NEW QUESTIONS

Question: Just wondering if anyone knows what sort of HIPAA compliancy issues, if any, would be involved with emailing back and forth with your patients?

Erick J. Janisse, C.O., C. Ped.

Question: Does anyone know where to purchase the old tailor's bunion pads that were discontinued by Pedifix a number of years ago? Doesn't anyone still make these or something like it?  These devices worked fantastic for relieving pressure at lateral 5th metatarsal.  I don't see them in any catalogues. Thanks.

- Scott Torppey, CO, C. Ped.

PREVIOUS QUESTIONS WITHOUT RESPONSES

Question: The new ABC Pedorthic Patient Care Experience Requirement mandates that the aspiring pedorthist to complete 1,000 hours of pedorthic patient care experience prior to applying for certification. These hours can be obtained at either accredited or non-accredited facilities; may be a combination of patient care and retail experience and may be obtained prior to, or after, completion of an approved pedorthic course. Serving as the clinical training portion of a pedorthist's education, this requirement provides students with practical application of the knowledge, skills and competencies gained during their education programs.

BOC requires “1,000 hours of supervised patient fitting experience." It does not further define these. BOC does say that some hours may occur before attending the fitting course." This requires experience, but leaves enough room that all candidates, retail and clinical, can achieve it. Many therapeutic shoefitters have used their experience to fill this requirement and move up to Pedorthist, all while remaining at their current place of employment.

How will these requirements affect our pedorthic practices and the pedorthic profession going forward? What affect is it having on your decision making about your future needs and options?

- Robert Schwartz, C. Ped., Eneslow Pedorthic Enterprises

Question: I have a question to pose to the Pedorthic community.   I have decided it is not worth my time and energy to be accredited for Medicare purposes. Is anyone else out there thinking this way?
I do not see reimbursement improving in the future or the government actually doing anything which will help PROVIDERS with their healthcare changes.  It seems to me there are going to be fewer and fewer providers.   I know two people who have been accepted to Medical school but are not going because of the direction things seem to be going. And look at our profession, where are the new pedorthists? It appears no one wants to join our profession as it is unsure where it is going right now.
I have moved my business to more cash based in the last few years and will continue as a Pedorthist, but I will no longer spend all the hours filling insurance unless it is private insurance (out of network of course) and then there will be a fee each time we file the forms.

I just wondered if I am the only one who is tired of all the requirements and fees coming from Medicare and no Pedorthic supporting entity seeming to be able to stop the snowball.  IE:  Some providers exempt from Surety Bonds if they own their own business but not us?

Curious to know,

- Lee A. Hunter, C. Ped.

Question: Do any owners have non-compete contracts with employees?  What are parameters for owners who have non-competes?

- PNW Reader

Report: Brand Preference Influences Differ by Teen Age Group

While product quality is the most important influence in how teens make brand decisions, it is far less important to those consumers aged 12- to 17-years-old than it is to consumers as a whole.  While it is can be expected that teens would have different influences than adults, it is interesting find key differences between younger teens and older teens as well.  Those are a couple of key findings in the latest Brand Strength Report Teen Edition recently released by The SportsOneSource Group. Despite Warnings, Flip-flops Have Become Universal Footwear

by Lisa Appleton

Flip-flops may be the most popular footwear on the planet. They have been in and out of American fashion since the 1950s, with a widespread revival in the 1990s for both men and women.
The same shoes are called thongs in Australia, chappals in India and zori in Japan. In developing countries, they are typically the cheapest footwear available, often made from recycled tires and sold for the equivalent of $1.

Yet for years, doctors have warned that wearing flip-flops for long distances could be painful. Podiatrists don’t recommend wearing them for daily use because they can cause pain in the feet, knees or even lower back. Full Text

Footwear News Names New Publisher

Condé Nast and the Fairchild Fashion Group announced on Tuesday that former Men’s Vogue publisher Marc Berger will take the business helm at Footwear News, effective immediately. Berger, who left Men’s Vogue when the magazine closed in November, began his career with Condé Nast at GQ in 1999, and over the years held advertising and publishing positions with Vanity Fair, Cargo and Details. His Heart Goes Into Soles

by Angela Mapes Turner

I fell in love in November 2006. The shoes were smooth, coffee-bean-brown leather. By miracle of optical illusion, their rounded toes made my long, narrow, size-10 feet look downright dainty, or so I liked to think. Their kitten heels made me feel feminine without adding unnecessary height. They could go with nearly anything, anywhere, and they did until that fateful April day when the heel on my left shoe broke.

I hand them to Chuck Bolinger of Chuck’s Foot Support with little hope for their revival. Full Text

Small Diabetic Foot Burns Can Cause Big Problems

by Charles C. Chante, MD

Patients with diabetes have an increased risk for foot burns, and once a burn occurs, the morbidity and mortality is quite high, it was said at the annual meeting of the American Burn Association. A patient might sustain a foot burn without being aware of the injury because of impaired sensation in the feet. Insensate feet lead to prolonged exposure= and deeper burns.

Walking on hot surfaces, soaking in hot water, and even car heaters can cause foot burns. Full Text

Sweet relief: New research could help prevent insulin resistance for Type 2 diabetics

...Researchers have concluded that those with Type 2 have decreased levels of this gene controller ARNT, which in part explains why Type 2 diabetics have this overnight glucose production problem.
This study, printed in the May 2009 issue of the online journal, Cell Metabolism, could mean improvement in treatment for patients in ethnic communities where Type 2 affects more people. Full Text

How Your Favorite Sneakers Got Their Names

by Ethan Trex

We all know that Nike is named after the Greek goddess of victory, and the origin of "Air Jordan" is hardly obscure, but where did some other famous shoes and brands get their names? Here are the explanations for a few favorites:

1. Converse Chuck Taylor All Star

Nearly everyone's walked around with his name on their feet at some point, but who the heck is Chuck Taylor? Full Text

BECOMING A CERTIFIED PEDORTHIST

Join A Growing Service Profession: Pedorthics

Pedorthics is a professional field covering the design, manufacture, modification and/or fit of footwear. Pedorthics encompasses shoes, orthoses and foot devices that prevent or alleviate foot problems caused by disease, overuse, injury or congenital defect. Certified Pedorthists work with patients and their footwear to conform to a doctor's footwear prescription as part of the patient's treatment

To become a certified pedorthist, you must take a pre-determined number of course hours in pedorthics and pass a certifying examination overseen by either The American Board for Certification in Orthotics, Prosthetics & Pedorthics® or Board for Orthotists/Prosthetists Certification, Int’l.  Both the ABC and BOC are independent certification bodies that set standards for the profession and are recognized by the National Commission for Certifying Agencies.
