Dear Friends,
This year we have reached a historic milestone at Unity Care – 15 years of serving our country’s most lonely and vulnerable youth – those in the Foster Care System. It’s quite overwhelming to imagine that in August of 1993 we opened our very first residential treatment home serving six boys, and today we now serve over 1000 youth and families annually across 18 programs in four counties.
As we pause and give thanks to all who have joined us in this journey, we must intensify our advocacy for those less fortunate and without voice. We will continue to push the envelope as we gear up for another 15 years of serving underserved communities in by fulfilling our mission of “providing quality youth programs for the purpose of creating healthier communities through life-long partnerships”.
Unity Care is proud to announce that we are expanding our geographical boundaries to provide Transitional Housing Placement Plus program services to emancipated foster youth in Orange County. Please read below for more details.
Lastly, we have some exciting program highlights from our Summer of Learning, Focus Media Project and Hip Hop 360 annual report below.
Best wishes,
André Chapman

THP+ Kicks Off in
Orange County
By utilizing a supervised, Scattered-Site THP+ Model, our participants live
independently in rented or leased apartments, single-family dwellings, or
condominiums at a subsidized rate. In addition to affordable housing,
Unity Care will provide comprehensive individualized case management services
for each youth. Through one-on-one meetings, staff will develop a
personalized plan tailored to fit individual circumstances in order to provide
the greatest possibility for success.
Prominent Hip Hop Rapper Supports Unity Care's Focus Media Project
Bay Area producer Traxamillion stopped by to support and help kick off Unity Care's summer Focus Media Project . Our youth had a chance to take pictures, receive personalized autographs and interact with the rapper.
Our fantastic facilitators are working closely with Unity Care's residential youth on several creative arts projects. The projects give our youth the opportunity and the tools to express themselves through artistic mediums such as video, art, photography and music.
Weekly sessions are held every Friday from 4:00 pm to 5:30pm until August 15th. Sessions are hosted at the Silicon Valley De-Bug community center located at 701 Lenzen Ave in San Jose.
In mid September, youth will showcase their hard work and efforts to create short documentaries, a magazine comprised of prose, poetry and photography as well as individual studio recorded music. More information regarding the finale presentation will be announced in the near future.
The Focus Media Project was made possible thanks to the generous support of our sponsors: The
Lucile Packard Foundation for Children's Health and the Arts Council Silicon Valley.
Youth Discuss
Health Disparities through Unnatural Causes Screenings
Thanks to the Health Trust, Unity Care will host screenings of
Unnatural Causes, a seven-part
documentary series exploring racial and social economic inequalities in health.
Several screenings will occur in July and August, which will educate, expose,
spark discussion and debate amongst our youth surrounding social determinants of
health. Youth in our Residential, Wraparound, THP+ and ILP programs will have the
opportunity to view and discuss episodes with our program managers.
After a recent screening of "In Sickness and in Wealth" a THP+ youth said, "Wealth is
obviously a key factor in determining ones health and well being because if you
are working long hours and are stressing out trying to figure out how you're
going to pay your bills, you're more likely to get sick."
Watch short Video Clips from the series below: To attend a public screening please
