PREVIOUS QUESTIONS WITH RESPONSES

Question: A patient with idiopathic neuropathy with a recurrent non-healing ulcer sub 2nd metatarsal head is referred for footwear and orthoses. The ulcer had healed briefly after total contact casting but came back quickly while wearing a shoe. The 2nd metatarsal is slightly elongated when compared to the remaining metatarsals. What are some of the key pedorthic modalities to help heal the ulcer and prevent recurrence?

Response: I would suggest sending her back to her orthopedist and having a dorsal oblique sliding 2nd metatarsal neck osteotomy performed. It can be secured with a Snap-Off screw from Wright Medical or Arthrex. You're battling pressure from "within" and you won't always win with just an orthotic. Good Luck.

Response: Your question is too generic, there are too many parameters that have to be filled in to answer your question. For example we need the weight of this patient. What is the foot structure like? For example, what is the arch height? Is the first ray shorter than the 2nd? Any hammer toes that may contribute to met pressure? Is there flexibility in the metatarsals? Do the patient's feet sweet excessively or are the feet very dry? What is the age of the patient? The shoes that were worn that caused the second break down -- what were they? Just some basic --- OTS shoes, or were they measured and fitted?

Yet a response to your question is: the basic modalities you probably are already aware of, such as using met pads, cut outs, rocker soles and - proper evaluated foot orthotics with appropriate material with proper padding. Reduce shear pressure using shear band.

Response: Robert, I have dealt with the same issue many times over the years. You say the 2nd met is elongated, is this noted from your cliinical exam or radiographs or by history. The best eval for lesser met length is an MO (medial oblique) radiograph. The best clinical eval is the prominent 2nd MTPJ plantarly, localized swelling beneath the met head (effusion), and a medial drift to the digit with mild hammering of the toe (loss of the capsular integrity and lever arms of the intrinsic muscles).

The ulcer sub 2nd in this situation is often seen with a short or hypermobile 1st met or an elevated 1st ray. Biomechanical approaches would need to address the 1st met elevatus (functional) and overloading of the 2nd MTPJ. I have had some success with loading the 1st ray, and unloading the 2nd as well as a plantarly applied met bar with a dorsal multi-density plastazote-PPT to float the 2nd MTPJ. This will need to be monitored for wear about every 100 miles or 3-4 months. The best approach is to shorten the long/prolapsed 2md met via an osteotomy (Wiel) if the patient can tolerate the surgery which would allow for easier accommodation.

I know there are many approaches to accommodating this problem; these are some ideas that have benefited my patients. I really enjoyed meeting you at Clearwater Beach and visiting with you.

Question: Any advice on getting correct sizes when you keep no stock?

New response from original writer: I thank each of you for your response to my question regarding fitting without stock. I feel vindicated! I work for a fairly new O,P&P owned and managed by a well-intentioned but totally clueless man who has no medical experience whatsoever. We have a teeny tiny store, stocked full of whatever he determines will make us the most money. He spends thousands of dollars on courting physicians with luncheons, etc. yet he expects his staff to fit patients with every kind of foot issue off two small racks of single shoe samples, all wide. I wont name the brand, but they are cookie cutter lasts. He chose these based on the "deal" he got
with the company. Here is a typical scenario...the patient picks a shoe out of the catalog. We have nothing similar to size her with. We order based on the Brannock measurements. It comes in and the shoe isn't right for that patient's foot. When we have to send the shoe back, because it did not fit he is not happy.

I have been a nurse for many years prior to getting my CPED. This has really given me an understanding of the pathology of so many foot conditions. I worked a lot with rehab and therapy, so I have a passion for biomechanics. I market outside accounts and have developed some great physician refferals as well as loyal nursing home accounts. I realize now that joining this company as my first pedorthic job was a mistake. I needed a mentor as well as a company that could provide me the tools I need to take care of my patients as I know I should. I was the little girl in braces and therapeutic shoes, so I have a true passion for this work.

Now I just need to find a new job where I can develope my skills. I would proudly sign my name and credentials were it not for the fact that I don't want to indicate the name of the company I work for. It is easy to make the connection through ABC. Anybody hiring?

The foot has 26 bones, 33 joints, 107 ligaments and 19 muscles. It's easy to see why taking care of our feet is one of the most important things we can do, and that a lot can go wrong with them if we don't.

"Your feet are the base of you. They're your support," says Sandy Connery, a certified pedorthist at FootHealth Centre. "It's rare to find someone who doesn't have foot problems at some point in their lives." Connery says the most common types of unhealthy feet she sees are those of folks with arthritis and diabetes, and those presenting with heel pain, caused by conditions such as plantar fasciitis. Put your best foot forward; Taking good care of your feet can help prevent other health problems

Proper foot care can help people stay on their feet, according to Joan Cullihall. The nurse with the Corner Brook Branch of the Victorian Order of Nurses (VON) says there are many health problems linked to the improper care of one’s feet, even more aches and pains throughout the body can be attributed to foot problems.

A major hurdle in the care of one’s feet is the general lack of knowledge and understanding of the importance of proper care and treatment — the reason for the neglect shown towards the part of the body that takes a person everywhere they go. Full Text

Insulin Seen Faster In 'Normalizing' Blood Sugar Levels, Shows Study

Insulin was found to more effectively control blood sugar levels than pills in dealing with diabetic patients during a study.

A research sponsored by Novo Nordisk A/S and Roche Holding AG and the government of China showed that in less than six days, 95 percent of insulin-user diabetics went back to their normal blood sugar levels. Those who took the metformin, the generic name of Glucophage pill of Bristol-Myers Squibb Co., reached their normal level after 9.3 days. Full Text

Metabolic syndrome a useless diagnosis

Diagnosing metabolic syndrome in elderly patients is of little if any clinical value, according to an analysis of two large prospective studies. Published online in the Lancet this week, both independent studies found that a diagnosis of metabolic syndrome was not predictive of cardiovascular events.

There was no increased risk conferred by such a diagnosis over and above that associated with individual components of the syndrome. And, in fact, the researchers said, Framingham-based algorithms were more likely to predict the risk of developing future cardiovascular disease than diagnosing metabolic syndrome. And even though such a diagnosis was associated with an increase risk of developing diabetes, it was no more predictive than a fasting plasma glucose. Full Text

Adidas Settles Suit Against Kmart Over Striped Shoes

Adidas AG settled its trademark- infringement lawsuit against Sears Holdings Corp.'s Kmart unit, just two days after the German apparel maker was awarded $304.6 million in a similar case over stripes on shoes.

Notice of the settlement was filed yesterday in federal court in Portland, Oregon, and a trial scheduled for July was canceled. Kmart was accused of copying Adidas's three-stripe design motif by selling athletic shoes with sets of two and four stripes. Details of the accord weren't available. Full Story
