11th Hour Program Launched by SGH Center for Spiritual Care |

Offered through Sharp Grossmont Hospital's Center
for Spiritual Care, the 11th Hour Program provides
compassionate care and companionship to patients
during the final hours of life. Specially trained
volunteers provide a comforting presence for patients
and their loved ones, including vigil, readings, music,
ritual or prayer to offer support through this critical
transition.
Supported by employees and volunteers, the 11th
Hour Program launched hospital-wide on February 1,
2010. Services will initially be available from 8 am - 8
pm, with the goal to expand to 24-hour availability as
the program attracts more interest from volunteers.
Upon referral, the on-call 11th Hour volunteer will
arrive within an hour of the request and provide a four-
hour vigil with the patient and family. The vigil may
include reading, music, ritual, prayer or other
requested support that is particularly meaningful to the
patient and family as the patient makes their
transition.
Referral for the 11th Hour Program should be made
when a patient is on comfort care with a DNR status
and estimated to expire within the following 24 to 48
hours. If possible, it might be helpful to make the
patient/family aware of the service in advance.

The Standard Intrafacility Transfer Order Set (3198) is now available on the Grossmont Hospital order set page on Sharpnet. |
|
This single page form addresses most of the
common non-medication orders associated with
patient transfers and should be used along with the
Centricity Active Order Medication Summary printout.
Together, these two order sets will help ensure that all
patient orders are addressed upon transfer to a
different level of care.

Hospital
--Use the form for all patient transfers to a higher or
lower
level of care within the facility (not for
discharges/admissions, such as with TCU, Acute
Rehab or BHS)
*Remember, Sharp Healthcare policies
prohibit the use of
generic or inclusive orders such as "transfer patient
with same orders" when transferring to a different
level
of care. Use this form instead!

PLEASE REVISE YOUR CALENDAR!
The next General Medical Staff Meeting will be held on
Wednesday, March 17 (not Tuesday, March 16).
It's going to be a special program! Doctor
Matt Iseman (an MD trained at Princeton and
Columbia who then became a stand-up comedian)
will entertain the medical staff.
And, Dr. Bill
Pogue will be "clowning around."
The show will start at 6 pm, but wine and hors
d'oeuvres will be served at 5:30 pm.
