Association (JAPMA vol 98, no 5, Sept/Oct
2008) has three articles of
interest to the Pedorthic
profession:
1. The Short-term Effectiveness of Full-Contact Custom-made Foot Orthoses and Prefabricated Shoe Inserts on Lower-Extremity Musculoskeletal Pain (Trotter and Pierrynowski, pg 357)
2. Computerized Analysis of Plantar Presssure Variation in Flip-Flops,Athletic Shoes, and Bare Feet (Carl and Barnet, pg 374)
3. A Comparison of Rearfoot Motion Control and Comfort between Custom and Semicustom Foot Orthotic Devices (Davis,Sifchock and DeLeo, pg 394).
Trotter et al concluded that 3 weeks of full-contact custom-made orthoses use is associated with statistically and clinically important pain decrease. They also concluded that a return of symptoms is predictable if custom orthotic use is followed by use of prefabricated inserts. Finally, they also concluded that patients who begin with prefabricated inserts and then wear custom-made orthotics may not experience noticeable short term improvements.
Carl et al used F-Scan analysis to compare peak plantar pressures walking barefoot vs flip flops vs athletic shoes. Predictably, they concluded that pressures were highest while barefoot and lowest in athletic shoes. They also concluded that flip-flops offer minor only shock protection when compared to walking barefoot but place the foot at greater risk for pathologic abnormalities.
Davis et al used instrumented
gait analysis to compare rearfoot motion
control by custom foot orthotics and
semicustom foot orthotics in uninjured subjects. They concluded that there
are few differences in rearfoot motion control between custom and semicustom
foot orthotics worn by uninjured subjects during walking and running
activities.

Question: One podiatrist recently told us that an OTC orthotic we sold (that had a metatarsal pad built in) caused a broken metatarsal bone in the person. I know nothing else yet (subjective or objective) about this case but do feel that he thinks we are willy-nilly on suggesting met pads (his position is that they are rarely needed) and the previously at least consistent trickle of referrals has dropped off. I did basic research for previous cases of metatarsal pads causing a bone to break, but couldn’t find anything. I about 1/3rd of our Lynco business is with metatarsal pads, to offload indicated forefoot pressure, however we will get people into the non-padded version if their initial reaction if of discomfort or annoyance. I’ll try to talk to the doctor, but I’d like to ask the PNW readers if they can share cases of bone breaks caused by OTC metatarsal pads?

Response: This patient wasn't a post-menopausal, small-boned women, by any chance was it? There are osteoporotic (mostly women--particularly those who smoke and/or don't excercise regularly) patients out there, for whom it doesn't take much stress to crack a met bone. However, it's my opinion that there typically are a number of co-existing factors--activities, trauma, improper footwear, etc. (often unrecognized) which could have caused or at the very least contributed to such a problem. And I think it's a little reckless for any doctor to make a snap judgment about cause and effect without having convincing corroborating evidence to support such a proclamation.
