ABC Adds Experience Requirement to C.Ped. Exam Eligibility Criteria

The American Board for Certification in Orthotics, Prosthetics and Pedorthics, Inc. (ABC) will be implementing a 1,000 hour pedorthic patient care requirement for all candidates seeking to become a Certified Pedorthist (C.Ped.).

Effective March 2, 2009, pedorthic exam candidates will be required to possess a minimum of 1,000 hours of patient care experience. This experience must be obtained in a pedorthic patient care facility prior to becoming eligible to sit for the ABC exam. The intent of the new patient care experience is to help provide students with practical application of the knowledge, skills and competencies gained during their educational program.

Having the pedorthic experience requirement ensures the public that all ABC Certified Pedorthists have met the highest certification standards out there today, said Robert S. Lin, CPO, FAAOP, president of ABC’s board of directors. Practical experience is a necessary component of training to become a contemporary C.Ped. We believe that practical experience, coupled with the requirement to complete a pedorthic education course, create the proper foundation for an individual to become a successful and competent patient care provider. Comment: Here we go again [re: accreditation]. Get the little guy first. Most Pedorthic facilities are struggling against the bigger O&P facilities anyway and here they get another hurdle thrown into their path. Thanks CMS and anybody who worked on this.

How are we (Pedorthists) integrated into the O&P profession (as our associations want us to believe) when there are double standards like this? No surprise that at least one association is basically OK with this sudden change of policy:

They are " prepared to work with each pedorthic organization to meet the new deadline. They’re here to help. Their staff and accreditation surveyors are prepared for the applications and site visits that will be necessary to meet the CMS requirement

Yes, we all knew that accreditation is here to stay but let's get everybody accredited without any exemptions for the "big guys!"

Question: Can readers recommend a central fab for shoe modifications? Our key individual for doing shoe mods is suffering through a medical condition that may knock him out of action for a long time, and all of our other pedorthists are totally busy with non-shoe mod customer service responsibilities. We have sent customers over to one orthopedic shoe repair business but he is too busy to take on much more. There seems to be a shortage of individuals nationally to replace the current generation of orthopedic shoe technicians. Any recommendations would be appreciated.

Question: Please advise on the proper coding for repair(s) of shoes and foot orthotics received under the Therapeutic Shoe Program under Medicare. Is it acceptable to use L4205, even though it is an L code not A? Thank you.

Question: I have a friend in another city who runs a retail shoe store. He is looking to get one of his employees certified as a C.Ped. and he also knows that a newly certified pedorthist would not be capable of stepping right in and seeing patients without significant on-the-job training - which he is not able to provide as the employee would be his first C.Ped. He is running into great resistance from local pedorthic and orthotic businesses when inquiring about the possibility of doing an internship. He has offered to pay a handsome sum for even just a week or two of observation and he is getting nowhere. I understand that they would feel threatened, but I also don't know how someone like this would get the necessary training. The schools sure don't seem to be turning out fully equipped and "field-ready" pedorthists. Any suggestions for him to help break down the barriers with the pedorthic/orthotic facilities...or regarding this problem in general?

Response: The University of Western Ontario, with the support of the Pedorthic Association of Canada's Canadian Certified Pedorthists (CPed(C)) offer an in-depth post-graduate programme that educates and concurrently trains students to enter the practice of pedorthics. During the one-year programme where all of the educational content is delivered utilizing web-based learning, the students are engaged in practicums across Canada in the areas of Clinical Assessment, Fabrication, and Footwear Modification.

Response: Is your friend willing to travel? We offer modest, free housing and would be happy to have a person do internship with our facility. We work in Orthopedic and Podiatric offices, as well as having a podiatric clinic, podiatric lab and pedorthic clinic within our school. We also have a small retail shoe store for some hands on experience in shoe fitting.

Response: I've also thought of this before. I would think a mini fellowship program could be a good solution. Most practitioners can't be away for any length of time. Perhaps a 2 or 3 week fellowship in different parts of the country would be a great idea. Maybe some of the larger shoe or materials companies could donate some funds to defray the costs and help pay for it. I would imagine they would get more bang for their buck if one of the fellows was using their products AND having their tuition defrayed by company X. The goodwill it would generate would create a customer for life.

I am not slamming anyone here but I feel as though the general competency level has been lowered in the last 5 years or so. I'm not blaming anyone but it seems almost like people are being processed through with less fundamental background or skills. Some people may have the book skills to answer enough questions to pass. That doesn't mean they are ready for anything more than a mid-level position in a high level facility.

Having fellowship's located in various regions of the country could be a great way to raise the skill level and training of C.Peds. It may also address the issue Erick raised that seems to be a "not in my backyard" problem. The thinking may be, why should I teach a competitor that works in a facility 5 miles away from me. I would think that the concern over someone stealing your play book wouldn't be as great if the person was from another state. I do some training and sales for Amfit. To date, I've probably hosted 10 + C.Peds or CO's from various parts of the country. I also instructed Masterfit University courses for over 10 years. Not once did I hold back info or feel that I needed to keep some tricks in my back pocket out of fear of competition.

