- Does the group have fewer than 25 full-time employees?
- Do the employees have annual average wages of less than $50,000?
- Does the group pay at least 50% of the cost of the employee-only premium?

The new version of the form contains questions designed to elicit information as to the true size of the group. All quotes must be underwritten correctly as either a small or large group to ensure the appropriate inclusion of benefits compliant with Mental Health Parity. |

New Group Application
| | The Virginia General Assembly passed legislation effective July 1, 2010 that extends State Continuation coverage from 90 days to 12 months for groups with fewer than 20 employees. In response, Coventry Health Care filed a new Group Application with the Bureau of Insurance reflecting this change. As a reminder, when the Group Application is submitted, all fields should be complete. Please use this application for groups with August and September 2010 effective dates. |
