NEW QUESTIONS

Question: I have been searching for a primary taxonomy code for Ceritifed Pedorthists with no luck thus far. All healthcare providers have a primary taxonomy code specific to their profession. Orthotists, Prosthetists, Certified Orthotic Fitters and even Music Therapists apparently have a taxonomy designation.

Question: I keep seeing ABC and BOC Pedorthists. What's the difference, and what do I need to do to be both? Our days are so busy, it is hard to keep up with changes.

Question: This announcement recently came out:

Medicare Bill Includes E-prescribing Bonus

Eligible physicians can expect some financial relief courtesy of the federal government simply by adding a jolt of modern technology to their practice. Under a measure approved by the Senate, doctors who are paid by Medicare would become eligible for bonus payments just for filing prescriptions electronically rather than writing them out by hand. The e-prescribing requirement, which also covers a reporting initiative, is part of a broader Medicare bill that first cleared the House in June.

The bill allows for a stepped-down system of bonus payments, running from 2009 through 2013. At the outset, doctors would be eligible for a 2% bump in reimbursement dollars for e-prescribing, which would gradually decrease over time as the technology becomes more common and less expensive. Conversely, physicians who don’t e-prescribe would start to see a reduction in pay starting in 2012. The Senate measure also requires the Government Accountability Office to study the effectiveness of e-prescribing, culminating in a comprehensive report by late 2012.

As a pedorthist who fills prescriptions it would be a tremendous help to receive E-prescriptions. Often the patient forgets to bring it or tell us that she has a prescription. Many prescriptions are verbal and subject to patient interpretation and memory. I know that you are not thinking about pedorthic prescriptions in the same way as others: I wish you would.

Does anyone currently send or receive prescriptions this way and what system do they use?

Question: We’re in the process of expanding our retail business to a building next door; however we’re keeping the lab we’ve used. The original design did not allow for great ventilation and the typical odors and fumes permeated the retail area. The dimensions are 11x 17, we create orthotics, shoe modifications and we modify prefabs a great deal of the time. Does anyone know of -- or have a resource for -- ventilation systems for a functional pedorthic lab in a retail environment? Or for that matter, does anyone have any suggestions?

Response: You raise a great question about outgassing and ingredients in the products you use for your orthotics.

Smells associated with orthotics primarily come from the raw materials and chemicals that are used in the manufacture of the product. This, of course, is why rubber products smell like rubber, etc. In the case of foams, some will smell just like plastic (which is pretty much unavoidable) and some will smell like ammonia. Cellular products, in particular foams, are made either with chemicals or gases. Any of those ingredients will come out of these foams when they are fabricated.

This makes us wonder about the hazards of the ingredients used to make the foam. Keep in mind that you inhale the ingredients when you smell them!

Chemically expanded foams have ingredients that can be harmful when inhaled (acetophenone, methyl styrene, semicarbazide) but foams expanded using inert gases are therefore safer and will smell less as well. So, if you can't move the lab, try to use the products that will have the least impact on the environment that you have to work in. In the long run, it will be a safer proposition all together.

Response: The Fume Buster does wonders for us. I purchased that through Miami Leather.They make a 3 foot & a 4 foot unit. I also have a JDS Air Tech 2000 from JDTOOLS.COM. Between the two, you can get some serious air filtration! If you don't have a door to seal off the air in your lab from the rest of the store, look into plastic draping that is used in the food industry to try to keep the odors contained in the air filtration environment.

DIABETES

Diabetic Foot Infection

Foot infections are common in patients with diabetes and are associated with high morbidity and risk of lower extremity amputation. Diabetic foot infections are classified as mild, moderate, or severe. Gram-positive bacteria, such as Staphylococcus aureus and beta-hemolytic streptococci, are the most common pathogens in previously untreated mild and moderate infection. Severe, chronic, or previously treated infections are often polymicrobial. The diagnosis of diabetic foot infection is based on the clinical signs and symptoms of local inflammation. Infected wounds should be cultured after debridement. Tissue specimens obtained by scraping the base of the ulcer with a scalpel or by wound or bone biopsy are strongly preferred to wound swabs. Diabetes Epidemic Hits the United States

Diabetes, called by many the disease of excess, now affects nearly 24 million people in the United States, an increase of more than 3 million in approximately two years, according to new 2007 prevalence data estimates released June 25, by the Centers for Disease Control and Prevention (CDC). This means that nearly 8 percent of the U.S. population has diabetes.

"There's no simple answer as to why we?re seeing this increase. So many more people are obese and overweight. Fat is the direct cause of diabetes. Couple that with improper eating, lack of exercise and the type of food people eat," explained Dr. Jewel Crawford, of the Morehouse School of Medicine.

In addition to the 24 million with diabetes, another 57 million people are estimated to have pre-diabetes, a condition that puts people at increased risk for diabetes. Among people with diabetes, those who do not know they have the disease decreased from 30 percent to 25 percent over a two-year period. Full Text

One sport shoe will not fit all physical activities, that is. Many times I will notice a club member "fast walking" with running shoes, or performing weight- bearing lower body exercises with running shoes, tennis shoes etc. But when we wear a shoe that is not meant for an activity, it can lead to our experiencing several conditions from poor performance to injury. In addition, now that it's summer, many of us have taken our activities outdoors (running on a different surface, for instance), or we may be participating in activities we hadn't until now. What does this mean? Well, it may mean that we need to reevaluate the shoes we're wearing. Not an easy task! Full Text
