Comment/Offer: My husband, Michael Soska, was a pedorthist who received his degree in NY. Unfortunately, he passed away last year at age 49 from cancer. I am in the process of dissolving his business, The Foot Support Center, and selling the building that it was in. I have several bookcases and a whole cabinet of pedorthic books and supplies. Would any of your readers be interested in any or all of these materials (for free)? I would hate to throw them away but I need to empty the building in a few weeks.

Comment: Every week the news-wire covers it seems the same subjects, those that are probably foremost in the mind of the C.Ped. trying to make a living. Insurance reimbursement, regulation compliance, facility accreditation. I hate it, you hate it, but we all have to deal with it. Every week I think again, but what about...

Perhaps one of the things that the industry should look for is non-reimbursable work. Secondary sales like an additional pair of shoes for the diabetic. Socks, fitting aids and the like. Stuff that you can buy and retail for a pretty good markup. I would love to see more discussion of any ideas that others have, things that work for them and perhaps more importantly what did not work for them. How much stock to carry? Should I stock?

Like others I'm sure, I get plenty of information seeking calls, and stop-in visits. I spend time, and perhaps expend effort, and the return is nil. How long, how often can one do this and not try to figure out how to get paid for their time? Do I try to collect some sort of "office visit" fee. A minimum fee? I'm a professional, certified, but in my state not licensed, because pedorthics is not regulated. Can I justify a fee? Should I? Of course, but how to get the public to swallow it. I'm not a doctor, but I apparently teach much more that the doctors in my corner of the world do to my customers. Or are they patients? Is such a fee ethical? Legal? It makes my head hurt sometimes, forget about their feet.

My practice is oriented to shoe modification, having grown out of a shoe repair background. I know that I do good work. Some of what I see, done by others, is awful. Since there is no regulation most of what I see that is poorly done seems to have been done by the local shoe repair guy. The person that has to use it then brings it to me to try to make it usable. Or comfortable. A surprising amount is done by some O & P facility that would not be able to make a respectable shoe modification if their insurance rates were dependant on it. Makes me wish for regulation sometimes. Then that makes me mad too. Did I say makes my head hurt?

Just my random thoughts for Monday. Discuss amongst yourselves. Write me. Call and leave nasty messages, or nice ones. Or just call and say hello.

At risk of getting windier and giving an unwelcome sales pitch, is there anyone out there in need of shoe modification services? Would they be interested in having their work done by a shop that understands their needs and can provide same at a reasonable cost, having them done by a C.Ped?

PREVIOUS QUESTIONS AND COMMENTS WITH RESPONSES

Question: Since pedorthists are not supposed to diagnose, should BOC Pedorthists or Certified Pedorthists be assessing for Loss of Protective Sensation in self referred clients? Is the use of the Semmes-Weinstein monofilament a diagnostic test that should not be used by pedorthists? If it is used by a pedorthist during an encounter with a client, what are the findings termed in their chart, i.e. LOPS but not peripheral neuropathy?

Response: Great Question. Good example of the necessity if careful consistent charting. When a Pedorthist uses a Semmes-Weinstein monofilament it is an assessment tool for Pedorthics not a diagnostic tool. By using one, you enhance your assessment of the patient, giving the Pedorthist one more piece of information to enable the best assessment you can possibly do. If the patient fails the test you know they have loss of feeling, you don’t need the why. It DOES NOT mean you tell the patient why they have a loss of feeling. You can state to the patient the test was failed and recommend they consult with their doctor about the loss of feeling shown by or according to the test results. You then send a report to the doctor; stating the patient failed the monofilament test for protective sensation and how many points failed on the test. I.e.: 6 of 10 etc. If you are doing custom orthotics you have to contact their doctor anyway to get an Rx so it gives you an opportunity to advise the doctor of the failed test and they realize you are part of the team for that patient’s continuing plan of care. One more marketing point of contact.

All of this is in no way diagnosing. You know the test results. They either feel the prick or they do not, then charting wise: state Pt failed Monofilament test points ? of ?, recommended pt to speak with Doctor due to failed test. That is all you need to state.

I do a test every time a diabetic patient (any diabetic whether they state they have loss of feeling or not -- they do not always know) or a patient who has PVD or CHF comes in for either their initial assessment or their 3, 6 or 12 month check ups. The sensation situation can change at any time. I have actually had patients who after different treatments regain some or all of their feeling.

Things like the Semmes-Weinstein are assessment TOOLS for us. The more tools we use the better job we can do for our patients. More information is always better.

