: Hello everyone,my name is Rodney Johnson and I have been Board Certified Pedorthist since 2006.  I started my own orthotic lab several years ago. I am really upset with what is happening to our field. Let me give you an example: Many people with different credentials are doing our job. MD's, DPM's, DC's, PT's, AT's and now even retail pharmacies. Many of these people are not trained in biomechanics or compensation, which can and do lead to foot ulcers. These people are equipped with the terms "pronation" and "supination" and they are stealing our jobs. Can these people explain the tri-planar motions of the STJ in regards to pronation and supination?

 

The previously mentioned professions are dispensing one or more of these products: Feet Orthotics, AZ braces, AFO's, Richie braces, DM shoes and insoles, custom shoes, etc. This is upsetting because you don't see a DPM, MD (Orthopedist) at Walgreens filling RX's for your monthly medications. Imagine this. You walk into Walgreens and you get your meds and then the same person asks you if you need a neuroma injection, or custom molded insole. Sounds strange to me.

 

We are required to be trained and it is our responsibility to be the best at what we do. We also have to spend large amounts of time and money on continuing education.  Does anybody have any ideas on how to compete with a retail giant like Walgreens or others like them, when they do not have to spend time and money or training or continuing education? The point I am trying to make is this: with so many people getting on the feet orthotic bandwagon, the market is becoming oversaturated and the title C. Ped. seems to mean less and less.

 

The other thing is this: what is PFA thinking (Current Pedorthics) saying it's a great idea for everyone to go to library system for "Custom Orthotics?" Example: there is a wholesale company that does "Custom Orthotics" with a 60-day money back guarantee for all of their accounts.  If they don't work, the patient brings them back to the office and the patient is issued a refund. HOW DO WE COMPETE WITH THAT? I guess I will just go to pharmacy school (HA-HA).

Response: I am in complete agreement with Shane Hayes' comments [responding in the last issue] on the diabetic Medicare shoe "gravy train."  The vast majority of Medicare-eligible diabetics who approach my practice are interested only in receiving "free" shoes, regardless of the possibility of poor outcomes.  I find this true, even amongst those who could otherwise afford better quality shoes and service.  I gently explain to my patients how the government, in typical fashion, has mucked up the diabetic shoe business and give them the choice of having their condition serviced and monitored by me or the clerk at the local drug store.  You pays your money and you takes your choice.

 

Shane is right; our scope of practice is far, far larger than dispensing diabetic shoes.  Quit whining and get along with your practice of pedorthics!

Illinois Governor Quinn Signs Law Improving Orthotic, Prosthetic Insurance Coverage

 

Governor Pat Quinn has signed a bill into law that will benefit thousands of orthotic and prosthetic users in Illinois covered by private health insurance plans. The law ensures that coverage for orthotic and prosthetic devices is the same as nearly all medical or surgical benefits. "Those covered by orthotic and prosthetic insurance policies will get stronger and better coverage that is in line with other medical and surgical insurance benefits," said Governor Quinn. "This new law should provide a greater degree of financial protection and security to those who depend upon these important devices and to their families."

 

In addition, the new law will give more orthotic and prosthetic users access to new, technologically-advanced and well-fitting devices. There are over 69,000 people in Illinois living with limb loss and a comparable number of people living with disabling diseases such as Spina Bifida, Cerebral Palsy and Muscular Dystrophy.

 

Governor Quinn signed into law HB 2652, which was sponsored by Senator Antonio Munoz (D-Chicago) and Representative Kevin Joyce (D-Worth). The law goes into effect June 1, 2010. Full Text

Dack's Shoes, one of Canada's oldest and best-known retailers of quality men's footwear, has slipped into bankruptcy after 175 years in business.  The company announced last week that it filed for bankruptcy Dec. 9 and plans to close all six of its remaining stores - four in Toronto and two in Alberta - after a chain-wide liquidation of merchandise.

Diabetic Foot Ulcers Linked to Early Death

Among diabetics, foot care is very important to prevent the development of foot ulcers as they have been linked to a higher risk of death.

Over time, uncontrolled diabetes can damage the blood vessels and nerves. Poor circulation and nerve damage in the feet makes people vulnerable to sustaining cuts or other injuries that may go unnoticed and progress into poorly healing ulcers, or sores. Severe cases can ultimately lead to complications requiring amputation.

Researchers studied 65,126 Norwegian adults - of which 1,339 were diabetic having no history of foot ulcers, while 155 had a history of the complication. Compared with other diabetic adults, those with a history of foot ulcers were 47 percent more likely to die during the 10 years of the study period. Full Text

Cell Transplant Saves Soldier From Diabetes

University of Miami doctors spared wounded airman Tre Porfirio a lifetime of severe diabetes with a first-of-its-kind emergency cell transplant from his own bullet-riddled kidney. Porfirio had been shot in the back in Afghanistan, forcing Walter Reed's doctors to remove much of his stomach and intestines. They had planned to rebuild his abdominal structure, until they realized that his pancreas was completely ruined, and without it he'd face crippling, life-threatening diabetes, reports the Miami Herald.

 

So in an apparent first, they flew the pancreas to the University of Miami, where diabetes researchers salvaged its remaining insulin-producing cells. It then shipped them back to Walter Reed where they were implanted in Porfirio's liver. There, they've already begun producing insulin again. Full Text

Pedorthic Pre-Certification Training Course February 1st - 14th, 2010
