Question: I operated on this 43 y/o female about 17 years ago for bilateral bunions. She had a distal metatarsal osteotomy with two-screw fixation. I saw her recently because she was concerned about discoloration over the incision of her right foot.

She noticed this discoloration three years ago, but did not tell anyone about it. The discoloration has not changed in size since she first noticed it. She has no pain in her feet and wears 3" heels routinely.

Foot x-rays taken a few weeks ago show that the screw heads are not prominent and are countersunk into the metatarsal. The discoloration on the skin is located over the screws. I have performed thousands of osteotomies with internal fixation and have not seen this type of discoloration before. Has anyone encountered this and could the discoloration be a result of something else?

Response: In response to Mr. Schwartz's inquiry asking if any PNW readers are aware of any studies establishing the medical necessity and efficacy of custom made foot orthotics, I offer the following:

The current issue of the Journal of the American Podiatric Medical
Association (JAPMA vol 98, no 5, Sept/Oct 2008) has three articles of
interest to the Pedorthic profession:

1. The Short-term Effectiveness of Full-Contact Custom-made Foot Orthoses and Prefabricated Shoe Inserts on Lower-Extremity Musculoskeletal Pain (Trotter and Pierrynowski, pg 357)

2. Computerized Analysis of Plantar Presssure Variation in Flip-Flops,Athletic Shoes, and Bare Feet (Carl and Barnet, pg 374)

3. A Comparison of Rearfoot Motion Control and Comfort between Custom and Semicustom Foot Orthotic Devices (Davis,Sifchock and DeLeo, pg 394).

Trotter et al concluded that 3 weeks of full-contact custom-made orthoses use is associated with statistically and clinically important pain decrease. They also concluded that a return of symptoms is predictable if custom orthotic use is followed by use of prefabricated inserts. Finally, they also concluded that patients who begin with prefabricated inserts and then wear custom-made orthotics may not experience noticeable short term improvements.

Carl et al used F-Scan analysis to compare peak plantar pressures walking barefoot vs flip flops vs athletic shoes. Predictably, they concluded that pressures were highest while barefoot and lowest in athletic shoes. They also concluded that flip-flops offer minor only shock protection when compared to walking barefoot but place the foot at greater risk for pathologic abnormalities.

Davis et al used instrumented gait analysis to compare rearfoot motion
control by custom foot orthotics and semicustom foot orthotics in uninjured subjects. They concluded that there are few differences in rearfoot motion control between custom and semicustom foot orthotics worn by uninjured subjects during walking and running activities.

Question: One podiatrist recently told us that an OTC orthotic we sold (that had a metatarsal pad built in) caused a broken metatarsal bone in the person. I know nothing else yet (subjective or objective) about this case but do feel that he thinks we are willy-nilly on suggesting met pads (his position is that they are rarely needed) and the previously at least consistent trickle of referrals has dropped off. I did basic research for previous cases of metatarsal pads causing a bone to break, but couldn’t find anything. I about 1/3^rd of our Lynco business is with metatarsal pads, to offload indicated forefoot pressure, however we will get people into the non-padded version if their initial reaction if of discomfort or annoyance. I’ll try to talk to the doctor, but I’d like to ask the PNW readers if they can share cases of bone breaks caused by OTC metatarsal pads?

Response: This patient wasn't a post-menopausal, small-boned women, by any chance was it? There are osteoporotic (mostly women--particularly those who smoke and/or don't excercise regularly) patients out there, for whom it doesn't take much stress to crack a met bone. However, it's my opinion that there typically are a number of co-existing factors--activities, trauma, improper footwear, etc. (often unrecognized) which could have caused or at the very least contributed to such a problem. And I think it's a little reckless for any doctor to make a snap judgment about cause and effect without having convincing corroborating evidence to support such a proclamation.

Question: I am interested in PNW readers' thoughts regarding physician detailing and marketing in general. How are certified pedorthists detailing for new referral sources? What marketing strategies work best?

- Edith James, C. Ped.

Response: We found three ways of obtaining orthopedic physician referrals and I agree that most podiatric physicians will only utilize a pedorthist to manufacture their feet orthoses at wholesale or to refer their patients for shoes or modifications to shoes, as most pedorthists are considered a competitor to the podiatrist, regretfully.

1) The obvious - providing superior service, thus one physician referring your company to another physician.

2) Lunch presentations with chafing dishes filled with a choice of either 1) broasted chicken, corn served in a crock pot, overnight salad, fresh fruit, a veggie tray and rolls or 2) Mexican food - chicken, shredded beef, cheese and ground beef enchilada's, all the trimmings which easily can make taco salads for those desiring a more healthy meal (which is inexpensive to serve if you make the food yourselves). The Mexican dish worked quite well until other vendors began serving mexican food. The physician and staff get burnt out with the routine lunches i.e.: spagetti, lasagna, submarines and pizza.

