Question: Hello everyone,my name is Rodney Johnson and I have been Board Certified Pedorthist since 2006.  I started my own orthotic lab several years ago. I am really upset with what is happening to our field. Let me give you an example: Many people with different credentials are doing our job. MD's, DPM's, DC's, PT's, AT's and now even retail pharmacies. Many of these people are not trained in biomechanics or compensation, which can and do lead to foot ulcers. These people are equipped with the terms "pronation" and "supination" and they are stealing our jobs. Can these people explain the tri-planar motions of the STJ in regards to pronation and supination?

The previously mentioned professions are dispensing one or more of these products: Feet Orthotics, AZ braces, AFO's, Richie braces, DM shoes and insoles, custom shoes, etc. This is upsetting because you don't see a DPM, MD (Orthopedist) at Walgreens filling RX's for your monthly medications. Imagine this. You walk into Walgreens and you get your meds and then the same person asks you if you need a neuroma injection, or custom molded insole. Sounds strange to me.

We are required to be trained and it is our responsibility to be the best at what we do. We also have to spend large amounts of time and money on continuing education.  Does anybody have any ideas on how to compete with a retail giant like Walgreens or others like them, when they do not have to spend time and money or training or continuing education? The point I am trying to make is this: with so many people getting on the feet orthotic bandwagon, the market is becoming oversaturated and the title C. Ped. seems to mean less and less.

The other thing is this: what is PFA thinking (Current Pedorthics) saying it's a great idea for everyone to go to library system for "Custom Orthotics?" Example: there is a wholesale company that does "Custom Orthotics" with a 60-day money back guarantee for all of their accounts.  If they don't work, the patient brings them back to the office and the patient is issued a refund. HOW DO WE COMPETE WITH THAT? I guess I will just go to pharmacy school (HA-HA).

Comment: Pharmacies/Walgreens, the list goes on regarding large and tiny facilities that have no fitting rooms/no equipment etc. Look at the October 2008 DMEPOS Quality Standards; they are specific in black and white under appendix C:
 
Appendix C: Custom Fabricated and Custom Fitted Orthoses, Prosthetic Devices, External Breast Prostheses, Therapeutic Shoes and Inserts, and their Accessories and Supplies; Custom-Made Somatic, Ocular and Facial Prostheses
 
The supplier shall be trained in a broad range of treatment options to ensure that the item(s) prescribed is/are optimal for the beneficiary's condition. The provision of custom fabricated or custom fitted devices (i.e., other than off-the-shelf items) requires access to a facility with the equipment necessary to fulfill the supplier's responsibility to provide follow-up treatment, including modification, adjustment, maintenance and repair of the item(s). Individuals supplying the item(s) set out in this appendix must possess specialized education, training, and experience in fitting, and certification and/or licensing.

It's clear that these facilities do not have the necessary equipment to follow up, modify, adjust, provide maintenance, and repair a diabetic shoe and or custom insert. Hate to say how many times the individuals have provided footwear with so-called custom foot orthotics from these facilities and are sent to my office for repairs/maintenance and, above all else, to alleviate pre-ulcerative calluses that were not cared for properly. Now they have taken all available funds for the year for the diabetic shoe program and we have to fix the problem.  
How much clearer can what is required to provide these services be written by the government? Print the Appendix out and provide to your local Pharmacist in charge of the pharmacy. This has worked very well, as they realize whose certification/license is responsible and being used at that location.

Question: Now that OTC rocker sole shoes are commercially available, what disorders are they being prescribed for, and how has this affected your practice?

Response:
Two astounding cases in our practice:  Cerebral Palsy patient, female age 60 with muscle spasticity who has had toe shuffling gait her entire life was able to stand straight with near normal gait.  She wears with custom orthoses.

2nd case:  6'4" 62 year old male with hx of back fx's entered our facility with 2 canes, bent forward posture.  Put MBT's on him and he stood straight and walked without canes immediately.  He does require custom orthoses for stability medially, laterally.  The new stability rocker soles should be even better for him.

This footwear category is exciting in possibilities for enhancing patient wellbeing.  More widths and stability being available in newly developed products will aid fitting challenges.

Response: Here in San Francisco, we just had a DPM for the local podiatry school do a guest lecture program on KGO radio.  In it she denigrated rocker sole shoes, saying that these shoes are mostly bad for people and are widely over dispensed.  She went on to say that she has seen many people with problems from these shoes.  Most of the DPM's in my area are loathe to prescribe them, especially because they do not come in multiple widths, and even more so because they do not work well with hard rigid plastic orthotics.  When we do get people in from a doctor it is usually because the patient asked the doctor and that doctor usually says, "I don't know, but give them a try and see."

