  • Documentation - Blood Use
  • A note from Jeffrey Marxen MD, Chair, Medical Records Committee:

    Physicians, and especially surgeons, please use the words "blood loss or hemorrhage" when justifying the need for blood in your progress notes. This will save medical records personnel time in filling out and inserting those pesky, but necessary, physician query forms regarding anemia and it will also save you time as there will be one less item to address and complete when you visit medical records.

    An inadequate progress note entry to justify giving blood, which would mandate insertion of a form for completion, would be: Post operative anemia - non complicated.

    An appropriate entry which means no form to fill out and a shorter more enjoyable time in medical records would be: Postop (shows the anemia was not present on admission) Anemia due to Acute Blood Loss (further classifies the type of anemia).

    With the remainder of the information in the chart, the medical records coding personnel will be able to determine whether it's a complication or in most instances not a complication.
