| 26-50 Employers - We Want You! |

Effective immediately, we will accept the Employer Risk Questionnaire (ERQ) for groups with 26 to 50 enrollees.
- Submit the ERQ along with the two most recent renewals.
- Using Statements of Health (either the Coventry Health Care form or the universal form) will still get the best rate.
- Illustrative rates presume use of Statements of Health.
- Call your Account Executive to talk about the impact of using the ERQ instead of Statements of Health.
- Groups with 2 to 25 enrollees are still required to submit Statements of Health.
