Question: We have the box for Soles 4 Souls in our office. Great response from our clients, so much so that we can't afford to ship all the shoes out!!! Being a small business this is an expense beyond what we can do on a monthly basis. Any hints on how to send these without breaking the bank? I love the program and want to keep participating, help!

Question: I would like to find a central fabrication facility that does high quality custom orthosis in a timely fashion. I have not been very impressed with the quality and options of custom orthotics offered by some of the Diabetic shoe manufacturing companies. Some are little more than a good OTS orthotic. Any advice would be greatly appreciated.

Question: Like Dennis Hagan, I am having difficulty understanding the terminology "SCAFO" and what the device really is. I have looked in a textbook for orthotists and cannot find a picture of a SCAFO. Would someone who fabricates or supplies SCAFOs please email PNW a picture of one of these devices so it can be posted?

Comment: Since the last discussion on developing green practices in PNW, we see continued effort being made to improve our conservation-preservation practices. It is a challenge to sustain green practices in the current climate of personal behavior. Waste is everywhere. The shoe boxes we open are loaded with paper and plastic wrappings that mostly get thrown away. We put products we sell into shopping bags that we throw away. We drink water from bottles and cups that we throw away. Most recyclable products and materials are thrown in the trash. Soles 4 Souls has invented footwear recycling. The more shoes that people take out of their closets and distribute to the needy, the better it is for us as well as needy people.

We have developed habits of consumption over the past fifty years, ever since the addition of plastic into our lives. Plastic spoons and plates were not originally intended for disposal after using, rather a less expensive, less breakable alternative to glass. For most of us it was just an excuse not to have to wash and dry dishes and glasses, forks and knives. Consumption became a national pastime with the introduction of Pampers disposable diapers and the replacement of automobiles every two to three years beginning in the 1960’s.

Now we face it in all corners of the globe, as the Third World becomes westernized and joins the consumption society.

It is time to step up to the plate and reverse this pattern that has become a conditioned reflex for us all. I am asking that each person who reads Pedorthic Newswire be a resource to Save Our Planet Earth. This can become the theme of our profession, our own lives, and all of society. It has to start with education. We can and must include the subject in all of our education programs.

I ask each of you who read Pedorthic Newswire to reply to this plea. What have you observed? What are you doing? What behavior modification have you begun to Save Our Planet Earth. Response: Comfort Shoe Specialists recycles. We have paper recycling bins strategically placed throughout our facility. We do feel frustrated by excessive paper coming in footwear, especially those manufactured in China. While we all want our products unblemished from shipping, sturdier boxes (which also present better) with substantial protective paper would better serve this function instead of 2-3 pieces of wadded paper inside each shoe. Finn Comfort shoes do a nice job with minimal trash.

A retired Marine comes by once/month for can recycling.

We use materials conservatively to reduce cutoffs and waste, good for both landfills and COGS.

We also participate in Soles4Souls as an official drop off destination, using their logo in newsletters and promotions.

Being a conservationist of our planet is a personal goal and is worth our discussion as a group/profession. It takes effort but isn’t a Good Earth worth it?

Response: We recycle all of the "office" paper, cardboard, glass, plastic #1 and #2, magazines, newspapers and aluminum and tin cans. We have asked our vendors not to use plastic peanuts in packing merchandise - particularly non-breakable stuff. We use paper cups for the water dispenser - but don't recycle them. We use "5 year" flourescent tubes.

And I'm going to find out if the recycling company I use will take shoe boxes, and the paper, etc. that comes in them. Bob, Thanks for the nudge!

Response: I fully agree that we all need to do our part in protecting the environment. Our cumulative small actions of recycling, renewing and eliminating waste does have a major impact on the Earth.

One small example is our commitment at Hersco is reusing shipping boxes when possible. As a central fabrication facility all of our work is shipped to us through the major carriers (UPS, Fed Ex, USPS and DHL). Rather than print our logo onto our own boxes we reuse those sent in to us. We then place an 8 x 11 logo label onto the reused box. The label also includes a short explanation of why we are recycling the boxes. This program reduces the need for harvesting trees, in order that new shipping boxes can be made, and it reduces waste from our facility.

Response: Thanks for raising a very important issue the environment and how addressing environmental concerns can not only help reduce the hazards of global warming, but it can also make us more economically healthy.

