Question: We’re in the process of expanding our retail business to a building next door; however we’re keeping the lab we’ve used. The original design did not allow for great ventilation and the typical odors and fumes permeated the retail area .The dimensions are 11x 17, we create orthotics ,shoe modifications and we modify prefabs a great deal of the time . Does anyone know of or has a resource for Ventilation systems for a functional pedorthic lab in a retail environment? Or for that matter odes anyone have any suggestions?

Question: In regards to pedorthics and AFOs I am quite confused at this point. It is clear that this is a divided issue. Somewhere I was given the understanding that my scope of practice as a C. Ped. was sub-malleolar and that any involvement of the ankle itself was to be left to the CO in our office. Apparently this must not be the case across the board and I was wondering at what point C. Peds began including the lower leg into their practice. Are the lines limiting scope of practice grey and somehow left to personal interpretation? If pedorthists were always allowed to include the lower leg then I don’t understand why it wasn’t included in my Pedorthic education (including the Pedorthic exam itself)? AFOs are obviously more revenue which would be great for my practice but at what point is it to be done by an orthotist rather than myself?

Concerning the
inclusion of a pedorthic scope of practice within any certifying organization
that enables a pedorthist to cast, fit, and dispense an Ankle Foot Orthosis
(AFO)*, or Subtalar Control Foot Orthosis (SCFO)**: A certifying organization’s
purpose is to protect the public by establishing a code of ethics, standards of
practice, educational requirements, and testing of established competencies in
order to certify that an individual is competent, assuring the public of their
safety. (The certifying organization should be NCCA***
accredited.)
Question: A woman came in with pain and swelling along the course of PT tendon in tarsal tunnel. MRI confirms partial tear. Here's the tough part. She's a professional actress doing a one-woman show....on her feet several hours once or twice 6 days weekly and contracted for another year. She’s wearing a BK pneumatic walker when off stage, but how to treat her onstage? She is not willing to switch to shoe gear that will accept an AFO. Her present costume shoe is a dress shoe with 2" heel. Right now she's just using a compression dressing because she feels it helps. She is taking ultrasound since some studies do indicate it may speed healing. What pedorthic treatment can you recommend to protect the tendon?

Response: Geez,
talk about an uphill battle, it will be tough to get an effective item into the
shoe, plus, you need to be careful not to contribute to a fall off of the
heels. I suppose you could fiberglass the heel counter in the shoe and add
a strap and buckle for support. In my life before Pedorthics we utilized a
"open basketweave" ankle taping for lateral ankle sprains, I would imagine that
the horseshoe strips could be reversed, and the patient could be taped in a
plantarflexed and mildly supinated position, taking stress off the tendon during
the performances. Find a good ATC or PT in your area and make a
friend.

Response: I would recommend the FABS for her to wear on stage under the compression sock. These are small enough to fit into a heel and yet will give her significant compression and support.

Response: I use
a simple taping technique called a navicular sling using McConnell tape to
relieve some of the stretch and tension on the posterior tib tendon. I use
two strips of tape and start from the peroneal notch and pull under the foot and
up ending at the top of the instep. The strips are overlapped about 50%
and you can pull up as much as necessary and comfortable to the patient to
relieve the tendon. I think taping in this instance may help her at work
because it is very low profile and can be hidden under her stocking. She
needs to be in a lower heel if at all possible to improve the effectiveness of
the taping and by all means she should continue to have physical therapy
treatments including ultrasound/phonophoresis, and/or intophoresis directly on
the area of the tear. If you want or need any further information

Response: In
reference to your famale patient with the PT partial tear, if she has to wear a
dress shoe with 2" heel, what if you filled the midfoot (base of heel to the
ball area, externally)with medium density crepe (20-30 durometer) incorporate a
slight medial wedge to stabilize the midfoot. This is not the ideal
treatment however you would be providing some support along with the
compression dressing and not sacrificing the costume appearance at least for the
period she is on stage.

Response: In the case of gastroc-soleus equinus a stretching program would be indicated, this could be wall walking or another form of calf stretching exercise. The foot should also be protected against excessive pronatory forces and may require orthotic control and support. I would urge caution using the Z-Coil shoes as the spring action of the heel could with sudden dorsiflexion (especially in a child) lead to an Achilles tendonitis.

The Orthotic is called Carbon Fibers and they are off the shelf. Their phone number is 1800-848-7332. They can advise you as to how to determine the correct size the child will need. They are totally rigid and require very wide sneakers Xw New Balance. They enable the foot to do heel strike and toe off. They are very effective as a continuing form of heel stretch.
