Standing Rule Revision re: Plagiarism in the Medical Record |
|
Please note the approved revision to Rule #8 of the Standing Rules and Regulations:
The attending physician shall be held responsible for the preparation of a complete medical record for each patient. Using computer technology to plagiarize any section of the medical record is unacceptable. This record shall include identification data; complaint; personal history; family history; history of present illness; physical examination; special reports such as consultation; clinical laboratory, x-ray and others; provisional diagnosis; medical or surgical treatment; pathological findings; progress notes; final diagnosis; condition on discharge; Medicare attestation form (if applicable), follow-up and autopsy report when available. No medical record shall be filed until it is complete except on order of the Medical Records Committee. Medical records shall be completed promptly and signed by the attending Medical Staff member within two (2) weeks following the patient's discharge. General after-care instruction will be given to the patient upon discharge and documented in the medical record. Such general instructions should include diet, medication and physical limitation.

Handwashing Compliance |
|
The most recent report indicates that physician compliance with handwashing procedures is down to 79%. The physician compliance rate had been 92%. Physicians are asked to recognize this "opportunity for improvement."

MEC Update |
|
Below is a summary of MEC discussions from the February 14, 2012, meeting:
- In preparation for the upcoming Joint Commission survey, the MEC reviewed the processes for OPPE (Ongoing Professional Practice Evaluation) and FPPE (Focused Professional Practice Evaluation).
- The QVR process is being reviewed and training to ensure consistent, objective QVRs is underway.
- The new cath lab has been licensed and is open.
- The MEC requested regular updates to the presentation on healthcare reform that was presented by Dan Gross in February.
- Value Based Purchasing statistics were reviewed and the MEC has requested information on best practice strategies.
- Volume has been high and the medical staff members were recognized for their efforts towards efficiency and timely discharges.
