Question: My 75-year-old mother has a curved spine that has worsened over the years and now causes her to lean heavily on the left when she walks. She uses a cane and has her shoes altered so that her left shoe has a half-inch lift, and for a long time that was all she needed. Lately she has been experiencing severe foot pain along the outer edge of her left foot when she steps on it or stands. She has seen a doctor and had an x-ray of her foot and is waiting for the results. Assuming there is nothing physically wrong with her foot, does anyone have a recommendation for a foot care product such as an orthotic that would somehow even out the stress on her foot and take the pressure away from the outer edge? She also wants to know if she should see a podiatrist. Any ideas are appreciated.

Response: I think a gel thotic that P.T.I. Orthotics offers is awsome for your mom. The phone number is 303-443-9999 ask for Aaron. The only thing is that the orthotics can get a little pricey but it is worth it.

Response: She probably has one of the following pedorthic oriented diagnoses (it is good she has seen a physician for an x-ray, as the lateral column is important to the locking mechanism of midtarsal joint and foot function: 1) lateral column overload; 2) impending stress fracture; or 3) peroneal tendonitis. Shifting the weight from the lateral side of her foot is a great idea, but use caution when adding a lateral wedge if greater than 3/16 inch, this may cause the subtalar joint to unlock and cause a medial column overload. If she has a plantar flexed first metatarsal, this would also cause stress to the first ray if you put the lateral wedge on the shoe, inside the shoe or on an insert or orthosis. You could try adding either a lateral forefoot or lateral heel wedge versus a full sole wedge, which would be more gentle in transfering weight. As for the x-rays keep in mind that if it is a stress fracture, it may not show up for 3 or more weeks post trauma. Stiffening up the sole of the shoe or placing a carbon graphite plate, or piece of 1/8 inch polypropylene inside the shoe will splint the foot during the healing phase. However, a thorough biomechanical examination is in order when determining what foot orthosis to dispense, because we should use caution when tranferring weight when we don't know what the forefoot plane of deformity is and her hindfoot, midfoot and forefoot has not been described in your question. When in doubt, one could take an insole and cut out the first metatarsal head or entire area distal toward the toe. This would allow the first ray to function in it's own axis and not cause a medial column overload. This could also raise the arch some in case she does have peroneal irritation/tendonitis. You would in essence be transferring weight without taking the risk of posting the segments of the foot incorrectly. Of course, some might say a cuboid pad, lateral flare, Brooks addiction, etc. are good stabilization products, but we would need more assessment information before making a formal recommendation. Lastly if the lift on the left side is too high, she will most likely bear more load on that limb. Consider consulting the prescribing physician to approve decreasing the lift height.

Response: It sounds to me like she is supinating (rolling outward) and causing an overload on the lateral column. If her shoe modification is correct and the build up is not broken down then she could use some internal correction. Without seeing her foot it's hard to make a proper dx and tx. If you could take a few digital photos of her standing barefoot on a hard floor that would help. To reproduce her lift you can take the photos with a magazine or book of the same thickness placed under her short side. The pics can be taken from straight ahead and straight behind.

There is a premade device that I designed and patented along with Dr. Arthur Manoli. It's designed to control inversion and the cavus foot. It's available from DJ Ortho and it's called the Arch Rival. Obviously a custom device (if done properly) is a good solution. in my experience too many practitioners do not know how to make a proper device for that condition. Again, doing it via the Web isnt the best but a few pics can help. You can post them here or email them to me. What city is your Mother located in?

