· Dictated discharge summary medication list must match the patient's discharge medication list
· Documentation of LV assessment
· Documentation of reason, if patient is not on an ACE or ARB.

The Heart and Vascular Center at Sharp Grossmont is making preparations for the Heart and Vascular Conference 2011 on November 5-6 at La Costa Resort and Spa in Carlsbad.
