| Off-Cycle Renewals |

Although economic recovery is in the forecast, we realize that stabilization does not occur for every group at the same time. Some employers still find it necessary to make an off-cycle benefit change. Coventry Health Care can accommodate the request to do so, but would like to make our broker partners aware of our policy surrounding these changes.
Coventry Health Care will approach these benefit changes as an off-cycle renewal. We will underwrite the group for a new contract, considering changes to group size and employee demographics, as well as proposed benefit changes. Once the group has been underwritten, Coventry Health Care will propose a new 12-month contract to become effective on the first of the following month.
Those employers who have Section 125 benefit plans allowing for pre-tax premium deductions may find it prudent to seek the appropriate advice before proceeding with an off-cycle benefit change. |

Group Application - This form has been updated to add the option for groups with over 100 subscribers the opportunity to waive the pre-existing condition exclusion. Though this is the only change to this form at this time, we would like to take this opportunity to ask that Group Applications be complete when submitted. In addition to confirming the rates and benefits offered to our clients, these forms contain the annual group size survey necessary for Coventry Health Care to accurately report to the Centers for Medicare and Medicaid Services. Please begin use this form for any group with an effective date on or after March 1, 2010. Our website has been updated to ensure that this is the only version of the Group Application currently available in the Broker Tool Kit.

Enrollment/Health Statement Form - This form has been updated to include a more extensive health questionnaire. Further, many employer groups were using the Health Statement Form to process mid-year additions, changes, and terminations. The current version of the form addresses this practical usage and adds fields necessary to ensure that all appropriate information is captured for these changes. Please begin to use this form to request medically underwritten quotes for groups effective on or after April 1, 2010. Our website has been updated to remove the previous Health Statement Form and replace it with the recently approved form. |
