NEW QUESTIONS OR COMMENTS


Comment:  I just visited my local Walgreens in Weston, Florida and saw that they have a big Doctor Comfort Display.  The Walgreen's store saleswoman told me that Walgreens had just partnered with Dr. Comfort and that they accepted Medicare if I had diabetes.  However, when I asked her if the store employed a Pedorthist or Podiatrist, she had no idea what I was talking about.

I was surprised that Doctor Comfort would do this.  Is anyone else concerned about this?

Question: I have a couple of wholesale accounts that I make custom orthotics for, and they heard that Medicare is paying/reimbursing for custom orthotics for non-diabetics. They said that they billed L3020 (Custom Orthotics) and got paid! So I asked to go and see the EOB and the paid for the L3020 at 320.00 a pair, has any one else heard or seen that going on? Or did I just let the cat out of the bag?

Response:  Medicare will pay for an L3020 orthotic ONLY IF the orthotic is used as an integral part of a metal AFO. You should not bill Medicare for an L3020 otherwise.

Question:
  I have a 21-year-old patient who has a small foot, about a size 1-2sh.  Are there companies that make a small 3/4" heel or higher to accommodate this individual?  Most of the models available are Mary Jane styles.  I can modify the shoe accordingly to make it fit better, I just need the shoe.  This is a rare situation and I am not looking for inventory, just some styles that can hopefully become options.  If I have flour, I can make bread.  Please help. Thanks.

Response:  Keeping Pace has these sizes, but if you are looking for a Mary Jane shoe, try the Mini Bells shoes, size 32, 33 or 34.

Question:  I have been ordering Drew shoes for different stores throughout my last twenty-plus years. Read more.

Response:
  Mr. Johnson, I also have used the Lori and it does not fit every patient, client or customer. I have yet to have one returned, but I sell very few, as I fit the shoe to the foot not the foot to the shoe.  As for orthotics, I make them for the shoe, but (and this is my opinion and what I do with my business) I never use heat moldable.  If someone qualifies for TSD then they need more protection than heat moldable, especially the type I have seen done in my area. But then, this is a whole blog monster of its own. I have also used the Desiree, Delite, Bloom and several others from Drew as well as SAS, Brooks, P.W. Minor and others.   As a Pedorthist it is my job to choose from all inventory to find the proper fit for the patient. It is also my responsibility (a four letter word to many) to know how each shoe style in my inventory fits a multitude of different feet. Many times I sell two shoes to each person because I know which shoes fit alike and will keep the person comfortable; people like more than one pair of shoes. If you fit them properly, the shoes will sell themselves.

The ability to fit shoes properly is what SETS A PEDORTHIST APART FROM ALL OTHER SHOE SALESPEOPLE and all other shoe stores. It is a focus of our training. A good Pedorthist should be able to go from the Brannock measurement, a quick foot assessment and knowledge of their own inventory (key) to know which size and width would work for each individual foot, doing the same quick assessment whether the person you are fitting is a person with diabetes, arthritis, bunions or just someone wanting to buy shoes - longer assessments, of course, for patients.

Mr. Johnson has every right to be upset, if he feels the need, though.  He just may want to revisit his business plan. I have owned my company for 5 years (still a baby, I know, and focused on improvement daily). I went back to look at my returns and we have only had about 2% of shoes returned over that time. Most of the time it is because a patient, client or customer was not able to make the mindset change to the comfort shoe world.  But this is not the vendors' responsibility.  We have more issues with shoes on back order and deposits.

The buck stops with the Pedorthist, again, our JOB no, our CAREER is to know what shoe would fit best. If the style does not work for your patrons, as Mr. Torppey stated [in his response in the last issue] stop selling it. In the big picture the shoe  will go away if no one buys it to sell.  But also I say stop putting the blame on anyone but yourself for buying the shoe to carry if your patrons don't like the way you fit it. Once we make the decision to carry a style, unfortunately our recourse for our decision is to put it on the sale table, Ebay or charity and stop carrying the style (the gamble of retail.) Some vendors, if approached right, will trade inventory.

