Question: Cracking Sound of 1st MPJ: A 10 Y/O female complains of "cracking" of the 1st MTPJ while walking. It has been present only a few weeks. There is very mild pain in the joint only present first thing in the morning or after prolonged sitting. She hears cracking mainly while barefoot or with sandals, but it is not present with athletic shoes. She plays soccer and ice skates. The cracking occurs during gait at heel-off and going onto the toes to push off. It is audible from about 15 feet away. It is only present with closed kinetic chain dorsi-flexion. She has a right foot varus, ankle equinus with early heel-off. There is mild pronation during midstance. Open kinetic chain ROM 1st MTPJ is normal w/o crepitus or pain. Any thoughts to the cause/treatment would be appreciated.

Retail season starts strong, but a long haul ahead

Sales were better than expected this past Black Friday, but will it last till Christmas?

In Rochester, N.Y., the roads were clogged at 4 a.m. last Friday as shoppers hit the malls looking for bargains. In Burlington, Mass., it took as long as 25 minutes to find a parking spot early that day. And at Lenox Square in Atlanta, throngs of shoppers hunted for discounts at shoe retailer Nine West.
 
Yes, Americans love the sound of 40 percent off. The deep discounts and promotions, retail analysts believe, were successful at getting Americans to open up their pocketbooks on Black Friday, one of the biggest shopping days of the year. Sales rose 3 percent above 2007 levels – better than expected, according to ShopperTrak RCT, a tracking firm based in Chicago. Full Text

Nike, Starbucks call for climate policies, renewable energy
 
Nike Inc., Starbucks Corp., Levi Strauss & Co. and two other U.S. companies called Wednesday for aggressive policies to limit global warming to help rescue the country from an economic crisis.
 
The coalition appealed for steep cuts in U.S. greenhouse-gas emissions, investment in renewable energy from the wind and sun, and limits on polluting coal-fired power plants. Congress and President-elect Barack Obama should take action on the matter early in 2009, they said. Full Text

Holiday Outlook Gloomy, Stores Offer Big Sales
 
After a brutal autumn, footwear retailers are rolling out deep discounts as a last-ditch effort to attract consumers into stores this holiday season. Store owners across the board told Footwear News last week they are predicting a highly promotional retail environment — perhaps more so than in any season in memory.
 
“I haven’t seen anything as promotional as this in a while,” said Melissa Gallagher, VP and DMM of men’s footwear at Barneys New York. “This year is much more challenging than [the industry] is used to.” Full Text

Can New Prez Turn Around Keds Business?

Keds is starting with a new leader at the helm, but the brand’s prospects for a revival are still uncertain — especially amid a challenging economic climate. Retailers interviewed last week said Kristin Kohler Burrows, who is joining Keds from Camuto Group, has a rocky road ahead within Collective Brands Inc.’s Stride Rite division.
 
“[Keds] has seen some resurgence in some pockets, but it just hasn’t made a big splash. It hasn’t taken off quite yet,” said Corky Fulton, owner of four Sagesport stores in North and South Carolina. “Particularly the champion oxford — they really can’t make that become a fashion thing [again], except for a little product placement.” Full Text

Daily Care Helps Avoid Diabetic Foot Complications
 
Dr. Jennifer Sartori, a member of the American College of Foot and Ankle Surgeons with offices in Portsmouth, reminds residents with diabetes that following simple foot-care tips can reduce their risk of toe, foot or leg amputations.
 
People with diabetes are 10 times more likely to have a lower limb amputated than people without diabetes, according to the American Diabetes Association. Sartori says the disease can cause poor blood circulation and nerve damage in the feet, making them vulnerable to developing ulcers, infections, deformities and brittle bones. Full Text

Genetic Screening May ID Diabetes Risk

Genetic screening can help identify people at risk for type 2 diabetes, but it adds little to traditional methods for determining risk, two new studies show. Researchers searched for 16 recently identified genetic variants associated with an increased risk for diabetes in one study and 18 diabetes-linked variants in another.

Pedorthics is a professional field covering the design, manufacture, modification and/or fit of footwear. Pedorthics encompasses shoes, orthoses and foot devices that prevent or alleviate foot problems caused by disease, overuse, injury or congenital defect. Certified Pedorthists work with patients and their footwear to conform to a doctor's footwear prescription as part of the patient's treatment

To become a certified pedorthist, you must take a pre-determined number of course hours in pedorthics and pass a certifying examination overseen by either The American Board for Certification in Orthotics, Prosthetics & Pedorthics® or Board for Orthotists/Prosthetists Certification, Int’l.  Both the ABC and BOC are independent certification bodies that set standards for the profession and are recognized by the National Commission for Certifying Agencies.
