Question: I am interested in PNW readers' thoughts regarding physician detailing and marketing in general. How are certified pedorthists detailing for new referral sources? What marketing strategies work best?

Question from an earlier issue: As a general rule, when designing foot orthoses for flexible foot conditions we put the rear foot into a neutral position and accommodate the position of the forefoot after the rear foot is corrected. In other words, for a flexible pes plano valgus foot, we post the rear foot with a varus (inner) post. If the forefoot is then put into a varus position, we put an accommodative varus post to keep the foot in plantagrade. After managing an orthotics laboratory and doing many trade shows, I have talked to all types of practitioners in the field. I find that more orthotics with forefoot posts are designed by people in the western part of the country than in the east. Obviously we are all looking to relieve the discomforts and pain experienced by our patients. How many practitioners have success when using forefoot posts and how many rarely use them at all?

Response: My name is Rodney Johnson C. Ped., owner of Innovative Orthotics L.L.C.. I currently use intrinsic forefoot posting when I take a neutral suspension cast, so in other words I capture the deformity in the forefoot and then apply the functional method post at the Talonavicular joint. It sounds like you are trying to produce a method called a biaxial or triaxial method. For more information on these methods you can contact The Robert M. Palmer Institute of Biomechanics in Elwood, Indiana. The way I practice with the forefoot posting intrinsicly (inner), I have a very high success rate and almost no adjustments needed. Shoe store goes extra mile to find its products

It's an odd location for a shoe store, next to the old railroad bridge on Shore Road. Having thrived there for 33 years, Footmarks has become something of a landmark - and a curiosity to those who aren't among the women and children who are its customers. What it is and how it has survived against chain-store competition says a lot not only about local retail, but also about the state of the regional and national economies.

Before it was Footmarks, the store sold shoes as Hilton's Bargain Center, owner Jim VanArsdale said this week. VanArsdale was a shoe wholesaler in Philadelphia then, and when the store became available in 1976, he took the opportunity to go retail. Full Text

WSA exhibitors hone in on comfort

Women have spoken and the shoe industry has obliged: Pretty shoes are now comfortable. Or, at least their manufacturers are doing everything they can to make them so. The World Shoe Association, the bi-annual shoe and accessory convention at Las Vegas Convention Center and The Venetian, was in town, and comfort was the big trend generating buzz on showroom floors.

If you're picturing Crocs and nurse shoes, you have the wrong idea. We're talking footwear with a strong fashion focus. Velvet Angels, a new line designed by former head designer of LAMB, Joe Ponce, boasts a few nose bleed-inducing heels named after bars in L.A. The line's bowed heels, flats and kitten heels all come with silicone-padded insoles.

"For me it was a natural transition because I come from an athletic footwear background," Ponce said. "And when I started designing the line my wife said, 'Why can't these ever be comfortable?" Some of the Velvet Angels shoes, sold locally in C-level and Shooz, hurt just to look at them and could consequently struggle with comfort claims. Full Text

Payless shoe stores set for Middle East

Collective Brands Inc. is entering its franchised expansion with a foray into the Middle East as it looks for global growth opportunities.

The Topeka-based company, which owns and operates the Payless ShoeSource and Stride Rite chains, on Wednesday announced a multi-year agreement with M.H. Alshaya, a franchisee in the Middle East, to open up to 200 Payless stores in the region. M.H. Alshaya also operates retail locations for Starbucks, H&M, MAC and TheBody Shop. Full Text

UI study on diabetic foot ulcers invites participants

Adults age 18 or older with diabetes and an open sore on the foot are invited to participate in a University of Iowa research study that will help determine if diabetic foot ulcers that may not appear infected can lead to infection-related complications.

Promising new diabetes drug has a downside

Q: I was diagnosed with diabetes nine years ago. I've always had a hard time keeping my blood sugars in control. I try to watch my diet, and I know I should exercise more. I take metformin and glyburide. My doctor says my dose of both is already the highest she can give me. Rather than starting insulin, she prescribed Byetta. I heard this can damage the pancreas. Can you give me more details about the drug?

A: Exenatide (trade name Byetta) is a new type of treatment for people with type 2 diabetes. Like insulin, it must be injected, usually twice a day. But Byetta has a very different action than insulin. Rather than lowering blood sugar directly, Byetta helps the pancreas send out the right amount of insulin after eating. Other advantages of Byetta that help blood-sugar control include decreased sugar production by the liver and some delay in how quickly food leaves the stomach.

This sounds great, but the drug is expensive and there are some potentially harmful side effects. Full Text

Athletic Footwear a Ray of Sunshine in Cloudy August

As the numbers roll in for the retail fiscal month of August, it is pretty clear that retailers are looking to athletic footwear to keep footwear sales numbers in positive territory. While the later back-to-school season has clearly impacted retailers, a resurgent mall business and trend-right athletic product and better perceived value are driving strong sales growth for the sport footwear business. While retail comp store sales were estimated to be flat for the month, there is strong evidence that athletic footwear sales were up significantly for August. Full Text