Maybe this is an idea that some of the larger companies should explore. Often times one gets too comfortable with their tools and skills. Nothing wrong with that but it can also lead to complacency or stagnation. Working around different C.Peds in different facilities would be a benefit to C.Peds, patients and suppliers. I would be interested in discussing this with any other concerned parties.

Response: Erick, Perhaps facilities across the US could partnership and have an exchange program take a non-competing C. Ped. into your facility (and give him a room at your house) if the other facility does the same in return. It would almost be like a residency. Two or four weeks in another environment would really help. Universities exchange students all the time!

Response: I would recommend looking for a locum tenen, especially if he is willing to pay a salary. It will not be easy, but I suspect one can be found. Or send the new pedorthist to a facility that is out of the treatment area and pay an educational fee.

Question: Cracking Sound of 1st MPJ: A 10 Y/O female complains of "cracking" of the 1st MTPJ while walking. It has been present only a few weeks. There is very mild pain in the joint only present first thing in the morning or after prolonged sitting. She hears cracking mainly while barefoot or with sandals, but it is not present with athletic shoes. She plays soccer and ice skates. The cracking occurs during gait at heel-off and going onto the toes to push off. It is audible from about 15 feet away. It is only present with closed kinetic chain dorsi-flexion. She has a right foot varus, ankle equinus with early heel-off. There is mild pronation during midstance. Open kinetic chain ROM 1st MTPJ is normal w/o crepitus or pain. Any thoughts to the cause/treatment would be appreciated.

Response: In response to Robert S Schwartz, CPed regarding the 10yo female with 1st MTPJ cracking sound during closed kinetic chain propulsion, I offer the following thoughts:

This clinical case presentation is consistent with 1st MTPJ impingement secondary to metararsus primus elevatus. Although reliable information can be obtained from a skilled physical examination, the most reliable assessment is made via weight bearing radiographs. You should expect to see
the dorsal cortex of the 1st metatarsal diverge distally from the dorsal cortex of the 2nd metatarsal on lateral view. The clinical importance of this finding has to do with normal 1st ray biomechanics. Normally, the 1st metatarsal will plantarflex and evert during propulsion. This is requisite for the normal 65-75 degrees of closed kinetic chain 1st MTPJ dorsiflexion. In the absence of this 1st metatarsal plantarflexion-eversion, the base of the proximal phalanx of the hallux will impinge on the dorsal head of the 1st metarsal. Over time, this is expected to create DJD and limitus. Often this can be treated with a 1st ray cut-out.

I encourage you to refer this young lady to a knowledgable podiatrist for weight bearing radiographs.

Marwit Capital Partners II L.P. announced that Boot Barn Holding Corp. (Boot Barn or the Company), its western footwear and apparel retailer, completed the purchase of fourteen retail stores in Wyoming (12) and Nevada (2) from BTWW Retail, L.P.

The transaction was completed pursuant to BTWW’s sale of assets under Section 363 of the U.S. Bankruptcy Code and closed effective November 26, 2008. Limping Chinese Market Put Indian Footwear on Fast Track

The Indian retail industry is growing dynamically, and so is its footwear retail industry. The easy availability of raw material and skilled manpower at relatively much lower cost is expected to keep the demand for Indian footwear buoyant in near future. "Indian Footwear Industry Analysis", a new report by RNCOS, expects the Indian footwear retail market to grow at a CAGR of over 20% through 2011.

According to Shushmul Maheshwari, Chief Executive, RNCOS, "Even the ongoing financial turmoil will not be able to significantly impact the demand for Indian footwear in global markets. This is because the demand for high quality footwear made in Europe or other regions of the world will certainly suffer a setback as people will prefer buying inexpensive, or precisely, medium or low-priced footwear. 2 Diabetes Drugs Double Fracture Risk in Women

Two widely prescribed diabetes drugs, Avandia and Actos, double the risk of fractures in women but not in men, a new British analysis finds.

Avandia (rosiglitazone) and Actos (pioglitazone) are used to lower blood sugar in patients with type 2 diabetes. Recent studies have suggested that the risk for heart failure, death and heart attack were increased with Avandia, touching off a controversy that resulted in new U.S. Food and Drug Administration-mandated label warnings about the drug. Diabetic Foot Infections and Treatment

Diabetic foot infections cause frequent visits to the health care team and may lead to amputation. However, late last year, MacroChem declared that it implemented the option they previously obtained. It now has the sole license right for pexiganan. This is used to treat the mild variety of this condition.

The president and CEO of MacroChem said that this will broaden their company's product portfolio. Pexiganan will join their other foot product called EcoNail (R) which is used to treat nail fungus. Both drugs therefore would treat foot diseases by specialists who will prescribe them. This will be mainly podiatrists. These two products will also interest the primary care physicians and physician specialists. Full Text
