BROKER BONUS PROGRAM
Coventry Health Care is pleased to announce a new broker bonus program with the potential to earn you $50.00 per contract. To be eligible, simply sell a group between 5 and 99 subscribers with an effective date between February 1 and April 1, 2010.

NEW PLAN DESIGNS: CDHP Rx
Coventry Health Care is introducing three new plan designs. These designs, called CDHP Rx, blend elements of qualified high deductible health plans (QHDHP) and the consumer driven health plans (CDHP) we have offered for several years. In these plans, like other CDHPs, members have an in-network deductible of at least $1,000 that applies to all services, except for preventive services and services rendered at a PCP's office. In addition, like QHDHPs, the deductible applies to pharmacy benefits. However, unlike QHDHPs, the deductible does not apply to preferred generic drugs on Tier One of the Preferred Drug List. With these designs, employers can introduce the concept of consumerism to employees and assure that they continue to have access to a PCP and preferred generic drugs without the imposition of a high deductible. These plan designs will be available for new and renewing groups effective February 1, 2010 and after, and will be visible in benefitexpress in the coming weeks.
NEW ADDITION: Prevacid Over-the-Counter
Since Prevacid has become available over-the-counter (OTC), Coventry Health Care has added it to the list of OTC medications eligible for coverage through insurance benefits.  Members can now get a prescription from their providers for Prevacid OTC and pay a Tier One copayment at the pharmacy. NEW FORMS: Small Group Quote Request Form & Certified Employee List

Many of you are familiar with the Request for Medically Underwritten Proposal form previously made available. In an effort to streamline the process and make it easier for you to obtain a quote in a timely manner, we have revised and shortened this form. We hope you find this form to be serviceable and self-explanatory. Please feel free to send any feedback to your Account Executive. 

As another initiative in our quest to streamline our quoting process, we are introducing the Certified Employee List. When this list is submitted along with a business license, Coventry Health Care will not require W2s, W4s, or Schedule Ks, unless the information submitted conflicts with medical questionnaires received.

Both of these forms are available on www.southernhealth.com in the Broker Tool Kit under New Business Forms.
