| Breakthrough Studies on Osteoarthritis Emphasize the Role of Weight Loss | |
 In a recently presented prospective study, weight loss in obese patients with knee osteoarthritis was shown to have beneficial structure-modifying effects on the cartilage in knees. This finding has enormous health implications and offers a ground-breaking conclusion - weight loss now becomes the only therapy ever shown to have beneficial effects on knee articular cartilage. No drug has ever been shown to have such a benefit. It only took a mean weight loss of 9% to make a meaningful, clinically important difference in cartilage structure. This resulted in a significantly lower rate of loss in cartilage thickness in the medial joint compartment when compared to patients with minimal weight loss or actual weight gain. Those with a greater weight loss also showed improvement in cartilage quality based on MRI tests. Another report from Paris followed 44 obese patients with knee osteoarthritis who underwent bariatric gastric surgery. They experienced an average 20% reduction in their BMI, and, at 6 months post surgery, the group's average osteoarthritis pain scores dropped dramatically. Serum levels of markers for collagen synthesis increased by 32% and markers for cartilage degradation declined by 36%. These findings emphasize the importance of weight loss in patients with osteoarthritis, especially when it involves the knees. | Benefits of Probiotics for Bacterial Vaginosis | |
 Evidence continues to mount in favor of the use of probiotics for treating medical problems and maintaining health. Probiotics are defined as live microorganisms that confer a beneficial effect when administered in adequate amounts to a person. There are different types of these bacteria and the varying forms can be helpful in different medical situations. Several mechanisms of action have been proposed for the potential beneficial effects of probiotics, including their contribution to intestinal microbial balance and their ability to inhibit the growth of pathogenic bacteria. Probiotics have been studied for use in many conditions, including bowel disorders and infections, vaginal and bladder infections, immune disorders, and pulmonary and upper respiratory infections. This month, we will focus on bacterial vaginal infections known as bacterial vaginosis, or BV. Millions of women suffer from bacterial vaginosis each year, a condition which causes discomfort, pelvic infections, increased susceptibility to HIV, and preterm deliveries. The triggers that lead to BV include hormone deficiency, sexual activity, and contraception. These transform the normal microbial environment in the vagina from one with a predominance of lactobacilli to one with coliform pathogens (bacteria from the colon). Until recently, the options for prevention and treatment of this common condition were limited to repeat courses of antibiotics. Since current literature suggests that it is an imbalance in the normal vaginal bacteria that influences the development of BV, the restoration of the normal vaginal flora with the use of probiotics has been proposed. The main microbe found to be beneficial for this purpose is the lactobacillus. Several sub-types of lactobacilli exist and proper treatment requires using the appropriate mix. Many studies have now been completed, and the results are nearly universally positive. In the great majority of cases, the women were cured - a result not commonly seen with the use of recurrent antibiotics. The lactobacillus seems to achieve this high cure rate through a variety of proposed mechanisms, including assisting in the maintenance of an acidic pH, and producing substances that both kill the "bad" bacteria and then block them from adhering to the vaginal mucosa and retarding their growth. It is very important for women with a persistent vaginal discharge or odor to come in for an examination. Studies have shown that many women who self-treat with over-the-counter products for vaginal yeast infections have misdiagnosed themselves. We are excited to be able to now offer our patients with BV a safe and effective long-term solution to a vexing medical problem. |

| Stress and Your Looks | |
 Women lead busy and stressful lives, a fact which can affect health in many ways. In terms of looks, your hair and skin can be greatly impacted. Stress can cause your body to stop growing your hair, sending it into the telegen, or resting, phase. This will lead to more and more hair loss. Other undesirable effects of stress include the production of more adrenaline, which then causes the skin to produce more sebum. This then leads to the production of more oil, which can cause pimples! Stress also results in premature aging of the skin by reducing levels of DHEA, the rejuvenating adrenal hormone. Women under stress can easily age 12 years beyond their chronological age. If you care about your health and your looks, work hard on reducing your stress. | Is Calcium a Friend or Foe? |
