Comment: Since the last discussion on developing green practices in PNW, we see continued effort being made to improve our conservation-preservation practices. It is a challenge to sustain green practices in the current climate of personal behavior. Waste is everywhere. The shoe boxes we open are loaded with paper and plastic wrappings that mostly get thrown away. We put products we sell into shopping bags that we throw away. We drink water from bottles and cups that we throw away. Most recyclable products and materials are thrown in the trash. Soles 4 Souls has invented footwear recycling. The more shoes that people take out of their closets and distribute to the needy, the better it is for us as well as needy people.

We have developed habits of consumption over the past fifty years, ever since the addition of plastic into our lives. Plastic spoons and plates were not originally intended for disposal after using, rather a less expensive, less breakable alternative to glass. For most of us it was just an excuse not to have to wash and dry dishes and glasses, forks and knives. Consumption became a national pastime with the introduction of Pampers disposable diapers and the replacement of automobiles every two to three years beginning in the 1960’s.

Now we face it in all corners of the globe, as the Third World becomes westernized and joins the consumption society.

It is time to step up to the plate and reverse this pattern that has become a conditioned reflex for us all. I am asking that each person who reads Pedorthic Newswire be a resource to Save Our Planet Earth. This can become the theme of our profession, our own lives, and all of society. It has to start with education. We can and must include the subject in all of our education programs.

I ask each of you who read Pedorthic Newswire to reply to this plea. What have you observed? What are you doing? What behavior modification have you begun to Save Our Planet Earth. PREVIOUS QUESTIONS WITH RESPONSES

Question: With the diversity that exists in how certified pedorthists "practice" their profession, should there be sub-designations or even different credentials to show the type of pedorthic practice that an individual wants to focus? Physicians have cardiologists, endocrinologists, rheumatologists, etc.. Pedorthists have retailists, foot orthotists, TSDists, etc. but when a search is done on ABC's website, a customer has no way of knowing if a CPed does TSD footwear, orthotics, shoe mods, AFOs, retail footwear or some or all. A Certifed Prosthetist obviously does prosthetic services. A Certified Orthotist one would think does not specialize in just neck braces or knee braces or back braces or foot orthotics, so they have no specialization. Only CPeds seem to limit their scope of practice to specific services. Does this create a need to be CPed (cfo) or CPed (ret) or CPed (tsd)?

Response: I strongly agree with you. This may be a topic for another discussion, but it has been my experience that there are a growing number of retail stores wanting C. Peds to be part of their staff. However many of these retail stores do not fabricate orthoses or do shoe mods and may do only a very limited number of customized 'footbeds'..the C. Ped. strictly fits and sells shoes and whatever other products the store sells (just like all the 'normal' staff members) and the retail store 'uses' their credentials to make them look better and has no clear understanding of what a C. Ped. really is or what an experienced C. Ped. is capable of doing.

Bike shops are hiring C. Peds, shoe stores are hiring C. Peds. What percentage of the time are these C. Peds actually functioning as C. Peds verses standing around a store or behind a register? I know this is a problem because it's happened to me personally. Businesses outside the industry don't necessarily have a realistic view of what skills an experienced C. Ped. possesses, and yet more and more retail stores seem eager to hire us for our credentials...without offering us the right environment to use them in.

I guess what I'm trying to say is, there should be a new category called a 'Retail C.Ped.', and one for a 'Clinical/Fabrication C.Ped.' The two are entirely different and the compensation should be different. There is alot more to Pedorthics than fitting and selling shoes or selling a product. I think we need to make a delineation between types of C. Peds because there currently exists many different types of C. Peds....some whose skills and passions are limited to shoe fitting/selling in a retail setting, and others with fabrication skills, shoe fitting skills, shoe mod skills, in addition to more medical/clinical skills, experience and backgrounds.

I strongly suggest that we create categories to differentiate types of C. Peds and also help to educate the public more regarding the differences.

Question: I have been hearing more and more certified pedorthists lately talking about doing AFO's. I know that some SCAFO's do fall into the scope of practice of a certified pedorthist. I have also been taking an "informal survey" over the last few months of recently certified C. Peds who I meet at meetings and courses and it seems to me that few - if any - schools are actually teaching the skills and knowledge necessary for a practitioner to be qualified to provide SCAFO's or AFO's. I am confused as to how we can say we should provide SCAFO's and AFO's when the educational foundation is not in place. Am I wrong in my line of thinking here?

Previous response: In response to the question concerning "SCAFO's??? and AFOs, I find the gilded comments leading up to your question disturbing. However the simple answer to your question is "YES."

New response: In specific and direct response to Mr. Hagan’s comment in last week’s Pedorthic Newswire: Thank you for your answer to my question. I understand that you feel I am off the mark in my sentiment that pedorthists may need more education if they are going to properly fit and fabricate SCAFO’s and AFO’s. You are certainly not alone in your stance. However, I have to assume that the three question marks after the word SCAFO in your response indicate that you are unsure as to exactly what a SCAFO is. So, while I am sure it was not your intent, I greatly appreciate your help in proving my point.