We are a group of seven stores in Maine, and in 1993 we joined a group that the governor had called together to address the issue of waste management. We looked at our waste stream, and found that it was fairly narrow and focused, and that it would not take much effort to segregate it by category, and handle it properly. We sorted our carboard, our paper, our boxes and tissue paper, our plastic, our shoe sticks plastic or wood, and our gel packs. Then we set up collection areas in each of our stores, where we recycled them. We also collect worn shoes to donate to homeless shelters. The net result has been pretty much the elimination of all costs for recycling and trash removal. The carboard and various paper types are picked up be a recycler, the plastic goes back to our bag supplier, the plastic or wooden sticks go to garden centers for marking plant types (at least one more use before they are jettisoned!) and the gel packs go to people who dry flowers. We send old displays to other store types, and have donated old desks to libraries. My wife, Kathy Wellehan, set this program up, and we began recycling 95% of our solid waste, and we received the Governor’s Waste Management Award in 1994 for this effort. We have continued at this level every year since then, and the only thing that goes into our trash is food waste. This has been a tremendous expense reduction, as well as cutting the burden on our environment.

In 2003 we joined the Governor’s Carbon Challenge, and began to change our light bulbs to the most energy efficient bulbs possible, reviewed and reduced our lighting so that it was only on in areas where it was needed, and when needed. We put sensors in areas that are used irregularly. We also changed our winter heating requirements to 68 degrees from 72, and our summer cooling requirements to 75 degrees. We have used bio-diesel in our truck, and moved our two company vehicles to Priuses. Every use of energy is questioned. This has resulted in a decrease in our greenhouse gases of around 11%, and a corresponding drop in what our energy bills would have been. We have also switched our shoe bags from being oil based plastic to vegetable based plastic, so that they will degrade within a year. Of course, no bags is a better idea, and some of our customers prefer this. We were honored for this work by Efficiency Maine, and named as the Business Leader of the Year by the Maine Chapter of the Sierra Club. I have spoken to a variety of gatherings on both the need and the way to cut energy use, and build a healthier new world and healthier new economy, and am involved in the Cool Communities Movement, sponsored by the Sierra Club (aka Cool Cities). We are helping our towns and townspeople to improve their energy efficiency, and there is great progress being made in Maine on this front. Many people are moving in this direction, and it is one of the most easy propositions that I have ever been involved in trying to implement.

Question: I have been hearing more and more certified pedorthists lately talking about doing AFO's. I know that some SCAFO's do fall into the scope of practice of a certified pedorthist. I have also been taking an "informal survey" over the last few months of recently certified C. Peds who I meet at meetings and courses and it seems to me that few - if any - schools are actually teaching the skills and knowledge necessary for a practitioner to be qualified to provide SCAFO's or AFO's. I am confused as to how we can say we should provide SCAFO's and AFO's when the educational foundation is not in place. Am I wrong in my line of thinking here?

Reponse: I would also like to add my two cents worth to the previous discussion initiated by Mr. Erick Janisse’s comments and concerns regarding educational preparation of Certified Pedorthist (qualifying them to fit SCAFOs and AFOs). As a result of Mr. Janisse’s self conducted, informal survey, he makes the assumption that few if any schools are teaching the skills and knowledge necessary to qualify a practitioner to provide SCAFOs and AFOs."

First I would like to remind Mr. Janisse that schools may teach, but knowledge is the understanding acquired through experience, association and / or educational processes . I have been in the medical field many years (prior to becoming a licensed C. Ped.) and have observed a wide range of knowledge amongst individuals within each scope of practice. Schools may present well-balanced curriculums with both broad range and focused, in-depth teaching, but if a student is not truly passionate about learning all that they can, that student will never be an honest representation of the education that was offered. I wonder if the participants of Mr. Janisse’s survey might be that type of student or maybe their Pedorthic interest was focused elsewhere, so they were never really motivated to learn SCAFOs/AFOs. All that any school can really do for their students is to lay a strong foundation. It is then up to the student to continue the building by pursuing further educational opportunities.

While I can’t attest to other school’s curriculum, I can assure Mr. Janisse that OSU (in particular, Rick Sevier) does a fine job of basic preparation of their Pedorthic students to evaluate and fit for SCAFOs and AFOs (considering the limited length of all Pedorthic programs). I understand that many do not agree with me, but I would love to see an advanced practice program/certification developed. Lack of adequate knowledge is not exclusive to a few C. Peds. I know of several CO/CPs that were grandfathered in as C. Peds but are honest enough to tell you that they know very little about feet. Some CO/CPs appear to have very little knowledge of disease processes, along with the inherent implications and risk involved with bracing issues. While I do wonder how prevalent these situations are, I would never presume that they are commonplace. I would remind Mr. Janisse of the old adage about what happens when one assumes."

