Comment [Editor's Note: This comment did

here]: As the Kentucky General Assembly comes to the end
of the 2008 session, it seems that the efforts to protect not only the pedorthic
profession but all of OP&P from the kind of restrictive practice act that
exists in Ohio were successful.
The practitioners in the State of Alabama refused to acknowledge
imaginary boundaries and set a precedent with clear definition of a pedorthic
device that reflects actual work being performed by practitioners. This
definition is supported by the BOC pedorthic scope of practice, the PFA,
Pedorthic educators at Temple University and Oklahoma State University. In all
fairness, without being in touch with other educators, I am unsure where they
stand on the issue or the depth of their pedorthic education programs.
Regardless, it is time to move forward. With mandatory facility
accreditation right around the corner, practitioners need to consider what is in
their best interest for the long run. BOC is certified by the National
Commission for Certifying Agencies (NCCA) and accredited by their parent, the
National Organization for Competency Assurance (NOCA). BOC is recognized by CMS
as one of the ten organizations that have been awarded "deemed status for
accreditation. And with transparent, democratically elected leadership BOC is
the logical choice.
It is critical that practitioners are proactive in
maintaining hard earned ground to protect the quality care that patients
deserve. All of us do not live or operate in urban areas and patient access to
qualified practitioners is an ethical issue.
I encourage everyone to take a look at the practice act
making it's way through the General Assembly in Pennsylvania. This seems to be
the only act introduced to date that isn't burdened with regulations and that
really does put patient care and access up front. This doesn't need to be a
fight. There are federal guidelines to consider and CMS sets the standards that
everyone uses for reimbursement. There is no ethical reason to supersede those
guidelines.
I encourage everyone to contact BOC for cross credentialing
and facility accreditation.

Question: I saw the response regarding having
fitting room to dispense diabetic shoes and inserts. It seems that we are
becoming stricter on selling and dispensing diabetic shoes. How is this
going to affect companies that sell these items and ship them straight to the
person's house? Will they still be able to do this? It is my
understanding that if I see a diabetic patient I must then again see them to
pick up inserts I made or shoes I ordered for them. But I cannot send the
item to patient. Yet there are companies that never see the client only
the cast and then ship completed items. The information that I get is very
confusing. Does anyone know the straight answer?

Question: I have been in pedorthics for 17 years, the last
six of which I have owned my own facility and lab. I work hard and have expanded
my lab to offer central fabrication services to Midwest customers, which is
starting to grow. I offer patient care, fitting shoes, making custom orthoses
and Medicare billing. I have over the counter products for people who don't need
custom.
I saw the survey for pedorthists released a few months ago and I am nowhere
near that income: it was depressing to say the least! So, what is everyone else
doing to generate that kind of money? As I am trained in gait analysis, I
have been wondering if I should charge for this service to boost revenue. Any
suggestions?

Response: Unless you are providing significant Medicare
(or other insurance work), there are only two ways to increase you salary.
One is to diversify your business further. You must carry and sell more
fashionable comfort shoes. Items that the person with minor foot problems
will buy from you instead of your competitor. Also, you must check your
margins. The average pedorthists in business for themselves are not great
business people. They fail to realize the true value of the services that
they offer and either under charge or don't charge at all. In my area a
local pedorthist went out of business after twenty years. For example,
they did not charge when they built plastizote pieces into the arch of the
shoe. There are 10 or 15 dollars right there. May not seem like
a lot, but let's say you do 50 of types of things a month (less than two shoe
mods. a day). There are 500 to 800 dollars a month that you are
missing out on right there. Also, you may not realize that the material
that you use has gone up significantly in price but your prices haven't
changed.
Do these two simple things and you will do a
lot better than you think.

Response: Pedorthist income can not always be compared to
apples to apples, sometimes it is apples to oranges. Some pedorthist own
their own businesses and command a high salary, where others work for someone
else and receive a lower salary. Others' income is based on what part of the
country they live in.
Still others may work in a company that
uses them to perform tasks above what is expected of a pedorthist like
myself. I handle duties that an Orthotist would generally do. This type of
work brings in big dollars for the company. For example, if I measure and
deliver a custom LSO or TLSO back brace, the company bills around
$1,000 dollars and up. If I cast and deliver a custom AFO or a
bilateral set of AFO's the company can bill out anywhere from $400-$1500
dollars. A custom knee brace can bill out close to $1200-$1800
dollars.
You already know how many foot orthotics it takes to make
that kind of money. So if I am generating higher dollar sales, my salary will
reflect that.
The best thing to do in running your business is
charge a fair price for what you do, don't gouge your clientele, maintain good
relations with your doctors, and do whatever you can to lower your running
expenses, inventory, labor rate and so on. Also keep your working
staff happy and they will earn you more money too. Doing these things will help
you grow your bottom line.

