Question: Here is a staffing and operational scenario that both of the credentialing organizations should address about individuals who are either an ABC CFts or a BOC Footwear Specialist.

Scenario: Depth shoes and custom inserts are ordered as Medicare TSD benefits for an individual. At the initial appointment, the BOC Pedorthist or ABC Certified Pedorthist assesses and confirms and documents the medical necessity of the physician’s orders for the footwear as well as the need for custom diabetic inserts. The pedorthist then makes models of the individual’s feet. The depth shoe portion of the order is turned over to the facility’s BOC Footwear Specialist or their ABC Certified Fitter of therapeutic shoes who then works with the client to determine the appropriate style and size of depth shoes. Later this employee, whose job responsibilities include working in the lab for the company as a lab technician, uses the pedorthist’s models of the TSD client to fabricate the custom inserts. This CFts/FS individual trims the custom inserts to fit in the depth shoes that had been selected. At the client’s next appointment, the pedorthist places the TSD shoes with the custom inserts on the client’s feet and then checks the final fit of the client’s feet with the depth shoes and custom inserts and makes any adjustments as needed.

Does having CFts or Footwear Specialist credentials disallow an individual from being a lab tech and fabricating custom orthotics?

Does the scenario described here show the opportunity to use BOC Footwear Specialists or ABC CFts as pedorthist-extenders, just as PA’s or nurse practitioners are physician extenders to handle some of the physician’s basic workload?

Response: This is in response to the recent question posed on theNewswire concerning the appropriateness of a CFts participating in the fabrication of custom orthoses.

In the scenario described, the CFts participates in the assessment and measuring for non-custom “depth shoes”, while the C. Ped. manages the custom foot orthoses portion of the care. The question is whether it is appropriate for the CFts to participate in the fabrication of the custom foot orthoses.

Providing fabrication services for custom fabricated orthoses does not violate the CFts scope of practice. Their scope of practice states: “An ABC certified fitter of therapeutic shoes is an individual trained and qualified in the provision of non-custom therapeutic shoes and non-custom diabetic multi-density inserts. A certified fitter of therapeutic shoes is competent to practice within the scope of non-custom therapeutic footwear only.”

The “provision” of non-custom therapeutic shoes and inserts refers to direct patient care services. If a CFts’s responsibilities within an organization includes fabrication and/or other duties that are not direct patient care, this is not addressed within their ABC scope of practice.

Question: Recently I was asked to help a person who was said to be a supinator who also has posterior tibial dysfunction (as reported by our local sports conditioning clinic, folks who have a strong reputation). I was not able to palpate the area, review heel position or check the flexibility of his foot, but gently questioned the diagnosis, as I always thought that PTD and a excessive supination to be an oxymoron. I asked if his issues were with his peroneal tendon rather than his PTT, but it was not. He is being treated at the clinic with a CFO with slight lateral wedges and they want him in a neutral, flexible and cushioned running shoe. I am not an expert here at all, so I was wondering if anyone out there has dealt with such a funky foot (or if even such a foot is possible)?

Response: I have found that feet with a 2 - 5 FF varus and a plantarflexed first ray, may cause PTTD Symptoms. However, I would be more able to answer the question if I knew what the resting calcaneal stance position to be, and what range of motion the midfoot had. Also, what the ROM of the forefoot upon the locked hindfoot in pronation and supination, would help me to determine what type of posting might be beneficial. There must be some pronatory action being applied at either the midfoot, hindfoot or forefoot or else a motion/function that is causing the PTTD symptoms. We typically do not recommend lateral posting for this foot diagnosis, as it will unlock the subtalar joint and cause additional strain on the PTTD. Perhaps a Reverse Dual Asis Posting might best benefit this patient, but I would need the information above to offer suggestions on using this technique. Please feel free to contact me for additional discussion on this foot type. Basically, I am needing to know if the forefoot or hindfoot is fully, partially or uncompensated. I would suggest you also present this case study to the podiatry forum at http://www.podiatrym.com/ to get insightful recommendations from podiatrists through out the country.

Langer Orthotics Business Sold

Langer, Inc. sold substantially all of the operating assets related to its Langer branded custom orthotics and related products business to an affiliate of The Orthotic Group, a manufacturer of prescription custom foot orthotics, orthotic footwear and gait analysis equipment headquartered in Markham, Ontario.

The company's Deer Park, NY facility as well as its sales office in Markham, Ontario are included in the transaction. The purchase price was approximately $4.7 million paid in cash at closing, including $475,000 to be placed in escrow to satisfy potential indemnification claims. Adidas Implements Hiring Freeze to Battle Economy

Adidas Group Chairman Herbert Hainer e-mailed all employees Friday to say that, due to the troubled economy, the company is imposing a hiring freeze, cutting external meetings, seminars and consultants, and aiming to pare travel expenses next year by 15%, reported The Oregonian. Shoe industry presses Congress to drop shoe tax

With the words “You have bailed out Wall street, now help Main street”, nearly 50 companies representing over 160 well-known footwear brands and retailers urged Congress to pass the Affordable Footwear Act. The act would end the shoe tax on lower to moderately-priced footwear and all children's shoes. Concerned about the slowing economy and the credit crunch that is sharply curtailing consumer spending, the footwear industry sent a letter last week asking for the Act to be included in a larger economic stimulus package Congress is expected to consider in a November lame duck session. The Affordable Footwear Act eliminates a hidden but costly import tariff that can add as much as 30 percent or more to the retail price of a pair of shoes. Established at the start of the Great Depression, the shoe tax was meant to protect domestic manufacturers from cheaper imports. But it still remains, even though 99 percent of all shoes sold in America are imported. Full Text

Researchers: Cost of Diabetes Care on Rise

Diabetes drug costs in the United States have almost doubled in six years to $12.5 billion as more people are diagnosed and patients receive newer, more expensive treatments, a study found.

Researchers, writing in this week's issue of the Archives of Internal Medicine, questioned whether these newer drugs are more effective than older, cheaper treatments. About 18 million people in the United States were diagnosed with diabetes in 2007, an increase from 8.1 million in 1994, according to the Centers for Disease Control and Prevention. Worldwide sales of diabetes drugs may jump to $22 billion by 2016, analysts say. Full Text

Larry Miller to Undergo Hyperbaric Treatment for Diabetes

Jazz owner and entrepreneur Larry Miller will be going inside a hyperbaric chamber to help him deal with complications from his diabetes and a bone infection. The chamber has become an effective tool and a unique way to speed up the healing of wounds.

Miller has continued therapy and rehab at home since he was released from the hospital more than two months ago. He's healing from the heart attack that put him there, but complications from his diabetes linger.