3) Inservices at the hospital near the Operating Room is a nice way to meet the physicians as they arrive and leave for surgery. Be careful to not provide food that has an aroma, as that is offensive to the staff as patients can not have food before surgery and this appears inhumane to tease them with the smell of food before their surgical procedure.

4) As for using a form of follow up to build your practice: We developed a follow-up form that is faxed to the referring physician and the family practitioner. It entails who referred the patient, their diagnosis and the treatment plan prescribed as well as what may need to be introduced to the patient based upon our scope of practice and our expertise. This educates the orthopedic physician or family physician on our services and what modalities are available for that diagnosis. Many family physicians know the patient has foot pain but are unsure what treatment modalities work for that symptom and don't know if it is requires a conservative or surgical answer. The most important objective of the form is to notify the physician(s) of 1) the patient's satisfaction or level of relief of symptoms or lack of relief of symptoms within an appropriate time frame (no more than 3 months from the dispensement of the product in most diagnoses) , 2) if the patient was non-compliant or denied a prescribed or recommended treatment modality and 3) what you feel is in the best interest of the patient to achieve optimal alignment or relief of symptoms so the physician can handle the situation appropriately on their follow-up visit. We have armed the physician with the information necessary to prescribe or encourage the patient, etc.

While this form sounds like we may be prescribing treatment, we are merely sharing our experience of that patient and our scope of practice. This form is then faxed to both physicians. Another form we have offers the physician to approve or disapprove of the services recommended. Both serve as "Thank You For Referring" as well as a "Patient Progress Report" - "Request For Prescription". Lastly, we developed a pamphlet that is titled: What Your Physician Wants You To Know...

In this pamphlet it educates the parent/consumer on what foot type they or their family member may have and when to seek medical evaluation by a malalignment specialist i.e.: orthopedic or podiatric physician, chiropractor, physical therapist, orthotist, pedorthist, osteopathic physician, etc. Therefore all disciplines have an opportunity to address the malalignment of the person reading the pamphlet and no healthcare specialty is discriminated upon. Yet the pamphlet educates even the person untrained with malalignment to recognize the protocols established by the RMPI for prevention of pathologies.

Another way we obtain accounts is we notify physicians that we are doing a study on a specific diagnosis. We indicate that we are desiring to make 20 pairs of feet orthoses, with a full warranty (remake if the patient, physician or ourselves are dissatisfied with the results) for a clinical study we are performing on that particular diagnosis. The physicians learns who we are, what our mission statement is, our objectives and our treatment modalities. We have gained accounts through this and they have paid off in the long run. Initially it is a risk and expense you must absorb, but if you perform well, have professional integrity you can gain the trust of the orthopedic physician (or potentially the podiatrist - although I have not used this marketing plan on any podiatrists for the reason stated as they considered podiatrists as competitors and make take advantage of the "free orthoses" and never utilize the pedorthists thereafter. We do bill insurance but write off the balance if the patient or insurance does not pay for these cases in the study. We have not lost revenues in this endeavor to this date.

Hope this helps anyone seeking a new marketing strategy.

Response: We have been very successful marketing to Podiatrists. We attend a local seminar held each year for the local docs, by the local docs. This has been very good for us to be in the doctors' arena; I actually have a doctor that drags other doctors to my booth and tells them how great I am in front of me, a little weird feeling, but I smile and hand them Rx pads with a map on the back. If you can find local groups like this and participate in their programs it is well worth it. Last year I met one of the doctors, face to face, who would never send people to me or even talk to me on the phone. That broke the ice and he has sent a few people to me. Patient feedback to the doc's is also very important. Orthopedic surgeons are tougher, but once you get them on board they tend to be very loyal. They are too busy to shop around.

Don't forget the PT's, they have the doctors' ear and can suggest orthoses for their patients.

Question: I have recently become aware of a COF who is operating a multi-level marketing plan, recuiting people to fit and dispense diabetic shoes under the TSB. There is a "requirement" that they become "board certified" within 90 days. There are currently 5 certified shoe fitters using his business address, according to the ABC website.

Is this legal? Is this ethical?

Response: Basically, yes, this is legal. Just like AMWAY and AVON. However, should it be is another question. My own opinion is that if you do not carry adequate inventory and the equipment necessary to fit and modify the shoes and inserts you should not be able to dispense them. But the rules are very lax and that is just the way it is.

Question from an earlier issue: As a general rule, when designing foot orthoses for flexible foot conditions we put the rear foot into a neutral position and accommodate the position of the forefoot after the rear foot is corrected. In other words, for a flexible pes plano valgus foot, we post the rear foot with a varus (inner) post. If the forefoot is then put into a varus position, we put an accommodative varus post to keep the foot in plantagrade. After managing an orthotics laboratory and doing many trade shows, I have talked to all types of practitioners in the field. I find that more orthotics with forefoot posts are designed by people in the western part of the country than in the east. Obviously we are all looking to relieve the discomforts and pain experienced by our patients. How many practitioners have success when using forefoot posts and how many rarely use them at all?

