NEW QUESTIONS AND COMMENTS

Question: I have been hearing more and more certified pedorthists lately talking about doing AFO's. I know that some SCAFO's do fall into the scope of practice of a certified pedorthist. I have also been taking an "informal survey" over the last few months of recently certified C. Peds who I meet at meetings and courses and it seems to me that few - if any - schools are actually teaching the skills and knowledge necessary for a practitioner to be qualified to provide SCAFO's or AFO's. I am confused as to how we can say we should provide SCAFO's and AFO's when the educational foundation is not in place. Am I wrong in my line of thinking here?

Question: I would appreciate a suggestion on how to treat a condition called equino varus or tip toeing. My son has been walking on tiptoes since he started walking at 12 months. I was told not to worry about it for at least several years. His condition didn’t change so I took him to a pediatric orthopedist at age 5.

It turned out he was born with tight achilles and very little ankle dorsi flexion. I was told by the orthopedist that if he doesn't walk normally by age 9 or 10 surgical corrections would be an option. He loves to play sports and it requires a lot of running around. He is now 12 years old.

At age 5 or 6 he wore a night splint with 5° dorsiflexion to stretch the achilles, a SMO at age 6, then hinged AFO, did some physical therapy and exercises but the problem had not been resolved. He’s done much better since then and tries to use his full foot but every now and then I can still see him come up on his toes whether he is wearing high-top sneakers or barefoot.

Is there another device/appliance out there I don’t know about, or perhaps a certain shoe that can keep the foot down. After all, I am trying to achieve a heel-to-toe walking pattern. Any suggestions will be helpful.

Question: Are ABC Certified Pedorthists aware that by cross-credentialing to become a BOC Pedorthist you can then increase your scope of practice to include the supply of custom AFOs? The BOC Pedorthist scope of practice is defined as pedorthic devices used below the knee.

Comment: Like Jim Murray recently posted and other respondents earlier have done to voice their concerns about individuals disguising themselves with their first name and initial for their last name (or aliases), I have had a similar reaction about the anonymous posting of questions or responses to Pedorthic Newswire.  However, after talking with the editor of the PNW (Faye) about this issue, I think that there are some reasons that the policy of accepting anonymous questions should be continued.   Anonymous responses, now that is a different matter in my opinion.

One reason that a person may want to remain anonymous is that the person may want to pose a question that shows their inexperience or lack of knowledge about the subject matter and is somewhat fearful to put their full name out there. By allowing anonymous questions, PNW may get more questions about basic (or not so basic) issues that need addressing.  Not everyone can feel comfortable (or brave enough) about asking a question to such a large and varied experience level audience as the 5,000 individuals who are now emailed PNW.

Because some of the responses to questions or issues raised can be fairly pointed (brutally honest?) by some respondents, there appears to be a certain percentage of individuals who want to remain anonymous to avoid taking a personal pounding on what others may see as a "dumb" question. I personally do not think there are many dumb questions, and our field is built on an apprecticeship learning process or inexperienced individuals learning from hopefully more knowledgeable individuals.

Most individuals that participate with PNW are secure enough about their opinion or knowledge base so as to want to put their full name and rank out there and take whatever pointed dialogue that then occurs as part of life and living in a world that has lots of diversity of knowledge and opinions.  Either way (anonymous or full name and rank), I think it is better for the PNW community as a whole to encourage questions by anyone who is willing to take the time to submit an email so that more information is shared, more opinions are voiced, and the group as a whole is challenged to think and to become more informed about issues (small or great) that impact our community. Having given this opinion, I guess the anonymous responses bug me more than the anonymous questions. Particularly when a response is so blantantly wrong.

Response: I found a great resource for facility accreditation. Check out http://www.facilityaccreditation.com/. It has several forms, manuals and other materials for facility accreditation. It is targeted towards pharmacy, podiatry and DME, but has a great list of what logs and documents are required. Most are available for download for a pretty reasonable fee. They even have a tutorial and mock inspection available.

Question: A patient with idiopathic neuropathy with a recurrent non-healing ulcer sub 2nd metatarsal head is referred for footwear and orthoses. The ulcer had healed briefly after total contact casting but came back quickly while wearing a shoe. The 2nd metatarsal is slightly elongated when compared to the remaining metatarsals. What are some of the key pedorthic modalities to help heal the ulcer and prevent recurrence?

Response: In response to the foot ulcer under 2nd met head: I would also suggest the patient have an ankle-brachial index (ABI) done. It's quick very inexpensive diagnostic test. It is performed in most vascular clinics. The ABI is a comparison of foot systolic to arm systolic pressure. The ankle systolic is divided by arm systolic to give us an index. Normal is a one (1). Anything less than a result of one should be result in a peripheral vascular consult. Your patient may be having a vascular problem as well as a nervous system problem. No amount of off-loading will prevent future ulcerations without proper blood circulation to the foot.

