Question: Today's retail environment is being increasingly affected by consumers' preference for "green," or environmentally-friendly, products and companies. Some footwear companies have come up with "vegan" styles and other efforts at eco-friendly products. Are there any pedorthists who are brainstorming ways to make their businesses more "green?" Is there a way we can work together as an industry to help protect the planet?

PREVIOUS QUESTIONS WITH RESPONSES

Question: Recently, Cigna Government Services has been denying all our claims for toe fillers. The denial code is CO150 -"information submitted does not support this level of service." These denials started when Cigna started processing the claims, and we never had this problem when Palmetto GBA was the contractor. I have appealed these claims with medical documentation regarding the amuputations, but we are still getting the same denial. I have also reviewed the LCD and Medicare "news", and I do not see any specific benefit limitations for toe fillers. Does anyone know why these claims could be denying?

Response: Assuming you are using the RT or LT modifie, it appears Cigna is denying for too many services. It would be difficult to believe that this would be true for all your patients. Even for one, they would have to prove that there was a lack of medical necessity for an additional prosthesis, as L5000 do not come under the 5-year orthotic rule.

This sounds like a computer glitch. I would contact someone in the outreach and education department and ask them to look into this.

Response: When you bill, toe fillers are not covered unless you drop a set of foot beds. Otherwise you need to build a partial toe prosthetic and use code L5000. This will need an additional prescription and maybe a letter as well from the Doctor. If you get that then you bill for 3 A5513 and 1 L5000. Make sure you put which foot gets the partial prosthetic.

So either bill for A5500 plus 6 A5513

or bill for A5501 and make a custom shoe with toe filler included.

or bill for A5500 plus 3 A5513 and misc A code for each additional job

or bill for A5500 plus 3 A5513 and 1 L5000.

Technically when you do the diabetic program you are supposed to put Amputations in a custom shoe.

Question: Does anyone have a good way to collect deductibles and co-pays without scaring away our patients in the process? I know Medicare expects us to collect this money but I don’t want to scare off my clients or referral sources.

- Minerva

Response: The best way to collect deductibles and co-pays is to ask for this money prior to the service being performed. Gently but firmly inform your customer/patient that the law requires you to collect this fee. If compying with the law scares away your patients, then you have done yourself a favor.

Failure to attempt to collect co-payments and deductibles subjects you to both civil and criminal penalties. Therefore, if the patient goes elsewhere, that practitioner will assume the risk of fines, exclusion from Medicare, and possible jail time.

Response: Our policy is to collect the co-payments at the time of delivery. We have never had a referral source get upset about this as long as we are up front with the patient that this is what is expected. We have started giving written estimates about the cost of the service and what they will be expected to pay. This has eliminated the "memory problem" some people have when presented with a bill. My experience has been that the doctors are all concerned about getting their payments and understand that we also need to get paid. Most of our patients understand this and do not have a problem with it, either. Even those that have a problem with it still pay up front, and we have not seen a drop off in referrals or business. Response: Yes, I found a wonderful company, Green Flag; it’s extremely economical with a flat fee of about $13 an account no matter what the dollar amount to be collected. I have used this for years and have found it extremely beneficial. Green Flag takes care of collecting your money in a non-threatening way. Just last week I had a patient come into the office with a letter from Green Flag. He paid his balance and also handed me a prescription for more shoes. Response: Yes, Medicare wants us to collect 20% co-pay from our patients, and you must have some proof that you sent an invoice three times and there was no response. You can only do so much collecting this co-pay. If you have a ton of people that owe you the 20% co-pay and you have a hard time collecting, you can hire a collection agency to do it for you, but if it's only occasionally it would be just as easy to swallow it. One way of not scaring your clients is to perhaps tell them (preferably after they've been fitted) that your Medicare carrier will cover 80% for your shoes therefore your co-payment will be 20%. That way they know what to expect and there are no surprises later on when they get your bill. As far as the deductible goes.....most Medicare recipients are aware of their yearly deductable and that they must meet that first before their coverage kicks in. You can scare your patients with money talk only if you are scared yourself and don't believe in what you do.

Think about this: "it's not WHAT you say, it's HOW you say it". Hope this helps.

Response: Just be nice....we all have the same problem. Be very careful about waiving any co-pay. You must have great documentation of poverty. Otherwise you can hold the file and when the person meets their deductible you can call them later and offer the services. Or you can try and explain to the customer that they will have to pay the deductible somewhere along the way. Good luck.

Response: I use a system that really works well; I submitted 4 claims of overdue account through Jim at jim.hewlett@transworldsystems.com and within 5 weeks they were paid in full for a total amount collected of $3300.00. Jim’s service charges a flat fee, not a percentage, and cost me about $56.00 to collect $3300.00. They even give you a choice of how aggressive you want the collection process to be on each individual case. I use the soft approach with patients and a more firm approach with insurance companies. If you don’t want to alienate your patients or customers, I strongly advise you to try Jim’s service. I am glad I did. It definitely works. You can contact Jim at 530 229-0337 or 800 873-8005.

PREVIOUS COMMENT WITHOUT RESPONSES

Comment: I had previously written to advise that Blue Cross Blue Shield of Florida had changed their reimbursement practices to only pay for one pair of therapeutic shoes OR one pair of inserts but not both. I am pleased to report that effective February 15, 2008 they have revised their practices to reflect the same coverage as Medicare:

One pair of extra depth shoes and three pairs of inserts (direct mold or custom)

One pair of custom molded shoes and two pairs of custom inserts

In addition they have added the requirement that the provider must obtain certification of the patient's diabetic condition from the physician treating the patient. Of course, there may be policy exclusions under certain policies and generally speaking, you must be a network provider to bill for these services.

Aung FootHealth Clinics & Advanced Wound Care Center has reported that it is recruiting patients with lower extremity diabetic foot ulcers to participate in the Matrix clinical trial.

The Matrix (A Phase IIb Study of GAM501 in the treatment of diabetic ulcers in the lower extremities) trial is a randomized, double-blind, placebo-controlled, comparator arm (standard of care) safety and efficacy study to evaluate GAM501 that is topically administered once or twice over the course of the study. The Matrix trial will study the effects of Excellarate (GAM501) for the potential treatment of patients with neuropathic non-healing diabetic foot ulcers. The primary endpoint will be the effect of Excellarate on the incidence of complete ulcer closure at 12 weeks or earlier. Full Text

Stem cell transplant 'cures' diabetic mice

The prospect of using transplants derived from stem cells to reverse diabetes has come a step closer with the news that the technique seems to work in mice.

A team from Novocell in San Diego, California, told a stem cell conference in Evry, France, last week that this is the first hint therapies derived from human embryonic stem cells (hESCs) might cure type 1 diabetes. The only existing treatments for people with this autoimmune disease are regular insulin, or islet-cell transplants from cadavers. Full Text

Keep Your Feet Free of Pain, Injury

Most of the year, Chris Wheeler runs about 15 miles a week on a track in Webster. But with temperatures in the area in the single digits, Chris now hits the treadmill at the Midtown Athletic Club. He says the pounding is tough on his feet, but that was before he bought new sneakers.

"Basically, the arch of my foot would hurt," Chris said. "It would ache, and sometimes be within the first five minutes."

Podiatrists say common foot pain and foot injuries they see in walkers and exercisers include swelling, sprains, strains and even fractures. And most foot injuries all have the same cause. "It's usually all to do with the biomechanical aspect of the foot," said Edward Bonavilla, D.P.M. of Rochester Foot Care Associates. "Bearing weight in the wrong way or coming down hard on the wrong aspect that would cause an injury." Full Text