Response: Mr. Mishasek is correct pedorthists cannot diagnose, but it is absolutely essential that we assess. Otherwise, we may miss a treatment modality or fail to discover misdiagnoses. If I have a patient that comes in without a diagnosis from a doctor, I simply state, I believe you may have plantar faciitis, Mrs. Jones, but I am not a doctor and I cannot diagnose. If you want a diagnosis, you must see a physician. I have a list of recommended ones for your area.

Of course, for relatively minor problems the patient may choose an OTC remedy such as orthotics and/or specialty shoes that you may sell in your establishment, but that is the choice of the patient and is not prescribed by you. I believe that is perfectly legal and ethical. My lawyer does, too!

Response: Let's define assessment, findings and diagnosis first. An assessment is an evaluation of the health status of an individual by performing a physical examination after obtaining a health history. Physical Findings are the results of a physical examination. They are made visually, by auscultation (listening), palpation, smell, percussion, etc. Diagnoses is the art or act of recognizing the presence of disease from its signs or symptoms (physical findings), and deciding as to its character. The assessment is done first, producing findings second, and then a diagnosis is determined from findings. So, that being said, Pedorthists assess and record their findings. As stated, we do not diagnose. Utilization of a Semmes-Weinstein monofilament, as well as many other assessment tools/techniques, allows the pedorthist to gather, and record data vital to patient assessment and implementation. It is that data that we use to determine if the self referred individual should be referred onto an appropriate healthcare team member due to a possible medical issue. In the case where an individual has a prescription in hand, we use the data to ensure that we have a full understanding the prescription. If there is data gathered during an assessment that conflicts with the prescription, it is our responsibility to contact the physician for clarification before proceeding. Findings for the above encounter as best charted as pass/fail on a sensory exam, not a LOPS or Neuropathy exam.

Response: The Pedorthic industry was founded in order to make a mechanical/physical change in the podiatry field, since the foot problems grew, and the podiatry industry were not able to pay to much attention to many problems, therefore, the health department decided to come out with the Pedorthic field.

A Pedorthist has a major role in helping helpless patients, especially the DM patients. In order to help them to stay alive, and keep their drive to stay alive mentally and physically, by preventing ulcerations, gangrene and amputations, we have to do anything in our capabilities, and follow the protocols/rules,regulations and guidelines, and use every single method that we are trained for, in order to succeed our mission.

Using certain Dx tools does not mean that we are diagnosing, we where given the benefit and opportunity to use certain tools to find critical problems which the patients PCP or podiatrist has overseen. The same is true with the EMS system; the regular EMT's do not diagnose, and they treat patients with medications even though they haven't diagnosed. All they do is follow protocols and treat their findings. That is exactly what we were told and trained to do, treat your findings, and never cross your red line by treating above what you are trained in. There are always situations and fine lines that you may find yourself in. As long it is within your training and guidelines of practice you are to do anything in your power to assist your patients and do the right thing. If necessary, call the patient's doctor to discuss further.

Comment: Hi! My name is Crystal and I am a medical biller for a C. Ped. I have been a medical biller for several years now. My boss had shown me your most recent issue and brought to my attention the required documentation content. It seems that most people are thinking that you now are required to obtain medical records from the physician. I remembered getting information on a letter to send to the physicians explaining what was required of them for us to be able to bill our services to Medicare. The letter simply stated what had to be in the patients records that are at their office and what we required. In the letter it states that the patient's medical records must be made available if requested. (See the information HERE:) And it gave links to the website where you could find more information.

I went to the Web site and pulled up the "documentation inventory form." It states that:

"All medical information except for the CMN may be
maintained in the patient’s medical record (not

necessarily in the supplier’s file), but must be available

to the DME MAC upon request."

That said, as long as the documentation is in the physician's records, you do not need it in your files. If DME MAC requests it, then you can request it from the physician at that time. And only the record for where the physician wrote that the patient needs your products. Hopefully this clears up the confusion!

The letter states clearly what is expected from the doctors for Pedorthists to do their job

Hyperbaric Oxygen Therapy: Relief for Diabetic Foot Ulcers

Studies show that diabetic foot ulcers affect 15 percent of all people with diabetes and the most debilitating complication of diabetes would be amputation. Foot ulcers precede approximately 85 percent of all major amputations of the lower limb. And within a year of amputation, approximately 9 to 20 percent of all people with diabetes would have to undergo a second amputation. The effects of amputation, subsequent costly physical and mental rehabilitation and fast spiraling healthcare costs would further contribute to patients' distress.

For a long time, the mainstays of treatment for patients with lower limb ulcers have been surgical debridement and wound dressing. This leads to the ever-present danger of nonhealing wounds as most diabetic patients have very poor blood supply because of greater incidence of peripheral vascular disease extremities, even after debridement. Poor blood supply means no matter how many antibiotics are prescribed, the tissue concentration of these expensive antibiotic cocktails rarely reaches therapeutic levels.