Response: There are a few ways to increase your
$$$. Cut corners and don't tell the patient exactly what they can
get, give them what is profitable instead. Here are five ways
to increase your $$$, but my guess is you won't. Because
my guess is that you do things right.
A lot of pedorthists will make
custom inserts for everyone, need them or not. I know of one guy that
put arch cookies under a prefab and billed for K0629. (his company fired
him)
Very few pedorthists do follow-ups. The time it
takes to do a follow-up, is the same time it takes to see another patient.
And how many do a complete foot evaluation? If you
only measure and use an impression box, and that's it, you can see more patients
a day than a guy who does full foot eval for each patient.
Private insurance allows more than one pair of
shoes per year, and different amounts of inserts per pair. One
insurance we deal with allows two pair of shoes per year, but only one
pair of inserts per pair. On the other hand, there is an insurance that
allows up to $1,000 per year in any combination of shoes and inserts.
(i.e. 3 pair of shoes and 3 pair of inserts, or more
profitable, 1 pair of shoes and 5 pair of inserts. The Patient does
not know the difference, if you say this is what you get, they accept it.)
And lastly, buy cheaper shoes. I know a guy
that only goes to nursing homes and only offers 2 styles. Pedors stretch shoe
and a low-end sport shoe. Shoe cost is about $30. Discount Inserts
for about $40. Compare that to a company that sells quality shoes and
inserts for $125 or more. Profit, Profit, Profit.

Question: Does anyone know if one can provide one pair of
diabetic shoes without the 3 pairs of custom heat molded inserts? Two
examples would be: a) a patient has his own custom orthotics and cannot use
the plastazotes, or b) a patient still has few pairs left from previous
year and doesn't wish to receive more. When billing Medicare for A5500 only,
would the explanation be necessary why insets (A5512) were not
provided?

Response: Doctors will write
prescriptions all the time for just diabetic shoes. There is no stipulation that custom foot orthoses must be dispensed with
the shoes to be covered by Medicare or any other Insurance company.

PREVIOUS QUESTIONS WITHOUT
RESPONSES
Question: I had a question regarding
compression stockings. I know some of us are cross-credentialed and are
able to measure and dispense these items, but I'm curious as to what is
typically deemed as prescription strength. I typically don't dispense
anything above 10-20 mmHg without a prescription, but I was wondering if anyone
knows of the "proper" cut-off. In my mind, 20-25 mmHg is prescription
strength and not a retail item. But, one of the major insurance carriers
we work with doesn't reimburse for anything below 30mmHg. To my knowledge,
there are no regional differences for these items, any advice would be
appreciated.

when to the place I've been taking shoes for repair since I was much younger
than those boots.
I handed them over to Nick Camechis, owner of Capital Shoe Fixery, who
bothered to spend part of his weekend trying to figure out a way to regenerate
them. Finally, he said there was nothing he could do to save my boots, and for
his labor, creativity, time and generosity, Nick charged me precisely
nothing.
In some way, this business ethic must explain why Capital Shoe Fixery has
been in business now 70 years one of the few in our city that's endured so
long.

Experts agree that embarrassment and ways to prevent it are important
underlying motivations for social behavior - including purchasing decisions.
While it has many sources, issues related to personal appearance are at the top
of the list of its potential causes for most people.
In the field of foot care, podiatrists and other experts are acutely aware
that unattractive feet can cause significant emotional angst, driving those
embarrassed about their feet to seek a solution. Dr. Nicolas Romansky, a
Pennsylvania podiatrist, says that he commonly sees patients who hide their feet
out of embarrassment. He says, "There's a psychological overlay to foot
problems, especially with toenails."
Dr. Romansky says the distress resulting from unattractive toenails can be
so extreme, he's had married patients who refused to allow their spouse to see
their feet for years, sleeping and even having sex with socks on in order to
keep their feet hidden.

A new study shows tart cherries, one of today's hottest "Super Fruits," may
help reduce inflammation, potentially lowering the risk of type 2 diabetes and
cardiovascular disease in animals. The study was presented by University
of Michigan researchers today at the Experimental Biology annual meeting.
As science continues to reveal inflammation may be a marker for many chronic
diseases, the researchers say emerging studies like this are important in
examining the role diet may play in disease management and prevention.
At-risk rats with metabolic syndrome (obese, pre-diabetic) and lean,
healthy rats were fed a cherry-enriched "Western Diet," characterized by high
fat and moderate carbohydrate -- in line with the typical American diet.
Cherry-enriched diets, which consisted of whole tart cherry powder as 1 percent
of the diet, reduced two known markers of inflammation by up to 50
percent. TNF-alpha was reduced by 50 percent in the lean rats and 40
percent in the at-risk rats and interleukin 6 (IL-6) was lowered by 31 percent
in the at-risk rats and 38 percent in the lean rats.
