Question: Are any PNW readers aware of any studies establishing the medical necessity and efficacy of custom made foot orthotics?

PREVIOUS QUESTIONS WITH RESPONSES

Question: One podiatrist recently told us that an OTC orthotic we sold (that had a metatarsal pad built in) caused a broken metatarsal bone in the person. I know nothing else yet (subjective or objective) about this case but do feel that he thinks we are willy-nilly on suggesting met pads (his position is that they are rarely needed) and the previously at least consistent trickle of referrals has dropped off. I did basic research for previous cases of metatarsal pads causing a bone to break, but couldn’t find anything. I about 1/3^rd of our orthotic business is with metatarsal pads, to offload indicated forefoot pressure, however we will get people into the non-padded version if their initial reaction is of discomfort or annoyance. I’ll try to talk to the doctor, but I’d like to ask the PNW readers if they can share cases of bone breaks caused by OTC metatarsal pads?

Response: I have been fabricating feet orthoses for 17 years and have never heard of a metatarsal pad causing a broken metatarsal shaft. They can cause soft tissue irritation or discomfort. The doctor you speak of said the OTC device caused the break. If the OTC device did not address the underlying biomechanics of the foot (not controlling enough) and the patient walks or runs a lot of miles it is a possibility that the metatarsal would have been damaged with anything other than a custom biomechanical device. Maybe this is what the doctor meant. If this is the case a MT pad probably would not have made much of a difference. A break would indicate a blunt trauma or crushing injury. In the feet orthoses universe almost all metatarsal fx's we C. Ped's will deal will be stress fx's. Stress fx's are caused by faulty foot mechanics, over training or weak muscles, NOT an MT pad! Hang in there Chris. I don't think you did anything wrong.

Reponse: 25 yrs of practice and about 18,000 pairs of orthotics (maybe 1/4 with met pads) and never been accused of a broken metatarsal. I could possibly conceive of that if met pad was VERY large and placed too far distal. But with an OTC insert I doubt that was the case.

Question: I am interested in PNW readers' thoughts regarding physician detailing and marketing in general. How are certified pedorthists detailing for new referral sources? What marketing strategies work best?

Response: Marketing to community physicians has been both rewarding and frustrating for our pedorthic business. Rewarding due to the many referrals that we do get from local physicians that we were able to see face to face, and frustrating from the standpoint that drug company reps have spoiled medical offices with free lunches for the entire office staff just to see a portion of the group that are in that day and that care to stop in for the food. And drug companies know that it can take 6-8 repeat face to face visits to change prescribing habits. So it could be extremely expensive in food costs and pedorthic time costs to compete with the the drug industry for access to a doctor.

I was able to piggyback on a drug rep's lunch one time when I asked a key physician's nurse to set it up for me to do so, and I got to visit with 3 of the 5 physicians for free (other than my time). They were extremely receptive to know that we were a resource to help their patients and actually referred about one patient a month off of that one visit for about a year. I think I dropped the ball by not finding time to do a follow up report after each referral from the group during that year to keep our name in front of the physician.

We try to do service reports to podiatric physicians after a problematic encounter to let the referring specialist know what happened (or did not happen). But I have wondered if it would be effective to copy the primary care or rheumatologist or other key physician to to try to broaden our referral base. But the challenge has been the time and personnel resources to do a meaningful follow up report. Ideally the report would give feedback on that specific encounter, plus let physicians know what all the other services and technology are that are available here. As is, positive customer feedback to local physicians has broadened our referral base without our ever seeing some community physicians. I would be interested in hearing if other pedorthic businesses are using written reports and if so, how you do them in a timely, cost effective fashion.

Response: I have tried all the things that Mr. Stanley did and more. I still do and will continue to do this. These are the things that I have found out.

1. Forget podiatry, especially if you want to market making orthotics. They don't want you and basically feel they don't need you. M.D.s take months and months of marketing. Mailers every few months. Forget cold calls. Family doctors are the best because they will send to you just as soon as send to a DPM. Also detail ancillary medical professions as well.

2. Rx pads with discounts do bring in business but you will sell Superfeet and cheap met pads for shoes clients already have. Not at all profitable, so hopefully you have a sales person/C. Ped. who can increase the sale to make this profitable. You don't to pay a C. Ped. to sell only $250.00 a day in shoe repair and pre-made insoles and nothing else.

3. A lot of walk-in business may actually come from the doctor detailing but the client never tells you. So you have to ask each new client how they heard about you. And if you see that it was doctor find out who the doc was and send him/her a thank you note.

4. Medicare business is real tricky, especially now that DPMs are selling this out of their office. Forget about getting a single Rx from a DPM if they too are doing the program. Better to bypass the DPM entirely and go directly to internal MD, family MD, DO and ancillary medical professions.

Question: I have recently become aware of a COF who is operating a multi-level marketing plan, recuiting people to fit and dispense diabetic shoes under the TSB. There is a "requirement" that they become "board certified" within 90 days. There are currently 5 certified shoe fitters using his business address, according to the ABC website.

CMS Announces Provider Exemptions

On September 3, 2008, the Centers for Medicare & Medicaid Services (CMS) announced those Durable Medical Equipment Prosthetics/Orthotics, and Supplies (DMEPOS) providers that are exempt from meeting the quality standards for DMEPOS accreditation. CMS, at that time, stated that Orthotists, Prosthetists, and Pedorthotists are included in that exemption. CMS will issue a notice of proposed rulemaking in 2009 that will define quality standards designed specifically for anyone furnishing or providing orthotics and prosthetics in order to be reimbursed for such supplies and services under Medicare Part B. Opposition Clash on EU Footwear Duties

Retailers are urging the European Commission to accept that its plans for import taxes on leather shoes from China and Vietnam are dead and buried.

With 15 EU countries declaring their opposition to proposals to extend the life of import taxes on shoes from China and Vietnam, the British Retail Consortium (BRC) called Wednesday on the Commission to recognise that carrying on with its review of the duties is now pointless. Full Text

Forecasts Call for Lowest Holiday Sales Growth Since 1991

With many economic factors weighing heavily on consumers’ minds and wallets, retailers should expect only a modest increase in holiday sales over the 2007 season, according to two forecasts released this week.

Deloitte’s Retail group expects holiday sales, excluding motor vehicles and gasoline, to increase 2.5% to 3.0% during the November-to-January period, less than last year’s 3.4% increase, and one of the smallest gains since 1991’s 2.0% uptick. The TNS Retail Forward forecast is for 1.5% growth compared with 1.2% in 1991for the holiday fourth quarter in the key holiday retail segments combined. Full Text

Though Yuan attributes her love of shoes to the TV series Sex and the City, she isn't on the prowl for Manolo Blahniks. Instead the 27-year-old senior merchandiser in the Shanghai office of a Hong Kong-owned garment sourcing company called Porta Asiatica heads for shops operated by Belle International, where she can find something she likes for under $80. "They are very popular, and the price is low," says Yuan.

That combination has helped Belle International become China's No. 1 woman's shoe retailer, with a 22% market share in the country, according to CLSA. Full Text
