Response: It may depend on what state you live in that determines the amount of reimbursement. I can give you some idea about what we face in Wisconsin.
We have to do a pre-auth (PA) request for every diabetic shoe, as well as every pair of inserts. Currently, Wisconsin Medicaid will only cover one pair per auth, and they would have me pre-auth another pair only as need warrants.....aka when they wear out. Imagine trying to pre-auth every four months for a pair of TSD inserts, for something that is reimbursed less than Medicare fee schedule. Oh, and an auth can take 3 weeks.
They will usually cover it if it follows Medicaid guidelines, but it is much faster through Medicare. I believe that for our Missouri location, we also need auth AND a CMN for Medicaid. The requirements may vary from state-to-state, so if you are a provider for your state Medicaid, I would ask them. Anyone else out there have these kinds of issues from Medicaid? May as well open that can of worms.....

Question: We
occasionally see adult customers that have what I would describe as a
metatarsus adductus foot shape (either unilateral or bilateral). The
digits and metatarsals are deviating medially 8-12
degrees. There may not be any evidence of excess pressure or callusing on
tip of hallux from use of current footwear. Other than an obligue toe box
shoe style, i.e. PW Minor Erika, what other shoes should one consider? When
the customer is wanting a shoe toe box style that is not rounded or oblique, we
have found that going longer in length helps to fit to this foot shape,
And does trying to "pronate" the foot structure with lateral wedging improve
situation or create new problems? Or is this the situation of one
should accommodate fixed deformities and treat only flexible
deformities?
Question: With the increase in community aquired MRSA
infections, I would like to hear from other pedorthists working within a medical
setting as to what protocol they follow when working with pt.s with infected
ulceration. Wearing gloves, sanitizing everything that is used during
appointment(Brannock, pen, floor,chair arms,Scissors,etc..) are all a part of
mine. Also, what type of education are we providing to these
patients? When pt's sit down with me most of them want to take off their
socks. They then want to either remove or touch the bandaging they are
wearing to indicate the location of the ulcer. Most of these dressings are
saturated with drainage. If that pt. now has the infectious drainage on
their hands, just think what that hand will be touching (door handles,
countertops,pay phones,vending machines,gas pumps, ATM machines,etc.). I
have made it a standard of care to always put paper on the floor and advise the
pt. (especially a pt. with an ulcer) that I will remove their footwear and
bandaging and educate them on the importance of keeping the sight as clean as
possible.

Walking May Not Be Best First Step for Diabetics

Everyone knows that exercise is good for you, but if you are one of the more than 19 million Americans living with type 2 diabetes, reaping the benefits of exercising isn’t always as easy as a walk in the park.
Studies by the American Diabetes Association have proven that, among other benefits for people with type 2 diabetes, physical activity improves insulin sensitivity and helps bring elevated blood glucose levels into the normal range. Unfortunately, people with diabetes often experience diminished sensation in their lower limbs and are at risk for developing foot wounds.
