Seasons Greetings Community Partners,

 

Welcome to Unity Care Group’s bi-weekly Community Partner newsletter. This month we're highlighting Hip Hop 360's fantastic fall activities as well as enrolling foster youth under 18 years of age in San Jose's RISE program. Additionally, we invite you to join us this Friday, December 21st for Higher Ground's second annual Holiday Fundraiser. Enjoy inspirational R&B, Rock and Hip Hop music at Red Rock Cafe in Downtown Mountain View. All proceeds will go towards making our foster youth's holiday season a brighter one.

 

Unity Care’s level 11 program in San Jose is accepting referrals for males age 11-17 and females age 11-17. Unity Care also has openings in our Transitional Housing Program ages 18-22. Please contact Neil Liebert, our Clinical Intake Manager at (408) 971-9822 if you would like to inquire about upcoming available beds in these or any other Unity Care program. What's Happening at Unity Care

Reaching Independence Through Sustainable Employment/Education (RISE)

RISE is currently recruiting high school senior foster youth 18 and under to participate in their highly beneficial and FREE Academic and Career Advising program.

Youth receive valuable one-on-one assistance with:

  • Finding the Right College

  • Picking the Right Classes

  • Filing for FAFSA (free cash for college!)

  • Applying for College Scholarships

  • Finding Paid/Un-Pain Internships

  • Getting Connected with the Independent Living Program

Additionally, youth receive numerous cash rewards such as, $25 for attending the workshop, $25 for attending the field trip, $50 for increasing reading/writing skills and $50 for graduating from High School!

Step-by-Step Referral Procedure

 

Unity Care Group accepts children (ages 6 to 12)
and youth (ages 13 to 21) into its residential programs by referral only.

 

REFERRAL PROCEDURE

  • Call Neil Liebert, the Intake Manager at (408) 971-9822 for bed availability and initial referral screening.

  • For referrals to Santa Clara and Monterey County residential programs, fax the referral packet to the Intake Manager at
    (408) 971-2255.

  • Referral packets should contain as much information about the youth, their family and current circumstances as possible, so we can make an accurate determination of their suitability for our residential program. It will also enable us to determine which of our programs will be most appropriate, and offer the best outcome.

  • We thoroughly review all packets for appropriateness of services as well as goodness of fit into specific homes.

  • We conduct face-to-face interviews with each youth once suitability is determined. Phone interviews may be conducted for youth who are located far away or when transportation is an issue.

  • Upon determination of acceptance, the placing agency is promptly notified.

  • Intake paperwork is faxed to the placing agency. The agency will complete and return the documents to Unity Care Group, Inc. before admission.

  • We arrange for and schedule transportation with the placing agency.

Hip Hop 360's Fall Updates!
 

Unity Care’s Hip Hop 360 after school program is back in full swing this fall with three different classes which enhance students’ creative abilities and correlate to California State Educational standards. Here’s a brief overview of what’s going on:

One Voice Youth Choir

Led by professional singer David Chaidez, Hip Hop 360’s One Voice Choir is back for its third semester at John J. Montgomery Elementary School. Students in the choir learn the basics of rhythm, harmony, and musical notation. The songs covered by the choir include classic contemporary tunes which makes the 8 week experience entertaining and engaging for all participants

Urban Art

The Hip Hop 360 Urban Art course is putting a positive spin on graffiti: an art form that is commonly viewed as negative. Instructor Ryan Arellano teaches students the basics of how to design their own graffiti letters. In addition to teaching the fundamentals of graffiti Ryan also teaches classical art concepts such as color theory. Students are given a clear explanation of the differences between legal and illegal graffiti and strongly discouraged from defacing school or public property. The graffiti class culminates with the students displaying their artistic pieces for their peers.

Breakdancing

 

Instructors Kevin Vu, Huynh, Raymond Mora, Ryan Arellano, and Leila Mccabe teach students the fundamentals of break-dancing in East San Jose at Elementary and middle schools. Students learn how to do essential individual moves as well as play breakdance themed games designed to improve their sense of rhythm and coordination. The Hip Hop 360 team is hard at work preparing our youth for performances which will take place at their respective schools at the end of the course.