As for Sales Reps, I have seen 3 of my reps through the 5 years I have had my business. Most of them have such huge territories they don't have time for everyone. But whether they are new or old Reps, it is very possible there are issues they need upper management to deal with. It is the way of "Corporate."  Owning our own businesses, we are corporate we don't have to answer to a Boss, just the mirror.

As to Mr. Rabing's small issue [in his response the last issue] about the comments being published by the competitor - I personally would be more worried if they did not publish it.  I appreciate their willingness to face the repercussions for publishing people's opinions and comments that are negative towards a competitor. Over the years they have also published good reviews and comments without editing (as far as I know).   This is a benefit to the Pedorthic Community, as well as the beauty of living in the United States of America!  To be able to "air" things out.

TOMS Shoes: Footwear for Philanthropy

A young crowd of Dallas fashion enthusiasts gathered in the shoe department of the NorthPark location of Neiman Marcus to watch a lighthearted match of ping-pong on Nov. 12.  Stacks of shoeboxes and rows of heels are common on the second level of the luxury department store, but a battered recreational table affixed between Manolo Blahniks and Christian Louboutins is quite abnormal.

A handful of public relations representatives bustled around the crowds queuing up the next opponent of Blake Mycoskie, chief shoe giver of TOMS shoes.  He visited Dallas last week for a special event to promote the new collection in his philanthropic footwear line.

The former SMU student owns TOMS shoes, a business that provides shoes for a cause. The company is based on the "One-for-One" concept; for every pair of shoes purchased, one pair is given to a child in need. Full Text

Doctor Studies Effectiveness of Drug for Diabetics' Feet

Dr. Mackie Walker at Carolina Musculoskeletal Institute has checked and rechecked the effectiveness of a new drug for diabetic peripheral neuropathy.

 

In 2004, Walker was skeptical of a PamLab drug rep's claims for a new medication called Metanx for treating diabetes-related pain in the feet. So he designed a small drug study to test it out and was pleased with the results. In 2005 he put 30 diabetic patients with foot pain on a standard dose of Metanx for a year and checked their pain levels after six months and at the end of the year. Full Text

Diabetics: Make the Most of Caring for Your Feet

Diabetics are prone to foot problems because high blood sugar (glucose) reduces blood flow, and causes hardening of the blood vessels and nerve damage (neuropathy) in the feet.

Poor blood circulation prevents foot wounds from healing properly. Nerve damage can cause numbness that may prevent you from realizing you have any wounds. Left untreated, wounds can become infected and cause gangrene (tissue death). If the gangrene isn't caught in time the infected toe or foot will have to be amputated.

Check Your Feet Daily For:
 
1. Blisters, ulcers, cuts and other sores. If you find any, get medical attention right away.

2. Changes in the shape of your feet. Don't try to force a deformed foot into regular shoes--see your health care provider for therapeutic shoes. Full Text

CU-Boulder Study: No Advantage for Runners with Prostheses

A study co-authored by a professor from the University of Colorado's Locomotion Laboratory shows there is no performance advantage for athletes who use prostheses made for running.

Rodger Kram, an associate professor of integrative physiology at CU, said the team of researchers examined sprinters who used the Flex-Foot Cheetah -- the running prosthesis used by Oscar Pistorius, the South African athlete who was temporarily barred from competing in the Beijing Olympics -- and similar prostheses for one leg.  The study was published in Biology Letters on Nov. 4; the lead author, Alena Grabowski, earned her doctoral degree at CU in 2007.

 

Using force-measuring treadmills at the Orthopedic Specialty Hospital in Murray, Utah, researchers measured ground reaction forces, or the force a runner applies to the ground, and the leg's "swing time," or the time it takes the runner to pick up a leg and put it back down.  Full Text

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. Pedorthic Pre-Certification Training Course February 1st - 14th, 2010

 

In cooperation with the ABC requirements, the Eneslow Pedorthic Institute (EPI) is offering a three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at the Eneslow Pedorthic Institute. Upon completing the course and successfully passing the competency tests, and fulfilling the 1,000 hour "Clinical practice Requirement" students are eligible to sit for the ABC exam to earn their Pedorthic Certification (CPED Pedorthist).
