Wars and revolts in the Middle East, earthquakes, tsunamis, financial crises, job losses, political battles - these are strange, hard times, and while doing what we can to ease the troubles of those most hurt, we must also continue to focus on our birth work with mothers, babies, and families. In fact, in this country, it seems that in these hard times, we mustn't give up on birth. We might argue that as birth goes, so goes society. We have an opportunity to make birth better and less costly.

 

Cost-effectiveness and avoidance of unnecessary risk are higher priorities than ever, which may bode well for midwifery care, accessibility to acceptability of doulas, and close examination and rejection of many of the "sacred cows" in maternity care, which I don't need to list for you here. Our society can only benefit as birth becomes safer and more humane.

I've been a part of the birth reform scene for 43 years (!) now, and am shocked and dismayed to see what's happened to the objects of my life's work -- safe and satisfying births and bonding among new families. Here's a little news that I hope will enlighten you and give you hope, direction and renewed energy!

Save the Date! Ina May Gaskin Visits Seattle, May 5-8.

Ina May GaskinI'm happy to share that the Midwives' Association of Washington State is organizing a series of events in May with world-famous midwife, Ina May Gaskin.

Events include a 5K walk on International Day of the Midwife (May 5th), the Midwives' Association Spring Conference where I'll be speaking along with Ina May and other Seattle speakers on Friday, May 6, and a large
public lecture and birth fair at Town Hall in Seattle on Sunday, May 8.  
TAALKathon in April
I'm participating in a TAALKathon on April 29th between 6 and 7 pm (I think) Pacific time. http://www.taalk.org/  TAALK stands for Talk about abuse to liberate kids. I'll talk about the impact of abuse on the childbearing woman and how women can turn the trauma into triumph through childbirth.

"It took two of us to birth this baby (uphill, no less)!" Penny with Su-chen Kuo, leader of midwifery in Taiwan, at a doula training last August, where Kathie Lindstrom and Penny trained 60 doulas and 8 doula trainers!

Cochrane Review of Continuous Support During Childbirth
In February, a new Cochrane Review of continuous support during childbirth was published. I was pleased when I saw that doulas are effective people in improving obstetric outcomes. Here are links to various reports on the new Cochrane review, which now includes 21 randomized controlled trials with 15,060 women (including 4 newly added trials that compared trained support people from the woman's own social network (partner, family friends) with doulas and hospital staff. This review still finds improved outcomes with continuous support, especially the support of a doula:
  • Less intrapartum analgesia
  • Fewer operative deliveries
  • Less dissatisfaction with childbirth
The benefits of support were greater if. . .
  • Epidurals were not routinely available
  • Providers were not on hospital staff
  • A  new finding with this review is that doulas are also more effective in improving outcomes than people in the woman's social network (partners, family, friends)