Black Friday Yields Mixed Results

Nearly 200 million people shopped in stores and online this past holiday weekend, but total spending was flat compared to the same timeframe last year, according to a survey released yesterday by the National Retail Federation (NRF). On average, consumers spent 8.5% less per person this year on Black Friday weekend, a product of deep discounts and fewer purchases of electronics. "Shopper traffic well surpassed our expectations," said Scott Krugman, NRF vice president. "We still have double-digit unemployment, we still have trouble in the housing market and consumers are still very cautious. Retailers are going to have their work cut out for them in terms of keeping the momentum going." Full Text

ComScore: Cyber Monday Sales Increase 5%

ComScore reported that online retailers racked up $887 million in sales on Cyber Monday, up 5% from last year. The amount is the highest single-day online spending on record after Dec. 9, 2008, when consumers splurged the exact same amount.

For the holiday season-to-date, $12.26 billion has been spent online, marking a 3% increase versus the corresponding days last year. Cyber Monday's sales uptick came after a 10% jump on Thanksgiving day, followed by an 11% increase on Black Friday last week. Full Text

WSA Shifts Summer Show Dates

The 2010 summer trade show calendar continues to shift as show management looks to minimize or eliminate overlap. The WSA Show is the latest to change their August show dates to avoid overlapping with the FFANY show.  WSA will now be held Aug. 9-11 at the Sands Expo and The Venetian in Las Vegas, while FFANY will be held Aug. 3-5 in New York and Magic will be Aug. 17-19 in Las Vegas. Donald Deane Doty, Jr., CPO Named ABC President 

Donald Deane Doty, Jr., CPO, assumed the role of President of the American Board of Certification in Orthotics, Prosthetics and Pedorthics, Inc. (ABC) board of directors on December 1.  As part of ABC's annual leadership transition, Doty takes over the presidency from Robert S. Lin, CPO, FAAOP of Wethersfield, Ct. Lin will continue to serve ABC as immediate past president of the board.

"I have complete confidence that with Cathy Carter at the helm and Deane Doty assuming the presidency, our tradition of excellence will continue," said Lin. "Deane brings a wealth of exam, committee and board experience in the role and his commitment to the ABC mission is evidenced by over 20 years of dedicated service."

Weymouth Hotelier Fears He May Lose Foot

A hotelier faces having his foot amputated after an allegation of misdiagnosis at Dorset County Hospital. David Knight, from Weymouth, is now seeking compensation from the crisis-hit Dorchester hospital. He claims that he now faces an uncertain future following the failure to diagnose a condition known as Charcot foot.

 

He claims that when he first attended the hospital there did not appear to be enough staff to cope and that hygiene was well below standard.  Mr Knight, 57, is now reliant on crutches to get around and is likely to have to undergo further surgery and possibly amputation of his right foot. He claims that if the condition had been diagnosed correctly he would not have suffered the health problems and possible surgery he now faces.

 

Mr Knight, said: "I only wish that the treatment I received in 2007 was as good as I have been getting recently in 2009 from the Diabetic Clinic. Full Text

Huge Increases Projected in Numbers of People with Diabetes

According to HealthDayNews, the number of people with diabetes in the United States is expected to double over the next 25 years, a new study predicts bringing the new total to about 44 million by 2034 from the 23.7 million currently.  Alarmingly, at the same time the cost to treat will likely triple rising from $113 billion in 2009 to $336 billion in 2034.

The researchers indicated that a factor in the escalating costs is the number of people living with diabetes for lengthy periods. Over extended periods the costs increase because of the need of dealing with significant complications such as end-stage renal disease, which requires dialysis or kidney transplants, visual impairments, and other physically challenging conditions. Funky-looking, rocker-soled shoes that manufacturers claim will help you burn calories and tone muscles are turning up everywhere, including several major retailers. Many online outlets carry a version of the shoe, and Avon is selling a low-cost version for Curves.

 

With the bottomless sea of fitness equipment on the market, skepticism is a standard reaction to new exercise aids.  Darleen Mostellar, mother of two, a local runner, founding member of the Pocono Area Running Club and 17-time marathon finisher, said, "I sell Avon and they sell the Curves rocker shoes, so I ordered them. The first time I wore them I thought, 'No way, they're going back!' They were just so different. I'm used to a running shoe - the stability, the balance."

 

But she gave it another shot. "The next day I tried again. I put them on and walked two miles. When I woke up the following morning, my legs hurt - that good pain you feel after a good workout. I had to think about what I'd done differently. Then it hit me; the shoes," said Mostellar who lives in Stroudsburg. "Even when you are standing still, you are still working your muscles just to stabilize yourself. So, I went out and got the sandals. I use them for cross-training on days when I don't run."

Pedorthic Pre-Certification Training Course February 1st - 14th, 2010

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. Pedorthic Pre-Certification Training Course February 1st - 14th, 2010

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC exam to earn their Pedorthic Certification (CPED Pedorthist).
