Welcome to the first edition of CIVHC's Voices on Value e-NEWS. This newsletter serves to inform, connect and promote Colorado and national efforts to move the needle on health care. My recent op-ed in the Denver Business Journal on the pending Supreme Court decision reflects the urgency with which Colorado must continue to move toward reaching the Triple Aim (better health, improved quality, cost containment). APCDfunding$4.5 Million in Funding Secured to Support All-Payer Claims Database

trust TCHF logoColorado's All Payer Claims Database (APCD) - a groundbreaking tool for measuring health care costs and utilization throughout Colorado - has achieved a major implementation milestone by securing $4.5 million in joint funding from The Colorado Trust and the Colorado Health Foundation.

 

The APCD is a secure, encrypted data warehouse that will include claims information from all commercial health insurers operating in Colorado, as well as Medicaid and Medicare.

Colorado is one of just seven markets nationwide selected by the Centers for Medicare and Medicaid Innovation (CMMI) for the Comprehensive Primary Care Initiative (CPCI). This landmark, four-year, multi-payer project aims to strengthen the primary care system while improving health care outcomes and controlling costs through a patient-centered medical home (PCMH) approach. Project launch is conditional on the signing of memoranda of understanding between CMMI and each participating payer.

PHPPioneerACOColorado's Physician Health Partners Chosen as Pioneer ACO to Improve Care, Reduce Costs

In January of this year, Physician Health Partners (PHP), a medical management company that serves as the strategic partner to two IPAs and two collaborative groups, was selected as one of 32 health care organizations nationwide to be a Pioneer Accountable Care Organization (ACO) by the Centers for Medicare and Medicaid (CMS) Innovation Center.

 

Denver-based PHP's network of 260 primary care physicians and 600 contracted specialists provide primary care for approximately 450,000 people across the seven metro counties, of which nearly 25,000 are covered by Medicare and are part of the ACO.

PalliativecarePalliative Care Best Practices Toolkit Now Available
binder coverIn 2010 and 2011, multiple members of CIVHC's Palliative Care Task Force worked together to update this step-by-step, comprehensive manual which serves as a guide to providing quality care for long-term care residents. The document addresses advance care planning and advance directives, pain and symptom management, spiritual and emotional issues, death practices and bereavement support for families and facility staff. A special section provides detailed guidelines for coordination of services with hospice care.

CIVHCconnectPayment Reform Goals and Statewide Metrics Established

CIVHC's workgroup progress on payment reform, statewide metrics and dashboards, and the All-Payer Claims Database was shared at the CIVHC Connect all-stakeholder meetings March 19th and 23rd.
 
Highlights include:   

 

I. An update was provided on CIVHC's progress towards the Payment Reform Midway Milepost goals for 2015 including an overview of our work on bundling, and collaborations around the Comprehensive Primary Care Initiative and Health Care Innovation Challenge. II. Statewide health care Metrics and Dashboards have been established to support building public will and to measure and demonstrate the key drivers to achieve Triple Aim goals. View all Metrics and Dashboards.

 

III. The All Payer Claims Database is currently collecting test data from 12 carriers plus Medicaid, and will release Tier 1 reports (aggregate data reports describing patterns such as the incidence and variation of targeted medical conditions, state and regional cost patterns, and utilization of services) late fall 2012.
