CENTRAL ARIZONA CHAPTER   

Wednesday,  November 16, 2011  1:30 to 3:30 PM

 The November monthly forum will be held at United Healthcare, 3141 N. 3rd Ave., Phoenix, (in the Cafeteria Room).  Please note the date - due to the Thanksgiving holiday, the AALHA meeting has been moved up 2 weeks.

Amy's Assisted Living
 

In Oradea, Romania, I completed medical school while caring for my parents. When I arrived in the United States, I wanted to utilize my medical and care giving experience to help seniorAmys 1s in the community. My husband and I opened our home, Amy's Assisted Living, to our residents in 2002. My husband and I are dedicated to our extended family that we care for on a daily basis. No matter what we are in the middle of doing, we find ourselves interacting with our residents; whether it is to join in on their conversations or play a game with them.

 

Since opening Amy's Assisted Living, we are filled with great pleasure and satisfaction. Our daughter, Jennifer, enjoys interacting with our residents and going on family outings together.

 

In our home we offer 24 Amys 2hour RN and doctor services. We believe that our residents should have daily stimulation and activities, including birthday celebrations and holiday parties. Our home offers around the clock care. We welcome outside services, including home health and hospice.

                          Discharge Documentation

 

Documentation is important (and necessary) when discharging a resident to move to a higher degree of care, house rule violation(s), death, or other incidents that may occur. In many states, documentation of a discharge is law.

 

A Discharge Summary can assist in assuring that proper documentation has been completed. A Discharge Summary should include, in addition to the basic information on the resident, the following items:  

  • Special Instructions
  • Reason for the discharge
  • To whom the resident was discharged
  • How the resident was transported
  • If a responsible party was handling the discharge, who are they, and what is their relationship to the resident
  • Who has been provided a copy of the medications and med sheet
  • Has the resident (or responsible party) acknowledged:  medications and med sheet have been provided, Medicare and other ID cards have been provided, items in the safe have been returned, a copy of emergency information has been provided and resident's furniture has been returned.
  • A list of people who have been given a copy of the Discharge Summary
  • Signature of resident (or responsible party) and the employee handling the discharge, and date