 A recent meta-analysis published in the British Medical Journal created quite a stir with its conclusion. It announced that using calcium supplements increases the risk of heart attacks! While this is technically the correct conclusion based on the data obtained from this review, what must be recognized are the great limitations of this study, a fact freely acknowledged by the study's researchers. The only subjects allowed into the review were women who did not take Vitamin D with their calcium supplements, nor were complex calcium supplements allowed. The only types of calcium permitted in the review were simple forms, predominantly calcium carbonate, along with a small amount of calcium citrate. No other forms of calcium supplementation were permitted, such as calcium hydroxy-apatite combined with calcium malate, as is supplied in our office. Additionally, the studies reviewed were not designed to evaluate the effects of calcium on the heart, and many of the patients' cardiac conditions were not evaluated prior to the start of the studies. It is well known that it is very important that adequate amounts of Vitamin D be taken along with calcium in order for the benefits of the calcium to be realized, and this study excluded all patients who used Vitamin D. It is also acknowledged that all calcium supplements are not the same and that calcium carbonate is a very poor choice. In this meta-analysis, the majority of the subjects used calcium carbonate. The logical conclusion from this meta-analysis is that all women should have their Vitamin D levels checked and be sure the levels are adequate prior to adding calcium. The calcium used as supplementation should be in a form most similar to calcium found in food, therefore, the best choice would be calcium hydroxy-apatite, and not calcium carbonate, and probably not calcium citrate. The very best way to obtain calcium is through the diet, and calcium supplementation should be taken at the end of meals when the stomach contains the most acid. Clearly, many women would be best served by eliminating inappropriate forms of calcium as supplementation and by having adequate levels of Vitamin D, which usually requires supplementation. | DHA Offers Brain Protection from Trauma | |
Traumatic brain injury remains the number one cause of death in persons under the age of 45 in the Western world. However, results of a study published in the journal "Neurotrauma" in July, 2010, may offer new hope to accident victims everywhere.
Though the research was conducted on rats, it showed that DHA (one of the Omega 3 fatty acids) protects neurons subjected to trauma and improves functional outcome. In the study, the rats were subjected to an acceleration injury to their heads and then were treated with varying doses of DHA. After 30 days, the rats underwent pathologic examination of their brains.
The rats treated with DHA showed significantly less injury in a dose response manner, including less amyloid precursor protein, a substance found in brains following trauma, and other protein markers of brain injury. The use of DHA in patients suffering a traumatic brain injury offers a safe, affordable, and readily available therapy to reduce permanent brain injury following brain trauma.
Our take on this is that not only should those injured receive DHA, but all persons should regularly be taking DHA, along with the other main Omega 3, EPA, to maximize health and brain function at all times. |
| Dr. Gersh Speaks to Doctors and Community Members | |
In August, Dr. Gersh gave a lecture to a full-house at the Lakeview Senior Center in Irvine on the various ways we can maintain brain function as we age. Many of the attendees requested a copy of the presentation's slide set, as they seriously wanted to follow its recommendations. Because of the overwhelming enthusiasm displayed, Dr. Gersh will give an evening seminar on the same subject at our office on September 27. All attendees can receive a copy of the slide set emailed to them.  Also in August, Dr. Gersh presented a Grand Rounds Lecture to about 150 doctors and nurses at Torrance Memorial Hospital. The title of the presentation was "Perimenopause - An Integrative Approach." A thorough review of this important phase in every woman's life was presented along with the conventional and holistic therapies available to deal with its myriad symptoms. Dr. Gersh welcomes all women who believe they may be experiencing the perimenopause to see her and learn all about the changes occurring in their bodies, the significance of these changes to their well-being, and what they can do to stay physically and emotionally healthy as they enter into the next phase of their lives. |

| The Doctor Is In |
As those of you who have seen me in the office over the past 3 years already know, I have been evolving into an Integrative Physician, reading voraciously and attending numerous courses in my quest to gain knowledge about this new, and critically needed, area of medicine. Being an Integrative Physician means believing that medical interventions should promote the body's own amazing innate ability to heal itself. The path taken by an Integrative Physician is to turn to the safest and most natural options first, followed by more invasive, riskier procedures and pharmaceuticals only when needed. My journey now leads me into the 2-year Fellowship program in Integrative Medicine at the University of Arizona. Founded by Dr. Andrew Weil, this program requires approximately 10 hours of course work each week, as well as 3 weeks on campus attending live seminars over the next 2 years. It is a very intensive, comprehensive course covering all areas of medicine, introducing us to many healing traditions outside of conventional Western medicine. Additionally, it deals with the use of lifestyle medicine, nutritional medicine, herbs, and supplements to help achieve optimal health. This is not simply the absence of disease, but the state of happiness and well-being one achieves only when the mind, body, and spirit are all in balance. I am excited and a little bit nervous when I consider the road ahead. Rest assured, however, that this commitment will have minimal impact on my office schedule and my availability to you. My hope is that over time I will become an even more knowledgeable physician, better able to serve your medical needs.
