New Group Application
Goodbye, Know Your Group Size form

The new group application is now available for use for 10/1/12 effective groups. With this group application you will no longer need the Know Your Group Size form. There is also new functionality with this form. The group can digitally sign the group application and submit it to us electronically, which makes for faster processing and turnaround times.
