Comment:
  I just visited my local Walgreens in Weston, Florida and saw that they have a big Doctor Comfort Display.  The Walgreen's store saleswoman told me that Walgreens had just partnered with Dr. Comfort and that they accepted Medicare if I had diabetes.  However, when I asked her if the store employed a Pedorthist or Podiatrist, she had no idea what I was talking about.

Response: Walgreens? Really? How can they do that? The Dr Comfort company knows better. Are the Medicare laws different in Florida? I don't think so. Seems to me that this particular shoe company needs to be watched. We are strong as a group, I wouldn't think they would want to upset us!

Response: As a facility surveyor I have seen many facilities (mostly pharmacies) providing diabetic footwear from Dr. Comfort with nothing but a certificate from Dr. Comfort.  The facilities were given the impression that the certificate is all that was needed to provide this service.  I cannot say that Dr. Comfort was responsible for the miscommunication.  Each of these facilities will be visited by a surveyor if they haven't been surveyed already, and they are informed of Medicare regulations and no longer allowed to provide this service for patients with Medicare.  However, in Florida, pharmacists are permitted to dispense custom and non-custom footwear to diabetics under their scope of privileges. Pharmacists in other states, especially New York, are petitioning their licensing boards for this privilege as well.

Response: Of course there should be some concern. It will dilute business for everyone. Besides Walgreen's getting into the Diabetic Shoe business, it would be good to know that from what I have heard Wal-Mart is also going to be tapping into this market also.  With their buying power and the number of stores, they will no doubt absorb a reasonable amount of business. Yet, one thing to keep in mind is that doctors will still have to write the prescriptions and the managing physicians will still have to sign the letters of medical necessity. Thus the question is, who will be fitting these shoes? Will the Doctors feel comfortable about referring their patients to these places? I think not, unless some of us who have the experience are working at these locations or overseeing and managing those that are fitting the shoes. From my experience most doctors who refer business out, instead of dispensing shoes and the foot orthoses themselves, usually send out the hard stuff that they don't want the hassle of dealing with. However, they still want the best for their patients and thus will still look for a  good Pedorthist to take care of their patients. With this said, it could prove to be a good job opportunity for some of us, and for others it could spell financial loss. Also, this may not work out for them, and they will just get out of the shoe business.  I guess we will just have to wait and see.

Economic Sting Could Hit More Firms

While some macroeconomic indicators are pointing to early signs of recovery, many footwear players will be on bankruptcy watch heading into the first quarter of 2010.

"This is going to be a tough holiday, and I think we'll see some additional bankruptcies after the holidays," said SportsOneSource analyst Matt Powell, noting that the recession will likely outlast the cash reserves of many companies, particularly smaller brands and retailers. "We'll continue to see the attrition of the smaller companies. That has had the biggest impact on the industry. As long at the unemployment rate continues to go up, people aren't going to feel like shopping."

Homeless Men Got New Shoes, Footcare

Homeless men received new shoes and basic foot care from doctors as part of a national day-before-Thanksgiving outreach program executed in 40 states. In Burlington, medical personnel and volunteers from Fletcher Allen Health Care will visit the Committee on Temporary Shelter on Wednesday to give out 100 pairs of new shoes and basic foot care.

The mission of the national "Our Hearts to Your Soles" program is to provide less fortunate people around the country with shoes and free foot exams with the belief that proper foot health is an essential part of everyday life. Full Text

Appalachia, Southeast Hit Hardest by Diabetes

While rates of obesity are climbing across America, they are especially high in sections of Appalachia and the Southeast, the U.S. Centers for Disease Control and Prevention reports in its first county-by-county survey.

Obesity and diabetes "are basically the two conditions of greatest concern for U.S. adults right now," said study lead author Edward Gregg, chief of epidemiology and statistics in the CDC's division of diabetes translation.

"They have been increasing for the last 10 to 15 years," he said. "This is the first time we've been able to look at the most local level, defining where the problem is." The findings are published in the Nov. 20 issue of the agency's journal, Morbidity and Mortality Weekly Report. Full Text

Treatments Available for Painful Foot, Leg Conditions of Diabetes

Painful nerve damage in diabetics, which is called diabetic peripheral neuropathy, is a most disabling and disturbing conditions that wreaks havoc on quality of life. It affects between a quarter and half of diabetes mellitus patients, leaving most unable to detect pinpricks or other sensations in their feet or legs.  Diabetic neuropathy is linked to more injurious falls and an increase in foot ulcers, which often results in amputation. The condition is believed to be caused by poorly controlled blood sugar levels.

Physicians who treat painful diabetic neuropathy should first rule out other causes of the pain.  They also should counsel patients to get their glucose levels as close as possible to the normal range. Many years ago I showed that when we infused normal males with glucose, they were less able to tolerate pain. Similarly, diabetics with high blood glucose levels feel more pain than those with normal blood sugar levels. Glucose appears to coat the opiate receptor making it impossible for the bodies' own morphine (endorphins) to regulate pain.  Full Text

Pedorthic Pre-Certification Training Course February 1st - 14th, 2010

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. Pedorthic Pre-Certification Training Course February 1st - 14th, 2010

 

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute.