Response: I would also like to add my two cents worth to the previous discussion initiated by Mr. Erick Janisse’s comments and concerns regarding PFA has sent pedorthists an urgent message to contact senators to vote for the Medicare Improvements for Patients and Providers Act (have to love how politicians can dress up legislation with a title that sounds like it will help orphans and widows when in reality it takes food off their table and puts bucks in someone else's pocket). The reason given by PFA is that it delays implementation of competive bidding for several years. Since TSD benefits and custom AFOs are not included under competitive bidding, then what items that a pedorthist might supply are affected by competitive bidding? Prefab orthotics, wheelchairs, items of that nature. How much of your business is custom AFOs and TSD footwear versus prefab?

Is competitive bidding the best reason to encourage pedorthists to call their senators and ask them to vote for this legislation? Maybe not.

Factor in one other issue that is supposed to be part of this bill.

The APMA has sent an urgent message to its membership to contact their senators to vote FOR this bill because it includes "exempting podiatric physicians and other licensed health care providers from DMEPOS supplier accreditation requirement."

So if you are a pedorthist that supplies Medicare TSD or AFO benefits, you will have to incur the initial and ongoing expense of accreditation while the podiatrist down the street that is no supplying TSD benefits to beneficiaries that you used to would not. Seems fair? Does not seem right?

I will be contacting my two senators to tell them that there is no logical reason that one supplier should be held to lower standards than another supplier. Accreditation is supposed to improve quality of care and reduce Medicare fraud. From what I see in my community accreditation should be used across the board with all suppliers. Question: I would appreciate a suggestion on how to treat a condition called equino varus or tip toeing. My son has been walking on tiptoes since he started walking at 12 months. I was told not to worry about it for at least several years. His condition didn’t change so I took him to a pediatric orthopedist at age 5.

It turned out he was born with tight achilles and very little ankle dorsi flexion. I was told by the orthopedist that if he doesn't walk normally by age 9 or 10 surgical corrections would be an option. He loves to play sports and it requires a lot of running around. He is now 12 years old.

At age 5 or 6 he wore a night splint with 5° dorsiflexion to stretch the achilles, a SMO at age 6, then hinged AFO, did some physical therapy and exercises but the problem had not been resolved. He’s done much better since then and tries to use his full foot but every now and then I can still see him come up on his toes whether he is wearing high-top sneakers or barefoot.

Is there another device/appliance out there I don’t know about, or perhaps a certain shoe that can keep the foot down. After all, I am trying to achieve a heel-to-toe walking pattern. Any suggestions will be helpful.

Response: I agree with James [response in issue # 266] that the child should be shown to a neurologist first. Also during evaluation it shoud be confirmed wether the child has spasm and fixed deformity, in that cases an orthoses should be avoided. We have noticed that relaxation reduces spasm, and practicing relaxation exercises with a good shoe insert really works.

The inside-out economics of shoes and places that sell them

My new running shoes are too tight. They felt fine in the store, but a month of jogging in them has confirmed they were meant for someone with narrower feet. Because this is not the first time I've been stuck with the wrong shoe, I'm studying to become a shoe salesman. Whatever knowledge was required to hold a job selling shoes during the 1960s, that's what I want. Those guys knew what they were doing.

The last time my feet were handled by someone who works in a shoe store was about 1972. That's roughly when the shoe industry began to recognize that if Americans were willing to pump their own gasoline, they'd fit themselves with footwear, too. Anyone under 30 and reading this is probably thinking, "Let a shoe store employee handle your feet? That's just weird."

Yes, kids, believe it or not there was a time you could shop for shoes in this country without ever bending over. But that concept is about as unimaginable for today's young adults as $3-per-gallon gasoline will be for their children. Full Text

DIABETES

Diabetes hospitalizes increasing numbers of U.K. children

Increasing numbers of children are being admitted to hospital for emergency treatment for diabetes. Official figures show that more than 3,300 young people in England were taken to hospital accident and emergency departments because of the condition last year, an increase of 8 per cent on the previous 12 months.

Diabetes charities described the figures as "shocking" and called on the health service to take urgent action to help more children manage the disease. Full Text

Online Purchasing Also on the Rise

The NPD Group, Inc., a leading market research company reports the European athletic footwear market is seeing the biggest growth in ‘Leisure & Classics’ or low performance athletic shoes, sneaker-type shoes designed primarily for fashion and style rather than pursuit of a specific sport. Annual 2007 figures for Europe’s Big 5* market show that sales of athletic footwear used for leisure grew by 2% while sales of athletic footwear used for sport declined 5%.
