Response: In response to the foot ulcer under 2nd met head: I would also suggest the patient have an ankle-brachial index (ABI) done. It's quick very inexpensive diagnostic test. It is performed in most vascular clinics. The ABI is a comparison of foot systolic to arm systolic pressure. The ankle systolic is divided by arm systolic to give us an index. Normal is a one (1). Anything less than a result of one should be result in a peripheral vascular consult. Your patient may be having a vascular problem as well as a nervous system problem. No amount of off-loading will prevent future ulcerations without proper blood circulation to the foot.

Response: Everybody has come up with a lot of explanations as to accommodating the ulcer. I had a very similar situation. Patient would have an ulcer under the third MTP, doctor would de bride the wound and put in a cast to give it a chance to heal. As soon as the patient came out of the cast and started weight bearing the ulcer would come back. Most of the recommendations previously stated had been tried.
I checked the mans range of motion at the subtaylor and found that the foot was plantar flexed about 5 to 10 degrees. Upon weight bearing the forces and pressures in the wound area only returned and the foot re-ulcerated. This kept occurring for two years. There was minimal forces under the heel and nearly 99 % pressure on the front of the foot. I placed 1/2 inch heel lift in the shoes bi lateral and placed the patient on an aggressive stretching program to stretch out the Gastric muscle. Within two weeks the ulcer had begun to scab over. In 30 days I began to take the lifts down with improved ROM. Once normal ROM is obtained we will build another FO that will begin to correct the mechanical breakdown which is adding to the ulceration. We may also add a rocker sole to the shoe for added relief of the MTP's. In order to get the pressure off the front of the foot we must first get it back on the heel. Never overlook the ROM issue.