New response: As an pedorthic pre-certification educator, I can assure you that we (Oklahoma State University, among other schools) include a module on SCFO’s and AFO’s during pre-cert classes in order to maintain compliance with CAPE, ABC and BOC standards. This does not include fabrication, but does include basic competencies required to cast for, indications for, ordering through central fab and fitting of these devices. There has been an advanced class in AFO fabrication hosted by at least 2 pedorthic schools (including OSU) and one private company within the past couple of years. Should there be more emphasis on AFO’s and training opportunities? Yes! And for those Pedorthists (and a few $75 crackerjack crossover COs) who feel they need a bit more AFO training, just let us know and the pedorthic educators (with some assistance from our O & P brothers and sisters) will be more than happy to provide classes.

On a personal note: I think that a few individuals who think highly of themselves conducting informal surveys are becoming an accepted malignancy infecting, affecting and eventually leading to the death or disfigurement of our profession. We can either be frogs in a pot of water, temperature slowly rising to boiling, or be practitioners who drive a stake in the ground and proclaim here, and no farther. Anyone with me?

Question: I would appreciate a suggestion on how to treat a condition called equino varus or tip toeing. My son has been walking on tiptoes since he started walking at 12 months. I was told not to worry about it for at least several years. His condition didn’t change so I took him to a pediatric orthopedist at age 5.

It turned out he was born with tight achilles and very little ankle dorsi flexion. I was told by the orthopedist that if he doesn't walk normally by age 9 or 10 surgical corrections would be an option. He loves to play sports and it requires a lot of running around. He is now 12 years old.

At age 5 or 6 he wore a night splint with 5� dorsiflexion to stretch the achilles, a SMO at age 6, then hinged AFO, did some physical therapy and exercises but the problem had not been resolved. He’s done much better since then and tries to use his full foot but every now and then I can still see him come up on his toes whether he is wearing high-top sneakers or barefoot.

Is there another device/appliance out there I don’t know about, or perhaps a certain shoe that can keep the foot down. After all, I am trying to achieve a heel-to-toe walking pattern. Any suggestions will be helpful.

Response: I don't disagree with the other respondents' suggestions regarding your son, but I would like to add that a visit to a good neurologist might be in order. In my opinion, certain developmental and/or brain function anomalies (sometimes associated with tip-toeing) should be ruled out before proceeding with other therapies. Soles4Souls Calls for Shoe Donations for Flood Victims

Soles4Souls is making a plea to footwear companies and the public to donate shoes to flood victims in the heartland.

We are asking our footwear partners and consumers across North America to help us reach out to victims in Iowa, Illinois and Indiana, founder and CEO Wayne Elsey said in a press release. Unfortunately, we can never predict when natural disasters like this week’s flooding in the Midwest will occur. We need individuals, shoe retailers and footwear brands to dig deep into their closets and stockrooms to help us get shoes on the feet of people to help them rebuild their communities.

Dave Lorency, president of Operation Compassion, a disaster relief agency, echoed the necessity for footwear donations. Getting a new pair of shoes will be a huge relief to a lot of these people, he said. Flood water ruins everything it comes into contact with, and trying to rebuild communities without decent shoes is next to impossible.

Bob Schwartz, CEO of Eneslow, The Foot Comfort Center in New York City is partnering with Soles4Souls to distribute the shoes to the needy. We ask our customers, neighbors and the community to drop off the shoes they never wear to either Eneslow location. We will act as a drop-off point and send the shoes to Soles4Souls for distribution to those in need. Eneslow store locations are 470 Park Avenue South @ 32nd Street in Manhattan and 254-61 Horace Harding Expressway in Little Neck, Queens.

Already, Soles4Souls has arranged to send 25,000 pairs of shoes to the affected three states, slated for a June 27 delivery. Information on donating shoes or cash to flood victims can be found at the Website Giveshoes.org.

What germs are riding on your shoes?

Do you ever consider the dirt on your shoes, which carry hundreds of thousands of germs and bacteria? Just what are you stepping in? On average, 421,000 units of bacteria reside on the outside of a shoe, and 2,887 bacteria units can be found on the inside, a study by the University of Arizona and The Rockport Company found.

Some of the bacteria found include those that cause intestinal and bloodstream infections, bacterial meningitis and pneumonia. Full Text

Ex-TBL Employee Buys GoLite Footwear

GoLite is back. Just over a month after Stratham, N.H.-based Timberland Co. announced it would shutter the trail racing shoe brand, GoLite has been bought by New England Footwear, a newly formed company headed by Doug Clark, former head of Timberland’s Innovation Factory. New England Footwear, also based in New Hampshire, was created specifically to make the purchase.

Terms of the deal were not immediately disclosed, but in purchasing the GoLite footwear business, New England footwear has obtained the rights to existing designs, all excess inventory and has the ability to make and market new styles under the name. Full Text

Australian researchers have suggested a strong relationship between obstructive sleep apnea (OSA), snoring and type-2 diabetes. According to the International Diabetes Federation (IDF), 40 per cent of patients with apnoea suffer from diabetes, while about 23 per cent of diabetics have apnoea.