Response: When I was in clinical practice, besides the corrective biomechanical things we all do as pedorthists I also committed my foot health ministry to helping patients whenever possible prevent problems from occurring in the first place, or to helping wounds heal more quickly if they had already occurred. Such services frequently focused on soft tissue (skin) management, of which, in my current capacity, I am an advocate and a proponent. At your mother’s age, there may not be much natural protection left on the plantar surfaces of her feet. Fat pad dissipation may have left her with bony prominences rubbing on skin and the shoes she is wearing, creating all manor of discomforts. And her physical problems may have exacerbated it all with extraordinary weight bearing requirements on her left side. (Are her shoes actually lifted fully from heel to toe with the addition of a ½ sole or is she actually just using a ½ heel lift? Is there any difference between the lateral and medial side of the elevation or is it even across the entire width of the foot? Perhaps she needs slightly more elevation on her lateral side? Really, only a physical examination of her feet and observation of her gait by a professional foot specialist such as a certified pedorthist will be able to address and deal with these mechanical issues.) You have not mentioned how new or old your mother’s shoes are, but perhaps it’s time to have your mother carefully measured and re-fitted using her Integrated Solution: a system consisting of a padded, engineered sock (not just some skinny nylons or thin cotton socks, by the way!) coupled with a commercial insert or a custom orthotic (it sounds like she may be a custom candidate but only her certified pedorthist will be able to make an appropriate recommendation following his or her evaluation) and the right shoes for her specific feet and whatever activities she engages in again something her pedorthist should be able to recommend. Assuming there is nothing that the x-ray identifies and none of the other soft tissues (nerves, blood vessels, tendons, ligaments, muscles) are being compromised, why not make an appointment for her with a certified pedorthist to fit her into a new Integrated Solution and see if improvement can be achieved? At a minimum her quality of life should be better and it may just result in the solution you and she are seeking!

Editor's Note: Thank you for the thorough and helpful responses to my question. I have forwarded them all to my mother, who was thrilled with the help. Pending results of the x-ray later this week (I didn't know it took so long to get x-ray results these days!) she will make an appointment with a Certified Pedorthist in her area and bring along these responses.

Comment: Here is the link to the latest Medicare announcement about surety bonds. Orthotists and prosthetists that practice in states with licensure are not required to buy a surety bond (within certain rules of what else they supply or who actually owns the supplier entity). From the language of this announcement, it appears that even in licensure states, pedorthists still have to buy the $50,000 bond. If someone can read this announcement and come up with a different interpretation, would you please share? Also if anyone has found a Medicare approved surety bond company that does not require a personal guarantee, would you please share with me at jmishasek@earthlink.net.

Response: In a previous life I was a commercial insurance underwriter, and my spouse now underwrites Surety bonds for a large commercial insurance company. Neither one of us understands why this bond requirement is needed. We are the provider of durable medical supplies-shoes and orthotics- and will invoice Medicare, a medicare HMO and /ora secondary insurer for payment for our services. What obliations do we have which would expose us to up to $50,000 in exposure through this procedure.

If anyone should be requesting protection via issuance of a bond in "our" favor, it should be the pedorthist providing the goods and services. Can anyone explain the exposure we are deemed to have created for ourselves?

Trusted Retail Brands to Benefit in the Year Ahead

The research consultancy Verdict has just released its latest analysis of the retail landscape in the UK, which shows that consumers are leaning heavily towards trusted retail brands including Marks & Spencer, Ikea and John Lewis, especially for large ticket items.

Verdict estimates that over the next couple of years a total of £3.9 billion will be available for retail survivors to pick up from the 100 retail chains which have collapsed since the start of 2008. The majority of this will be in clothing and footwear and furniture and floorcoverings, the two sectors most affected by the economic downturn. Full Text

New Shoe May Help Wearer Burn Calories

Some shoes are built for looks, others, for comfort. Chung shi shoes claim to be for burning calories, toning, tightening, improving posture, and other health benefits, according to their maker.

Foot Solutions, the store that sells this popular footwear, says the shoes combine new technology with the ancient wisdom of rice paddy workers returning to their huts. "Walking on... bamboo matt massaged their foot," explains Dallas franchise owner, Dianne Curry. "Their arch area, increased their circulation and the respiratory. And so that's the concept and the principle upon which chung shi's were built."

Chung shi's, and other shoes with similar concepts, have a 15-to-20 degree incline in the toe area. That rise is designed to keep the foot in constant motion, burning calories. "Because you are in movement and because you are using additional and more muscles, you also are burning more calories if you're using them on a regular basis," said Curry.

Crystal Gill, a pedorthist who specializes in fitting shoes, says the incline also can help alleviate pain. Full Text

Drug Cuts Amputation Risk in Diabetes

Treating type 2 diabetes patients with a cholesterol-lowering drug called fenofibrate cuts the risk of a first diabetes-related limb amputation by 36%, according to a new study published this week in The Lancet.

In cooperation with the ABC and BOC requirements, Eneslow Pedorthic Institute (EPI) is offering the three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, interactive CD’s and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at Eneslow Pedorthic Institute from August 10th through August 25th, 2009. The course includes the required tests for competency to sit for the ABC and BOC exams.