Response: I would like to provide a better understanding of this complex topic to those readers who might be beginners in pedorthics so that they may better understand the conversation that has been transpiring regarding forefoot posting. I have placed my comments in bold. The question reads:

"How many practitioners have success when using forefoot posts and how many rarely use them at all?"

First and foremost, I would like to re-word this question. How many people have success when using forefoot intrinsic posting versus forefoot extrinsic posting; as each posting technique provides a different function. For example, a forefoot intrinsic attempts to bring the first or fifth ray to the supporting surface, thus reducing the forefoot varus or valgus in the forefoot. An extrinsic forefoot posting brings the ground up to the forefoot varus or valgus position. In my experience, both can be extremely useful if used for the appropriate foot type. A flexible foot can usually tolerate an intrinsic posting easily, while a more rigid foot with limited midtarsal joint range of motion, will function or receive best results with a forefoot extrinsic post. Sometimes, we must use partial intrinsic and partial extrinsic to achieve the desired objective/goal. So, it is important to include the words intrinsic and extrinsic when discussing forefoot posting, otherwise, we aren't talking the same terms or function. Both feet can receive relief of symptoms (success), but the real question on this topic is in our attempt to achieve optimal anatomical alignment. We should not judge or success by merely relief of symptoms. This is the difference between intrinsic posting versus extrinsic posting. One holds the foot in it's deformed forefoot position (extrinsic) while the opposing (intrinsic) attempts to place the forefoot into it's desired optimally aligned position thus allowing all five metatarsal heads to purchase the ground so the foot becomes the rigid lever needed for propulsion, the biomechanical needs more accurately addressed with intrinsic posting, while extrinsic posting is a compromise to the science, when optimal alignment can not be achieved. Mina Out as Foot Locker USA Chief

Foot Locker, Inc. has requested that Richard T. Mina resign from his position as president and CEO of Foot Locker, Inc., U.S.A., effective Sept. 30. According to a filing with the Securities & Exchange Commission, Mina's employment contract with the company will actually be terminated on Oct. 31.

Retailers lacking multi-channel shopping strategies

UK citizens are becoming increasingly sophisticated in their online shopping habits and retailers risk being left behind if they cannot provide multi-channel experiences to meet these growing needs, according to new research from IBM. The IBM study interviewed over 4,000 consumers in the UK and US about their shopping preferences, and found that over 75 per cent of multi-channel consumers prefer the experience of shopping online and then in-store.

The research identified the most popular shopping categories among multi-channel consumers as Apparel/Accessories & Footwear followed by Home Improvement/DIY and Appliances. Full Text

Ryder Named Aetrex Women's Product Director

Aetrex Worldwide has hired industry veteran Susan Ryder as women’s product director. The company hopes Ryder will help increase the design appeal of its women’s products.

Failing Credit Market Hurts Small Retailers

In the wake of the massive crisis in the investment banking system and a proposed government bailout plan, market watchers predict that footwear firms will find it more difficult — and more expensive — to secure credit for the foreseeable future.

While the credit crunch is likely to affect both publicly traded, multibillion-dollar companies and mom-and-pop shops, independents will face the brunt of the credit fallout, experts from the investment banking community told Footwear News last week. Care Pack to Help Diabetics Avoid Foot Amputations

AN innovative foot care pack has been developed to help diebetics spot problems and, it is hoped, reduce the number of amputations. Podiatry manager Audrey Murdoch, who works for NHS Greater Glasgow and Clyde, got the idea for the pack after completing a travel scholarship at the Royal Melbourne Hospital in Australia.

She said: "Patients with diabetes can lose sensation in their feet, so if they do not feel pain, they can be unaware if they have caused damage when they stub their toe, or have a blister.

"If they don't check their feet regularly, this can lead to serious infection and, in some cases, to amputation. Full Text

New Treatments for Foot Wounds in Diabetics Can Save Limbs...and Lives

A blistered toe or a scraped heel may be minor annoyances to most people, but to long-term diabetics, these injuries can allow deadly bacteria to enter the body, leading to foot sores and ulcers.

Because these patients often have poor circulation in the lower limbs, they have a harder time fighting off this bacterial invasion, and if the infection spreads, it can lead to gangrene, amputations, or even death.

New approaches to treating foot wounds are, however, changing this dismal picture for more than 240 million people worldwide who have diabetes and for those who face the possibility of losing a lower extremity because of the disease, estimated at 60,000 people each year in the U.S. alone.

These new treatments range from bio-engineered skin repair and growth-factor therapy to new shock-wave therapy (currently under clinical trials). Full Text

Skateboarders know that they will quickly destroy their footwear, but still don’t always seek shoes that are indestructible.
