The Medical Records Committee, chaired by Jeffrey Marxen MD, continues to find inappropriate documentation in medical records. Please accept this as their reminder.

Direct comments in medical records regarding billing, reimbursement issues, or coding issues are inappropriate and should be taken up directly with the individuals or departments involved or, if the physician feels appropriate, in the form of a QVR.

Additionally, disparaging and/or demeaning written comments directed to medical records personnel regarding questions which they have posed with respect to coding issues in or outside the medical record are also inappropriate and sometimes unprofessional. Especially in this economic and medical/legal environment, we all need to be as helpful and positive as possible.