Response: Everybody has come up with a lot of explanations as to accommodating the ulcer. I had a very similar situation. Patient would have an ulcer under the third MTP, doctor would de bride the wound and put in a cast to give it a chance to heal. As soon as the patient came out of the cast and started weight bearing the ulcer would come back. Most of the recommendations previously stated had been tried.

I checked the mans range of motion at the subtaylor and found that the foot was plantar flexed about 5 to 10 degrees. Upon weight bearing the forces and pressures in the wound area only returned and the foot re-ulcerated. This kept occurring for two years. There was minimal forces under the heel and nearly 99 % pressure on the front of the foot. I placed 1/2 inch heel lift in the shoes bi lateral and placed the patient on an aggressive stretching program to stretch out the Gastric muscle. Within two weeks the ulcer had begun to scab over. In 30 days I began to take the lifts down with improved ROM. Once normal ROM is obtained we will build another FO that will begin to correct the mechanical breakdown which is adding to the ulceration. We may also add a rocker sole to the shoe for added relief of the MTP's. In order to get the pressure off the front of the foot we must first get it back on the heel. Never overlook the ROM issue.

Question: Any advice on getting correct sizes when you keep no stock?

Response from original writer: I thank each of you for your response to my question regarding fitting without stock. I feel vindicated! I work for a fairly new O,P&P owned and managed by a well-intentioned but totally clueless man who has no medical experience whatsoever. We have a teeny tiny store, stocked full of whatever he determines will make us the most money. He spends thousands of dollars on courting physicians with luncheons, etc. yet he expects his staff to fit patients with every kind of foot issue off two small racks of single shoe samples, all wide. I wont name the brand, but they are cookie cutter lasts. He chose these based on the "deal" he got
with the company. Here is a typical scenario...the patient picks a shoe out of the catalog. We have nothing similar to size her with. We order based on the Brannock measurements. It comes in and the shoe isn't right for that patient's foot. When we have to send the shoe back, because it did not fit he is not happy.

I have been a nurse for many years prior to getting my CPED. This has really given me an understanding of the pathology of so many foot conditions. I worked a lot with rehab and therapy, so I have a passion for biomechanics. I market outside accounts and have developed some great physician refferals as well as loyal nursing home accounts. I realize now that joining this company as my first pedorthic job was a mistake. I needed a mentor as well as a company that could provide me the tools I need to take care of my patients as I know I should. I was the little girl in braces and therapeutic shoes, so I have a true passion for this work.

Now I just need to find a new job where I can develope my skills. I would proudly sign my name and credentials were it not for the fact that I don't want to indicate the name of the company I work for. It is easy to make the connection through ABC. Anybody hiring?

Foot care starts even before baby walks

When 6-year-old Elizabeth Morrison stepped on a ceramic figurine and cut the heel of her foot last July, at first her mother wasn’t too concerned. Although Elizabeth walked on the ball of her foot for a few weeks, it didn’t appear that any of the figurine was missing, and the foot eventually healed, said Elizabeth’s mother, Darcy, who lives in Voorheesville.

“In the fall, we noticed there was a little bump there. So we took her to her pediatrician who recommended that we take her to a podiatrist,” Morrison recalled.

Dr. Mark Friedman, president of the Northeast Division of the New York State Podiatric Medical Association, sees children every day in his Albany office. Friedman said the bulk of what he sees in kids under 18 is flat feet, ingrown toenails, plantars warts and stress fractures from sports injuries. Full Text

Diabetic Woman Sues Over Pedicure

A Central Florida woman is suing over a $25 pedicure that cost her two toes and saddled her with $67,000 in medical bills, according to a lawsuit.

Lynn Hulland, 58, of Melbourne-- who as a diabetic is susceptible to skin ulcers from even minor wounds -- is demanding a trial and unspecified monetary damages, alleging that Regal Nail at Viera permanently disfigured her, Local 6 News partner Florida Today reported.

Hulland visited the salon at least once a month for three months, each time insisting that a manager made sure the nail technician understood that she has diabetes and that a razor couldn't be used on her feet, the woman's attorney, Sammy Cacciatore said. Full Text

Record Breaking Finish for Olympic Hopeful Gabriela Trana

Costa Rican Olympic hopeful, Gabriela Trana wins the Fargo Half Marathon with a record-breaking finish. Leading the race with a comfortable six-minute lead, Trana finished in just over 1 hour and 20 minutes, followed by last year’s winner Jennifer Hess in 1:26:07.

Gabriela Trana is a 28-year-old seeking to gain a spot in the Costa Rican team for the Beijing 2008 summer Olympics. Last year, Gabriela won her debut marathon in the Walt Disney World race with a bold move from fourth place to first within the last four miles. With Gabriela’s realization of her long distance ability after winning the marathon, she was ready to prepare for the Olympics.
