Comment: I received a letter today from the Pedorthic Footwear Association seeking, yet again, support for a name change to the Pedorthic Footcare Association, or maybe some other as-yet-to-be-determined name. I applaud them for their tenacity at the very least and for the appearance to involve the full membership in an issue that has been discussed, voted upon and soundly defeated in a yearly membership meeting where enough members were present for there to be a quorum on a bylaw change. Nevertheless, it is my opinion, and experience, that good marketing precludes drastic product name/logo changes in the middle of a rough time. Instead, I believe that PFA’s marketing activities should focus on further exposure and recognition of a long standing, foundational icon (PFA = Pedorthic Footwear Association) in order to draw more business through the memberships' front door, touting the quality, longevity and experience that PFA’s members are known for. Regards y’all.
Comment: I have just received a letter from PFA proposing a name change. My question is how do all the other Pedorthics feel about this? My opinion: we voted in November 2008 and voted NO! Let it go. Why do the board members insist upon continuing to try to get Pedorthics to vote for the name change. Did the board forget this has already gone to a vote? Isn’t one NO sufficient?
We have expanded our retail store to cover our losses with Medicare and added Manufacturing Orthotics and Shoe Modifications for other medical professionals with our lab just to survive the economy. And we are doing very well, but it will cost me an arm and a leg to get a surety bond because we are doing so well with the other sections of our business.
Comment: Here is a link that readers of Pedorthic Newswire should visit. It is a link to a HME News article on Medicare surety bonds. In the article is a link to a listing of supposedly approved sellers of surety bonds. I say supposedly as there could be issues distinct to the DME market. After all, DME suppliers have an appeal process after a recoupment. I have not seen any information on when a surety bond company would be asked to compensate Medicare. So one should be asking the surety bond company to spell out in writing the actually process by which the company would respond to a request by a DMERC for payment.
Comment: I have been searching the Insurance Industry for the best rates an the new $50,000 bond required for Medicare Providers and have been able to find the annual premium as low as $250 for qualified applicants. Please feel free to pass on my contact information to any of your readers who would like a bid on this bond.
Question: We see lots of patients who have hallux limitus and who wear high heel shoes. What shoe modifications would you recommend for such a patient?

If no symptoms, we suggest that the patient consider preventative care to slow down the progression of the hallux limitus such as healthier shoes, arch supports, Dr. Scholls Bunion Gel to limit friction on the dorsal joint, wider shoe if possible. Although the high heel shoe can hold the hallux in a dorsiflexed position and the arch raised due to the high heeled shoe, that it may be slowing down the pathology from becoming hallux rigidus. I would consider the patient's gait cycle to see if she was transferring the weight of the foot laterally as if compensating for the hallux limitus, even though she has no pain, but limited motion to the 1st MPJ. A sandaled shoe, high heeled, might be the best avenue for reducing stress on the dorsal aspect of the 1st MPJ if shoe pressure is a concern moreso than biomechanical positioning.
If she presents with pain, the above recommendations are still advisable, however, one could escavate under the first metatarsal head to allow it to plantarflex more and the phalange may glide over the distal end of the first metatarsal. Otherwise, a scaphoid pad in the dress shoe, may also provide the mechanical advantage to permit the same benefit as just described. Stretching the shoe or a lycra dress shoe upper may offer benefits.
We prefer to encourage movement rather than limit motion of the first MPJ, therefore we would use a morton's extension, a turf toe plate, or something of this nature, including a stiff sole, only if we could not obtain a more optimal anatomical position of the first and the first ray. If the foot showed signs of pronation due to faulty foot mechanics (heel valgus in gait - which is rare in a high heel shoe, unless PTTD is present, or a limb length descrepancy/pelvic tilt that caused undue pressure on the 1st MPJ, then a full sole lift or platform shoe (one shoe ground lower to adjust for the LLD or pelvic tilt) might produce a desired result.
Othewise, a medial sole wedge could serve to offload the symptomatic hallux limitus as well a forefoot rocker, met bar or internal dancer's pad, might provide relief.
The potential compensatory symptoms are vast, therefore I will limit my recommendations to the above suggestions without further information on the patient(s) specific cases.

Response: In Texas.....under Medicaid STAR PLUS..Superior does cover shoes and a pair of diabetic inserts.

Response: It may depend on what state you live in that determines the amount of reimbursement. I can give you some idea about what we face in Wisconsin.
We have to do a pre-auth (PA) request for every diabetic shoe, as well as every pair of inserts. Currently, Wisconsin Medicaid will only cover one pair per auth, and they would have me pre-auth another pair only as need warrants.....aka when they wear out. Imagine trying to pre-auth every four months for a pair of TSD inserts, for something that is reimbursed less than Medicare fee schedule. Oh, and an auth can take 3 weeks.
They will usually cover it if it follows Medicaid guidelines, but it is much faster through Medicare. I believe that for our Missouri location, we also need auth AND a CMN for Medicaid. The requirements may vary from state-to-state, so if you are a provider for your state Medicaid, I would ask them. Anyone else out there have these kinds of issues from Medicaid? May as well open that can of worms.....

PREVIOUS QUESTIONS AND COMMENTS WITHOUT RESPONSES
Question: We
occasionally see adult customers that have what I would describe as a
metatarsus adductus foot shape (either unilateral or bilateral). The
digits and metatarsals are deviating medially 8-12
degrees. There may not be any evidence of excess pressure or callusing on
tip of hallux from use of current footwear. Other than an obligue toe box
shoe style, i.e. PW Minor Erika, what other shoes should one consider? When
the customer is wanting a shoe toe box style that is not rounded or oblique, we
have found that going longer in length helps to fit to this foot shape,
And does trying to "pronate" the foot structure with lateral wedging improve
situation or create new problems? Or is this the situation of one
should accommodate fixed deformities and treat only flexible
deformities?
Question: With the increase in community aquired MRSA
infections, I would like to hear from other pedorthists working within a medical
setting as to what protocol they follow when working with pt.s with infected
ulceration. Wearing gloves, sanitizing everything that is used during
appointment(Brannock, pen, floor,chair arms,Scissors,etc..) are all a part of
mine. Also, what type of education are we providing to these
patients? When pt's sit down with me most of them want to take off their
socks. They then want to either remove or touch the bandaging they are
wearing to indicate the location of the ulcer. Most of these dressings are
saturated with drainage. If that pt. now has the infectious drainage on
their hands, just think what that hand will be touching (door handles,
countertops,pay phones,vending machines,gas pumps, ATM machines,etc.). I
have made it a standard of care to always put paper on the floor and advise the
pt. (especially a pt. with an ulcer) that I will remove their footwear and
bandaging and educate them on the importance of keeping the sight as clean as
possible.

Everyone knows that exercise is good for you, but if you are one of the more than 19 million Americans living with type 2 diabetes, reaping the benefits of exercising isn’t always as easy as a walk in the park.
Studies by the American Diabetes Association have proven that, among other benefits for people with type 2 diabetes, physical activity improves insulin sensitivity and helps bring elevated blood glucose levels into the normal range. specialize in diabetic wound care as well as pediatric and adult biomechanics.

In cooperation with the ABC and BOC requirements, Eneslow Pedorthic Institute (EPI) is offering the three-level competency training course for aspiring CPEDS. To provide maximum benefit in a minimum of time, a portion of the level one curriculum will be administered through a distance learning program that includes required readings, interactive CD’s and testing of all mandatory topics. Upon passing the level one test, students will attend the remainder of the pedorthic pre-certification course at Eneslow Pedorthic Institute from August 10th through August 25th, 2009. The course includes the required tests for competency to sit for the ABC and BOC exams.
